OBJECTIVE: Primary hyperparathyroidism (PHPT) in children and adolescents is a rare condition. Awareness should improve in order to lower threshold for screening and allow intervention before serious and permanent sequelac occur.
METHODS: A retrospective analysis of 15 children and adolescents with PHPT (age <20 yr) seen in our clinic between 1993 and 2006.

The efficacy and safety of various modes of medical treatment for primary hyperparathyroidism (PHPT) in pregnancy is largely unknown. This report describes two cases of PHPT in pregnancy that were temporarily treated with the calcimimetic cinacalcet. The first case was diagnosed in the 31st week of pregnancy. The patient was asymptomatic and had an albumin-corrected total calcium level (Ca(corr)) of 3.24 mmol/l. Secondary hyperparathyroidism is a common complication of both chronic kidney and end-stage renal disease. Vitamin D appears to play a central role in the pathogenesis of this condition and active vitamin D replacement is usually a necessary component of its treatment. We describe two female patients, 66 and 36 years of age, with both primary hyperparathyroidism and chondrosarcoma. Case 1 had a chondrosarcoma of the right scapula, and case 2 had chondrosarcoma of the left proximal tibia. Both patients underwent surgical resection of their chondrosarcoma and subsequent parathyroid surgery. Records of patients undergoing parathyroidectomy at our institute in the period 1991-2003 were retrospectively analyzed. Pancreatitis was associated in six of 87 patients (6.8%) with primary hyperparathyroidism (PHPT). Pancreatitis was the presenting symptom in five patients, while it developed postoperatively in one case. All patients with a past history of pancreatitis had suffered two or more attacks. Little is known about genetic alterations in parathyroid hyperplasias in uremic patients. Although overexpression of cyclin D1 and inactivation of MEN1 gene are found in subsets of common parathyroid adenomas, specific oncogenes or tumor suppressor genes contributing to monoclonal uremic hyperparathyroidism have not yet been identified. We have performed 16 operations on 14 patients with primary hyperparathyoidism caused by a solitary parathyroid adenoma in our department between 1st jan. 1990-31 dec. 1999. In osteoporosis, the amount of mineralized bone is diminished, but the remaining bone is of normal quality. In rickets and osteomalacia the trabeculae are irregularly mineralized, a feature reflected in the X-ray image.
