The purpose of this prospective study of 90 consecutive patients is to assess the long-term durability of ThuVEP in patients with benign prostatic hyperplasia (BPH) and prostate volumes ≥ 85 ml. Ninety patients with prostates ≥ 85 ml underwent ThuVEP between 2008 and 2010 at our institution. Patient demographics and short-term and long-term follow-up were evaluated. Maximum urinary flow rate (Qmax), post-void residual urine (PVR), international prostate symptom score (IPSS), quality of life (QoL), complications, and PSA were assessed at follow-up. Median age at surgery was 71 (66-75.25) years.

Holmium laser enucleation of prostate (HoLEP) is being performed with increasing frequency as a minimally invasive alternative to transurethral resection of the prostate (TURP) for the surgical management of benign prostatic hyperplasia (BPH). HoLEP offers the advantage of use of normal saline for irrigation, instead of glycine which is utilized in TURP, decreasing the likelihood of fluid absorption and effects on serum electrolytes. We describe a patient who underwent HoLEP for BPH and subsequently developed non-ion gap metabolic acidosis and hemodilution associated with volume overload.

Optical fibers have been used as a minimally invasive tool in various medical fields. We derive the parametric equations for the geometric rays of a periodic orbit inside a confocal cavity. Based on the derived formula, we demonstrate a passively mode-locked solid-state laser with a low pulse repetition rate to obtain a pulse train traveling along zigzag multi-pass trajectories. We achieve a stable mode-locked pulse train with a pulse repetition rate of 18 MHz by designing the cavity to satisfy the dual-M trajectory. Furthermore, by precisely adjusting cavity mirrors under the same experimental setup, we can reach pulse repetition rates of 12 and 9 MHz for the mode-locked laser
