How Real-Time Case-Based Malaria Surveillance Helps Zanzibar Get a Step Closer to Malaria Elimination: Description of Operational Platform and Resources.

Testing and treating asymptomatic populations have the potential to reduce the population's parasite reservoir and reduce malaria transmission. Zanzibar's malaria case notification (MCN) platform collects detailed sociodemographic and epidemiological data from all confirmed malaria cases to inform programmatic decision-making. We describe the design and operationalization process of the platform and other malaria surveillance resources that are enabling Zanzibar's progress toward malaria elimination. The MCN platform consists of an interactive short message service (SMS) system for case notific

Timing of the Arterial Switch Operation in Premature Neonates < 2 kg With d-Loop Transposition of the Great Arteries and Intact Ventricular Septum-A Case Series.

There is a paucity of data available regarding operative timing and approach for d-loop transposition of the great arteries (dTGA) with intact ventricular septum (IVS) in premature infants. We reviewed our surgical experience in a case series of five premature infants (<37 weeks gestational age) with dTGA/IVS and birthweights <2.0 kg. All five infants underwent primary arterial switch operation (ASO) at a median postnatal age of 19 days and weight of 1.8 kg. None of the infants had major postsurgical complications or needed reoperation. All infants were discharged home on room air. We demonstr

Desflurane anesthesia compared with total intravenous anesthesia on anesthesia-controlled operating room time in ambulatory surgery following strabotomy: a randomized controlled study.

BACKGROUND: Ophthalmic ambulatory surgery is preferred to be performed under general anesthesia either by total intravenous anesthesia (TIVA) or by inhalational anesthesia to increase the patient comfort. However, anesthesia-controlled time (ACT) can cause increased non-operative operating room (OR) time which may adversely affect the ORs efficiency. This study was aimed to compare the ACT of desflurane with that of propofol-remifentanil in strabismus ambulatory surgery. METHODS: From November 2016 to December 2017, a total of 200 strabismus patients (aged 18-60 years old, and scheduled for e

Analysis of time of drug use according to needle and syringe program operating hours in Melbourne, Australia: Effects on individual-level needle and syringe coverage.

BACKGROUND: Restricted needle and syringe program (NSP) operating hours in Australia have been reported as a barrier to access for people who inject drugs (PWID). We explored the prevalence of drug use occurring outside NSP operating hours with a particular focus on its impacts on individual-level needle and syringe coverage. METHODS: Using data from 584 participants in a cohort of PWID in Melbourne, Australia, we analyzed time and day of drug use for heroin, methamphetamine and pharmaceutical opioids. We related this drug use to the typical operating times of Melbourne's fixed-site NSPs, cat

Impact of increasing operative time on the incidence of early failure and complications following free tissue transfer? A risk factor analysis of 2,008 patients from the ACS-NSQIP database.

BACKGROUND: There is a scarcity of externally valid data that investigate the utility of operative time, a common clinical parameter, as a predictor of free flap failures. Our aim was to assess whether prolonged operative time correlates with early flap failure following free tissue transfer in the acute care setting using the American College of Surgeons National Surgical Quality Improvement Program database. METHODS: The 2005-2011 American College of Surgeons National Surgical Quality Improvement Program databases were reviewed for encounters that entailed a free tissue transfer via a CPT a…

Natural orifice specimen extraction in robotic sigmoid resection for diverticulitis is associated with reduced postoperative pain and shorter operative time compared with transabdominal specimen extraction: a retrospective single-center study.

BACKGROUND: Natural orifice specimen extraction (NOSE) has emerged as a minimally invasive alternative to transabdominal specimen extraction (TASE) in robotic sigmoid resection for diverticulitis. However, comparative perioperative data remain limited. This study compared perioperative and postoperative outcomes of robotic sigmoid resection using NOSE versus TASE. METHODS: Fifty-two patients undergoing elective robotic sigmoid resection for diverticulitis between 2022 and 2025 were retrospectively analyzed, including 26 NOSE and 26 TASE procedures. Primary endpoints were operative time, posto…

The effect of operative time on complication profile and length of hospital stay in autologous and implant-based breast reconstruction patients: An analysis of the 2007-2012 ACS-NSQIP database.

BACKGROUND: It is a generally accepted relationship that an increase in operative time results in an increase in postoperative morbidity. However, few studies have investigated the minute by minute effect of increased operation time on outcomes in reconstructive breast surgery. The authors examined the association between operation time and postoperative outcomes for autologous and implant-based breast reconstructions. METHODS: Data used in these analyses were extracted from all plastic surgery procedures identified in the 2007-2012 ACS-NSQIP datasets. Logistic regression models were used to …

Seasonality and the COVID-19 shock in Greek road accidents (2010-2024): Interrupted time-series evidence and diagnostic forecasts for operational road safety planning.

OBJECTIVE: We analyzed monthly administrative data on road crashes and casualties in Greece for the period April 2010-June 2024 (N = 171), with the aim of supporting operational road safety planning. METHODS: We applied interrupted time series analysis (ITSA) to separate stable seasonal patterns from the impact of the Corona virus disease 2019 (COVID-19) shock. The main analysis is based on segmented regression (ITSA) with monthly dummies for seasonality, estimated using ordinary least squares (OLS) with Newey-West heteroskedasticity and autocorrelation consistent (HAC) errors, with quasi-Poi

SMART-LAMP: A Smartphone-Operated Handheld Device for Real-Time Colorimetric Point-of-Care Diagnosis of Infectious Diseases via Loop-Mediated Isothermal Amplification.

Nucleic acid amplification diagnostics offer outstanding features of sensitivity and specificity. However, they still lack speed and robustness, require extensive infrastructure, and are neither affordable nor user-friendly. Thus, they have not been extensively applied in point-of-care diagnostics, particularly in low-resource settings. In this work, we have combined the loop-mediated isothermal amplification (LAMP) technology with a handheld portable device (SMART-LAMP) developed to perform real-time isothermal nucleic acid amplification reactions, based on simple colorimetric measurements, a

Anti-adhesion Gel versus No gel following Operative Hysteroscopy prior to Subsequent fertility Treatment or timed InterCourse (AGNOHSTIC), a randomised controlled trial: protocol.

STUDY QUESTIONS: Does the application of anti-adhesion gel, compared to no gel, following operative hysteroscopy to treat intrauterine pathology in women wishing to conceive increase the chance of conception leading to live birth? WHAT IS KNOWN ALREADY: Intrauterine adhesions (IUAs) following operative hysteroscopy may impair reproductive success in women of reproductive age. Anti-adhesion barrier gels may decrease the occurrence of IUAs, but the evidence on their effectiveness to improve reproductive outcomes is sparse and of low quality. STUDY DESIGN SIZE DURATION: This multicentre, parall

[The area of conflict between the surgeon's duty to inform about diagnosis, prognosis, operation, complications, time schedule, alternative methods and the patient's right of self-determination].

The surgeon's duty to inform patients determines the indication to a therapeutic and/ or diagnostic procedure. Despite ongoing information made available by the professional associations, the complaints against surgeons providing treatment are on the increase. Only careful health education information with records kept of the course of treatment adopted will safeguard the doctor in charge from patients' claims for damages. Case law demands that the doctor put the patient in a position to understand what is happening to him or her and for him or her to be able to make a decision freely. The pat
