Otological surgery is performed under the operating microscope with special microinstruments. The introduction of new technical means such as the argon laser, equipped with a fiberoptic handpiece, and the sceeter microdrill have improved the results according to the statistic of Berne. An important aspect is the approach to the structures in the temporal bone. The classical retroauricular and endaural incisions are replaced whenever possible by the less traumatic transmeatal approach directly through the external ear canal without external skin lesion. Ear drum perforations and cholesteatomas

Surgical technics for bypassing ankylosis of the stapes in the management of otosclerosis gradually evolved; modifications were prompted by problems which were revealed by the ongoing audits carried out by earlier surgeons. Auditing of performance must remain an integral part of any otologist's work. The natural history of otosclerosis should be taken into account so that a patient may be fully informed of treatment alternatives available. Stapedectomy will remain a most effective operation which can be recommended with confidence, provided the surgeon has acquired the skill to perform it prop

PURPOSE: A variety of lesions could arise from the GG area, or extend into this region from adjacent sites. The management of perigeniculate lesions includes observation, surgery, and radiation, according to the nature, the size of the lesion, and the accompanying symptoms. Preliminary experiences on the exclusive transcanal endoscopic approach to the GG area have shown safety and feasibility avoiding of any postauricular incision, or brain manipulation. The experience from two referral centers on patients treated for a GG lesion with a totally endoscopic approach is herein reported.

BACKGROUND: Surgical management of otosclerosis is technically challenging with studies demonstrating that outcomes are commensurate with surgical experience. Moreover, experts apply less force on the ossicular chain during prosthesis placement than their novice counterparts. Given the predicted decreasing patient pool and the rising cost of human temporal bone specimens it has become more challenging for trainees to receive adequate intraoperative or laboratory-based experience in this procedure. As such, there is a need for a low-cost training model for the procedure. Here we describe such a

INTRODUCTION: Various couplers exist for the Vibrant Soundbridge implant to connect the actuator to a middle ear structure. While the surgical indication mainly distinguishes between purely sensorineural hearing loss and mixed hearing loss, and audiological indication ranges are designed accordingly, we believe that the biomechanical distinction between forward and reverse cochlear stimulation is more meaningful. To evaluate this, we apply all common couplers in temporal bone experiments and correct the measurements for adequate comparability.

The aim of the study was to assess the effect of CO(2) laser on stapes prostheses and measure the heat transmission to the vestibule in experiment model. CO(2) laser was applied on two types of prostheses with power settings (2 and 6W; 0.05 s). Transmission of heat to the 'vestibule' was measured using type K thermocouple and DC-80 data logger during application of the laser on prostheses using a training model of temporal bone. Application of the laser on stainless steel prosthesis did not have any effect on the structure of the prosthesis. The use of the laser on the fluoroplastic-wire pisto

PURPOSE: Bone-cement (BC) ossiculoplasty is one of the options to solve ossicular chain problems. Many authors reported successful results in the early or mid-follow-up period; however, there is no long-term result in the literature. We aim to evaluate long term results of BC ossiculoplasty.
MATERIALS AND METHODS: Forty-eight patients who underwent BC ossiculoplasty as incudostapedial re-bridging by the same surgeon were invited to evaluation. Postoperative otomicroscopic examination was performed. Pre-operative and post-operative audiological results after longer follow up and graft success

OBJECTIVES: Ossicular discontinuity may be complete, with no contact between the disconnected ends, or partial, where normal contact at an ossicular joint or along a continuous bony segment of an ossicle is replaced by soft tissue or simply by contact of opposing bones. Complete ossicular discontinuity typically results in an audiometric pattern of a large, flat conductive hearing loss. In contrast, in cases where otomicroscopy reveals a normal external ear canal and tympanic membrane, high-frequency conductive hearing loss has been proposed as an indicator of partial ossicular discontinuity.

The application of 3D stereoscopic imaging in surgery represents an innovative approach for anatomical measurements without the use of ionizing radiation or tools contacting the anatomy. Achieving precise intraoperative measurements is crucial in microsurgery, yet conventional methods often lack accuracy due to technical limitations in microscopic zoom lens systems. This study investigates 3D imaging within a digital microscope for stapedotomy, focusing on its accuracy and clinical applicability in selecting optimal prosthesis lengths. We present an optimized calibration scheme for stereoscopi

Active middle ear implants or implantable hearing aids are used to treat sensorineural or combined hearing loss. Their coupling to the middle ear structures has a large impact on the success of rehabilitation. Practical issues such as the coupling site, influence of middle ear status, and forward and backward excitation of the inner ear are discussed in the context of biomechanics. For this purpose, experimental studies, model simulations, and current literature data are evaluated. The explanations are intended to contribute to a better understanding of certain procedures in hearing rehabilita

Limited data is available concerning the safety of active middle ear implants (AMEI) during Magnetic Resonance Imaging (MRI). Measurements in temporal bones are the gold standard for preclinical assessment of device safety. In this study the coupling stability of an actuator as used in a fully implantable AMEI was determined in temporal bones. Eleven temporal bones were implanted with the actuator according to the manufacturer's surgical guidelines. The actuator was coupled on the incus short process as recommended for sensorineural hearing loss. Temporal bones were exposed 10 times to the MRI
