Distal radius fracture risk reduction with a comprehensive osteoporosis management program.

PURPOSE: To study risk factors associated with osteoporotic distal radius fractures and evaluate the effectiveness of the screening and treatment components of a comprehensive osteoporosis program. METHODS: We retrospectively identified a cohort of patients aged 60 years or older from a large health maintenance organization. For the period 2002 to 2008, information on age, race, sex, diabetes status, osteoporosis diagnosis, osteoporosis screening activity, medications dispensed, and fracture events, including distal radius, proximal humerus, and hip fractures were recorded. We compared demogr

Falls risk factors: assessment and management to prevent falls and fractures.

Falls and fragility fractures are common, dangerous, and important public health challenges. They are best understood as geriatric syndromes with close relation to frailty and other aging-related health problems. They are associated with many risk factors, in all health domains - physical, psychological, social, and environmental. At a population level, the challenge is to improve the health and well-being of all older people to reduce the incidence of falls. At a clinical level, the challenge is to assess the individual risk factors and apply evidence-based individually tailored, multifactori

Management of Group B streptococcal sepsis risk in well, term newborns.

AIM: To investigate how clinically well, term newborns at risk of early-onset Group B streptococcal (EOGBS) disease are currently managed in the United Kingdom (UK). METHODS: Review of guidelines of UK neonatal units. RESULTS: One hundred and twenty-five guidelines covering 157 neonatal units were received (71% of UK units), three of which were excluded from the review. We found great variation in every aspect for the management of EOGBS disease risk including the following: definition of risk factors; management of at-risk newborns; choice of antibiotics. CONCLUSION: Our findings highlight

[Management of high-risk hypertensive patients for prevention from cardiovascular events].

Hypertension is the one of the strongest predictors of cardiovascular events. Before the treatment of hypertension, the overall evaluation of cardiovascular risks in individual patients should be performed. For the evaluation, factors related to diabetes mellitus, metabolic syndrome, chronic kidney disease, cardiac complications, and hypertensive target organ damage are evaluated in addition to blood pressure to distinguish high-risk groups. Especially, diabetes mellitus, chronic kidney disease and cardiac complications including coronary heart disease, congestive heart failure, and left ventr

[Postoperative hypoparathyroidism: risk factors and out-patient management following thyroid resections].

BACKGROUND: Postoperative hypoparathryroidism is the most common complication following thyroid resection. Currently the data about the quality of out-patient management is inadequate. PATIENTS AND METHODS: Between 2003 and 2006 a total of 1,966 resections were performed and retrospectively analyzed. RESULTS: Of the patients 14% developed temporary hypoparathyroidism and permanent hypoparathyroidism was seen in 0.37%. The extent of resection and female sex were significant risk factors. The recommendation to wean calcium substitution was only performed in 18% of affected patients. CONCLUSIO

Trust in risk management: a model-based review of empirical research.

This review of studies of trust in risk management was designed, in part, to examine the relations between the reviewed research and the consensus model of trust that has recently emerged in other fields of study. The review begins by briefly elaborating the consensus views on the dimensionality and function of trust. It then describes the various models of trust that have been developed in the field of risk management, comparing them with the consensus approach. The findings of previous reviews are outlined, followed by a delineation of the open questions addressed by the present review, the …

Clinical risk management of Stevens-Johnson syndrome/toxic epidermal necrolysis spectrum.

Clinical risk management concedes that risk is inherent to all health-care processes. Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare but potentially life-threatening reactions to medications. Risk management should be considered prior to starting, during, and after therapy. Prior to starting therapy, risks that need to be assessed include any specific patient groups that may be at greater risk for the development of SJS/TEN. Gene testing is in place for Chinese and Thai patients who are going to be exposed to carbamazepine. During therapy, it is important to recog…

Practical issues in the management of low-risk gestational trophoblast tumors.

Using data primarily from Charing Cross Hospital in London, we examined the organization and funding of patients' care and follow-up, the value of second evacuations, the indications for treatment escalation and the results of treating patients with persistent low levels of human chorionic gonadotropin (hCG) following a molar pregnancy. In the U.K. system the total cost per patient treated is approximately $30,000. Second evacuations appear to have only a modest chance (18%) of benefit in patients with hCG levels over 5,000 IU/L. Outcome analysis of patients with low-risk gestational trophobla

Does concern-driven risk management provide a viable alternative to QRA?

This article discusses a concept of concern-driven risk management, in which qualitative expert judgments about whether concerns warrant specified risk management interventions are used in preference to quantitative risk assessment (QRA) to guide risk management decisions. Where QRA emphasizes formal quantitative assessment of the probable consequences caused by the recommended actions, and comparison to the probable consequences of alternatives, including the status quo, concern-driven risk management instead emphasizes perceived urgency or severity of the situation motivating recommended int…

[Mitral valve prolapse: identification of high-risk patients and therapeutic management].

Mitral valve prolapse shows a wide spectrum from a benign anatomic variant to a progressive disease with severe cardiovascular morbidity and mortality. Echocardiography is the most important tool for diagnosis and risk stratification. Predictors for high risk are significant thickening of mitral leaflet of > 5 mm ("classic" prolapse), moderate to severe mitral regurgitation and reduced left ventricular function. These patients have an increased risk of infective endocarditis, cerebral ischemia and sudden cardiac death. Because of the risk for the development of severe mitral regurgitation requ

Weight management for type 2 diabetes mellitus: global cardiovascular risk reduction.

Most patients with type 2 diabetes mellitus are overweight or obese, and the relation between obesity, especially of the visceral compartment, and the risk for developing diabetes is well recognized. Excessive adipose tissue is associated with insulin resistance as well as the increased expression of proinflammatory cytokines and prothrombotic factors, all of which contribute to elevating the risk for coronary artery disease (CAD). In particular, abdominal obesity, or excess visceral adiposity, has been linked to a cluster of risk factors (high blood pressure, hypertriglyceridemia, low levels

Allergic contact dermatitis in children: strategies of prevention and risk management.

Over recent years, allergic contact dermatitis in children has repeatedly been reported as a significant clinical problem. It is generally accepted that allergic contact dermatitis is rare in the first years of life, and with increasing age (by the age of 10 years) reaches the incidence seen in adults. As in adults, metals are one of the most common sensitizers in children, along with rubber chemicals and fragrances. The influence of fashion trends and lifestyle such as piercing, decorative skin paintings, the hype of natural remedies and cosmetics (e.g. tea tree oil) or the use of cosmetical

Stakeholder views on flood risk management in Hungary's Upper Tisza Basin.

With escalating costs of flood mitigation and relief, a challenge for the Hungarian government is to develop a flood mitigation and insurance/relief system that is viewed as efficient and fair by the many stakeholders involved. To aid policymakers in this task, this article reports on a recent study to elicit stakeholder views on flood risk management in the Upper Tisza Basin, including views on appropriate means of reducing losses and for transferring the residual losses from the direct victims to taxpayers or an insurance pool. This study is part of a project to develop an integrated approac…

Computer-aided risk management--a software tool for the Hidep model.

OBJECTIVE: The Hidep risk management model has been developed and tested in clinical settings with promising results, but a tool facilitating the work has been suggested. The aim of the present study was to create and evaluate a computerized tool capable of creating overviews of the oral health situation as well as identifying risk factors and at-risk patients. The system developed should also facilitate the clinical work, for example, by assisting the user with automatic calculation of suitable Hidep groups and selection and printing of relevant patient information letters. Risk factors in the surgical management of perforated duodeno-pyloric ulcer.

The election of the treatment in the pyloric-duodenal perforations pursues a double objective: to reduce the operative mortality and to reduce the risk of ulcerous relapse in the long run. The authors carry out a retrospective study of 100 pyloric-duodenal perforations treated in 12 years. Three risk factors of immediate mortality are demonstrated: the age, 70 years or older; elapsed time, equal or superior to 24 hours and the existence of a situation of preoperative hemodynamic shock. The global mortality of the series was of 12% and the index of relapses of 12%. The authors outline a therape

[Perioperative risk factors and anesthetic management of patients for carotid endarterectomy].

Data from the records of 142 patients for carotid endarterectomy at Chugoku Rosai General Hospital between 1983 and 1995, were evaluated concerning perioperative risk factors and anesthetic management. As a preoperative anesthetic risk, the incidence of hypertension was the commonest (76%), and there was a significant incidence of ischemic heart disease (18%). Fentanyl and isoflurane have been used for anesthesia recently and the patients were closely observed and cared in the intensive care unit postoperatively. In order to prevent cerebral ischemia during the occlusion of the internal caroti
