The influence of right anterolateral thoracotomy in prepubescent female patients on late breast development and on the incidence of scoliosis.

BACKGROUND: It is assumed that a right anterolateral thoracotomy for correction of simple congenital cardiac defects (ie, atrial septal defect) achieves more favorable cosmetic results than a standard median sternotomy. METHODS: Ninety-five patients, 72 with right anterolateral thoracotomy and 23 with median sternotomy, who had corrective transatrial operations when they were younger than 12 years of age were contacted by questionnaire. The mean follow-up time was 23.1 years. Of these, 61 patients (46 thoracotomy and 15 sternotomy) were investigated clinically. Volume differences of the breas…

[Closure of atrial septal defect via small lateral thoracotomies, partial and full length sternotomies].

Surgical closure of atrial septal defect is considered a low risk procedure. Various minimally invasive surgical techniques spread over the conventional full-length sternotomy. But the risk profile and complication rates of the minimally invasive closure of atrial septal defect are not revealed sufficiently. We present the results of atrial septal defect surgery via small thoracotomies: right anterolateral, right posterolateral, partial sternotomies and full-length median sternotomies. Ninety four patients underwent surgical closure of atrial septal defect via minimal approaches through the pe

Repair of atrial septal defect through a right posterolateral thoracotomy: a cosmetic approach for female patients.

BACKGROUND: Because the operation for atrial septal defect is considered a low-risk procedure, the cosmetic result has become an important issue. Principally for cosmetic reasons, anterolateral thoracotomy is frequently used for closure of atrial septal defect in young female patients. However, in anterolateral thoracotomy, the skin incision frequently crosses the future breast line, which may cause breast and pectoral muscle maldevelopment. METHODS: We review the long-term results of a consecutive series of 126 patients in whom the atrial septal defect was closed through a right posterolater…

The prognosis of patients with non-small cell lung cancer found to have carcinomatous pleuritis at thoracotomy.

Non-small cell lung cancer with carcinomatous pleuritis is considered to be a contraindication of surgical resection. The objective of this study was to clarify the prognosis of patients with non-small cell lung cancer in whom carcinomatous pleuritis was found at thoracotomy. A questionnaire survey on the survival of patients with carcinomatous pleuritis found at thoracotomy between January 1985 and December 1994 was conducted by the Japan Clinical Oncology Group. According to the data collected from 21 hospitals, 8813 patients with non-small cell lung cancer underwent thoracotomy, 284 (3.2%) …

[A case report of total removal of infected pacemaker leads with cardiopulmonary bypass through right thoracotomy].

A 61-year-old man with septicemia had four infected pacemaker leads, which were impossible to remove using simple traction method. He received CABG previously, and SVG anastomosed to LAD was patent. Redo median sternotomy had a possibility to make damage to SVG. Total removal of infected pacemaker was performed successfully with cardiopulmonary bypass through right thoracotomy.

Ligation of the thoracic duct without thoracotomy for the treatment of postoperative chylothorax: a newly designed surgical procedure.

A newly designed surgical procedure of ligation of the thoracic duct without thoracotomy for the treatment of postoperative chylothorax is presented. A case of chylothorax following resection of carcinoma of the oesophagus was treated successfully with ligation of the thoracic duct without thoracotomy. Its advantages were very prominent consisting of maintenance of the integrity of the thorax, less traumatization, no need of a drainage tube and shorter hospitalization, much easier to expose the thoracic duct than the conventional transthoracic approach. We can find no similar report in any pub…

[Thoracotomy as treatment for pneumothorax associated with pneumocystic carinii pneumonia in a patient with hemophilia A and the acquired immunodeficiency syndrome].

Pneumocystis carinii pneumonia is the most common respiratory infection in patients with the acquired immunodeficiency syndrome, and inhalation of pentamidine aerosol is currently used for secondary prophylaxis. A 16-year-old patient with hemophilia A and the acquired immunodeficiency syndrome had a spontaneous pneumothorax during inhalation of pentamidine aerosol for secondary prophylaxis of Pneumocystis carinii pneumonia. Tube thoracostomy and pleurodesis were done without success. Thoracotomy was done 27 days after admission. The patient tolerated the procedure well, and the postoperative c…

The influence of cell type and lymph node metastases on survival of patients with carcinoma of the lung undergoing thoracotomy.

We studied 202 cases of bronchogenic carcinoma treated surgically between January 1, 1966 and December 31, 1970. Over all, adenocarcinoma was the most common cell type (36.1 per cent). Of 151 patients whose carcinomas were successfully resected, and who lived for at least 30 days postoperatively, 88 had lymph nodes free of cancer. Not surprisingly, 5-year survival was related to lymph node metatases and cell type. The best over-all 5-year survival rate was for large cell carcinoma; it was 52.0 per cent without nodal involvement. Similar figures for epidermoid carcinoma were 29.0 per cent over

Post-thoracotomy intercostal block: comparison of its effects on pulmonary function with those of intramuscular meperidine.

Study of 34 patients who had undergone thoracotomy revealed that the group given intercostal nerve block analgesia had a significantly smaller decline in vital capacity after operation than did the group given narcotic analgesia only. The postoperative increase in arterial CO2 tension of the nerve block group also was significantly smaller than that of the narcotic group. The study suggests that intercostal nerve block for post-thoracotomy analgesia offers some advantage in preserving effort-dependent pulmonary function when compared with postoperative narcotic analgesia.

From Thoracotomy to Robotic Platforms in Thoracic Surgery: Technical Evolution, Simulation-Based Training, and Surgical Outcomes.

Thoracic surgery has progressively moved from open thoracotomy to video-assisted and robot-assisted approaches, reducing access-related trauma while changing the technical environment in which pulmonary resections are performed and taught. This narrative review examines the clinical evolution of open, video-assisted thoracic surgery (VATS), and robot-assisted thoracic surgery (RATS), with particular attention to the evidence supporting their perioperative and oncological outcomes, the learning curves associated with minimally invasive surgery, and the role of simulation-based training. Randomi…

Left Ventricular Remodeling After Total Coronary Revascularization via Anterior Thoracotomy Versus Conventional Coronary Artery Bypass Grafting.

Total coronary revascularization via anterior thoracotomy (TCRAT) enables complete anatomical revascularization without sternotomy; however, data on its impact on left ventricular function remain limited. This study compared left ventricular functional outcomes between TCRAT and median sternotomy coronary artery bypass grafting (MS-CABG) in 554 patients undergoing elective isolated CABG at four centers (January 2020-January 2025) with preoperative and ≥3-month follow-up echocardiography. Patients were grouped as TCRAT (n = 241) or MS-CABG (n = 313). Stabilized inverse probability of treatment …

Central Aortic Cannulation in Minimally Invasive Cardiac Surgery via Right Thoracotomy: A Single-Center Retrospective Comparison.

Background/Objectives: Minimally invasive cardiac surgery via right thoracotomy commonly uses femoral arterial cannulation for cardiopulmonary bypass (CPB), which requires an additional groin incision and may be associated with access-related complications. Central aortic cannulation through the same thoracotomy allows antegrade perfusion without an extra incision. This study compared central aortic and femoral arterial cannulation strategies in minimally invasive cardiac surgery via right thoracotomy. Methods: This retrospective, single-center study included 139 consecutive patients undergoin…

Right thoracotomy approach for the management of uncommon type II endoleak after thoracic endovascular aortic repair.

An 80-year-old male with a ruptured pseudoaneurysm after aortic dissection repair underwent thoracic endovascular aneurysm repair. The aneurysm expanded from 43 mm to 89 mm over 4 years because of type II endoleak, with computed tomography angiography revealing a patent right intercostobronchial trunk and left bronchial artery on the lesser curvature of the aortic arch. These arteries were successfully dissected via right thoracotomy. During the 2-year postoperative period, the aneurysm remained stable at 80 mm. Open surgical management of the culprit vessel may be effective for type II endole…

Minimally invasive surgery via bilateral thoracotomy for treating left ventricular aneurysm with concomitant ventricular septal rupture.

BACKGROUND: Full median sternotomy is the traditional approach for the treatment of left ventricular aneurysms (LVA) with or without concomitant ventricular septal rupture (VSR). However, it has some disadvantages such as reduced breathing and exercise thoracic stability, which may increase the surgical risk for older or fragile patients. Herein, we report a case of successful minimally invasive bilateral thoracotomy. PATIENT AND METHOD: A 79-year-old male patient visited our center complaining of breathlessness and orthopnea 21 days after an acute myocardial infarction and percutaneous coron…

Minimally Invasive Surgery through Right Mini-Thoracotomy for Mitral Valve Infective Endocarditis: Contraindicated or Safely Possible?

Background: Mitral valve infective endocarditis (IE) still has a high mortality. Minimally invasive mitral valve surgery (MIMVS) is technically more challenging, especially in patients with endocarditis. Here, we compare the early postoperative outcome of patients with endocarditis and other indications for MIMVS. Methods: Two groups were formed, one consisting of patients who underwent surgery because of mitral valve endocarditis (IE group: n = 75) and the other group consisting of patients who had another indication for MIMVS (non-IE group: n = 862). Patients were observed for 30 postoperati
