Providing Ethical and Humane Care to Hospitalized, Incarcerated Patients With COVID-19.

The COVID-19 pandemic is devastating the health of hundreds of thousands of people who live and work in U.S. jails and prisons. Due to dozens of large outbreaks in correctional facilities, tens of thousands of seriously ill incarcerated people are receiving medical care in the community hospital setting. Yet community clinicians often have little knowledge of the basic rights and ethical principles governing care of seriously ill incarcerated patients. Such patients are legally entitled to make their own medical decisions just like non-incarcerated patients, and retain rights to appoint surrog

COVID-19 and the Nigerian correctional service: need for structured data.

Although prisoners are considered a vulnerable population, no data repository currently exists to monitor the COVID-19 incidence in Nigerian prisons. To better understand the impact of COVID-19 within the Nigerian prison system, prisons should develop detailed COVID-19 response protocols, implement enhanced point-of-care testing, and initiate contact tracing with meticulous data collection.

Caring for Youth in Juvenile Detention Centers: A Story of Hope.

During the past 3 years I have been visiting youth detention facilities to provide instruction on basic life skills. Two events discussed here encouraged me to get involved to help make a change. During my visits I continue to be struck by the discouragement and hopelessness of each of the youth. It touches my heart. I encourage them not to feel sorry for themselves and tell them they are not alone; there are people who want to help them. I emphasize that their lives can change, that they can have hope.

ACOG Committee Opinion No. 518: Intimate partner violence.

Intimate partner violence (IPV) is a significant yet preventable public health problem that affects millions of women regardless of age, economic status, race, religion, ethnicity, sexual orientation, or educational background. Individuals who are subjected to IPV may have lifelong consequences, including emotional trauma, lasting physical impairment, chronic health problems, and even death. Although women of all ages may experience IPV, it is most prevalent among women of reproductive age and contributes to gynecologic disorders, pregnancy complications, unintended pregnancy, and sexually tra

Accuracy of diagnostic mammography at facilities serving vulnerable women.

BACKGROUND: Breast cancer missed on diagnostic mammography may contribute to delayed diagnoses, whereas false-positive results may lead to u … false positive rates than at facilities serving nonvulnerable women. This may reflect concerns that vulnerable populations may be less likely to follow-up after abnormal diagnostic mammography or concerns that such populations have higher cancer prevalence.

Residential mobility and persistently depressed voting among disadvantaged adults in a large housing experiment.

This study examines the impact of residential mobility on electoral participation among the poor by matching data from Moving to Opportunity, a US-based multicity housing-mobility experiment, with nationwide individual voter data. Nearly all participants in the experiment were Black and Hispanic families who originally lived in high-poverty public housing developments. Notably, the study finds that receiving a housing voucher to move to a low-poverty neighborhood decreased adult participants' voter participation for nearly two decades-a negative impact equal to or outpacing that of the most ef

Trauma-Informed Principles in Practice: A Mixed-Method Study of Co-Producing Systems Change With People Who Have Experienced Multiple Disadvantage.

BACKGROUND: Health and social care services increasingly recognise the value of involving people with lived experience in service design and delivery. For people who have experienced multiple disadvantage (combinations of homelessness, mental ill health, addiction, involvement with the criminal justice systems or domestic violence/abuse), participating in professional settings may be challenging and can risk re-traumatisation and greater disillusionment. However, gaining confidence and competency to do this offers opportunities for personal and professional development, contributing to meaning

Co-producing data-intensive research with an underserved group: a case study and evaluation identifying pathways to impact.

INTRODUCTION: Co-production of research, where researchers and experts by experience work as equal partners throughout a research project, can improve the quality, relevance, implementation and impact of research. However, there is limited evidence on methods for successful co-production in data-intensive research with underserved groups. In partnership with the charity Voice of Young People in Care (VOYPIC) and a group of care experienced young people, the Administrative Data Research Centre Northern Ireland (ADRC NI) piloted and evaluated a co-production approach in a research project that u

Social Media Recruitment as a Potential Trigger for Vulnerability: Multistakeholder Interview Study.

BACKGROUND: More clinical studies use social media to increase recruitment accrual. However, empirical analyses focusing on the ethical aspects pertinent when targeting patients with vulnerable characteristics are lacking. OBJECTIVE: This study aims to explore expert and patient perspectives on vulnerability in the context of social media recruitment and seeks to explore how social media can reduce or amplify vulnerabilities. METHODS: As part of an international consortium that tests a therapeutic vaccine against hepatitis B (TherVacB), we conducted 30 qualitative interviews with multidiscip

Towards an Accessible Speech-based Obstructive Sleep Apnea Screening Tool for Underserved Populations.

Obstructive sleep apnea (OSA) is a chronic respiratory disorder characterized by recurrent interruptions in breathing during sleep. OSA is highly prevalent, affecting 30-70% of people with chronic conditions like hypertension and substance use. The gold standard for clinical OSA diagnosis is the polysomnography (PSG) test, which is a rather cumbersome and expensive procedure, and accordingly can be quite inconvenient for patients. Additionally, patients often have to wait for a long time before they can undergo PSG. As a result, other alternatives for screening OSA have gained attention. For i

Factors associated with vaccine adherence among an underserved population: the adult Travellers in Nouvelle-Aquitaine, France.

BACKGROUND: A measles epidemic affected the Nouvelle-Aquitaine region from November 2017 to May 2018 with clusters among Travellers. This indicates that measles vaccination rates among Travellers remain lower than in the general population. The objective of this study was to estimate the 'declarative vaccination' against measles, mumps and rubella (MMR) and to propose a conceptual framework to help identify determinants of MMR vaccination uptake among adult Travellers in Nouvelle-Aquitaine in 2019-20. METHODS: A cross-sectional study using random sampling was performed and included 612 adult

Community-based approaches to improve tuberculosis services: observations from preintervention and postintervention surveys in a high TB burden disadvantaged community in India.

An alarmingly high prevalence of tuberculosis (TB) was reported among the Saharia tribe in Madhya Pradesh, India. A community-based intervention study was undertaken to improve TB case finding during 2018-2021. The interventions mainly comprised active case detection through village TB volunteers using advocacy, communication and social mobilisation activities. A preintervention and postintervention survey design was adopted to assess the impact of intervention. The prevalence declined from 1357 (95% CI 1206 to 1527) to 752 (95% CI 646 to 875) per 100 000 population (p<0.001). The study findin

Latent Tuberculosis Infection Treatment Outcomes in an At-Risk Underserved Population in Rhode Island.

OBJECTIVES: Within the United States (US), significant racial and ethnic disparities exist in the rates of latent TB infection (LTBI) and active TB disease. A disproportionate number of TB disease cases result from untreated LTBI among individuals born outside the US. This study evaluates LTBI treatment outcomes among an underserved, at-risk population in Rhode Island. METHODS: A quantitative retrospective chart review of adult patients with a positive screening test assessed LTBI care cascade outcomes including referral, treatment initiation, and completion. RESULTS: Seventy-four percent of
