Placental Role in Fetal Neonatal Neurologic Outcomes.

Neuroplacentology explores the intricate relationship between the placenta, fetal brain development, and long-term neurologic outcomes. As the primary interface between mother and fetus, the placenta regulates the transfer of nutrients, oxygen, and waste products essential for growth and development. Disruptions in placental function and maternal conditions, such as preeclampsia and intrauterine growth restriction, can lead to preterm birth and brain injury. This neuro-placentology review highlights the impact of placental dysfunction on prematurity, hypoxic-ischemic encephalopathy, and congen

Fetal hydrocolpos leading to Pierre Robin sequence: an unreported effect of oligohydramnios sequence.

The presence of distal atretic vagina causing accumulation of fluid and mucus secretions in the proximal vaginal cavity resulted in fetal hydrocolpos. Obstructive uropathy developed gradually because of direct compression of hydrocolpos on bilateral lower ureters, resulting in oligohydramnios from decreased urine formation. Oligohydramnios inhibited normal mandibular development with resulting cleft palate and glossoptosis (Pierre Robin Sequence). The development of sequence of events in this case indicates Pierre Robin Sequence as another effect of Oligohydramnios Sequence arising out of defo

FAS/FAE: focusing prevention on women at risk.

Consumption of alcohol during pregnancy is well recognized as a risk factor associated with adverse fetal development. While precise safe or dangerous levels of maternal drinking have not been identified, it is clear that the women who drink most heavily are at the greatest risk. Prevention of alcohol-related birth defects requires development of programs directed to the special needs of addicted women and their families. The nature of addiction suggests that direct interventions focused on changing individual drinking behavior have the best chance of success.

[Current problems of alcoholic embryo- and fetopathy].

The authors consider the least studied questions of alcoholic embryo- and fetopathy in the mechanism of the teratogenic action of alcohol on the development of the embryo and fetus and emphasize the importance of the time of damage, genotype, and additional effects of the environment. On the basis of analysis of the literature and their own data the authors have identified the most important manifestations of the fetal alcohol syndrome at different stages of the child development. The questions of the differential diagnosis are also discussed.

PBPK Modeling Approach to Predict the Behavior of Drugs Cleared by Metabolism in Pregnant Subjects and Fetuses.

This study aimed to develop a physiologically based pharmacokinetic (PBPK) model that simulates metabolically cleared compounds' pharmacokinetics (PK) in pregnant subjects and fetuses. This model accounts for the differences in tissue sizes, blood flow rates, enzyme expression levels, plasma protein binding, and other physiological factors affecting the drugs' PK in both the pregnant woman and the fetus. The PBPKPlus™ module in GastroPlus® was used to model the PK of metoprolol, midazolam, and metronidazole for both non-pregnant and pregnant groups. For each of the three compounds, the model w

Elective laparoscopic fetal laser photocoagulation in twin-twin transfusion syndrome: a case report.

Twin-twin transfusion syndrome (TTTS) is a complication of multiple gestation in which arteriovenous vascular communications occur in a shared placenta. Due to these communications, there is an imbalance of blood flow between the developing fetuses. This results in 1 twin becoming the donor and the other becoming the recipient. In severe cases, the recipient may experience polycythemia, polyhydramnios, and hydrops fetalis, while in the donor oligohydramnios and severe anemia may develop. It has been reported that fetal mortality can reach as high as 60% to 80% if TTTS develops before 26 weeks'

Placental mitochondrial DNA mutation-related oxidative stress underlies subsequent risk of adverse prognosis in sFGR twins.

BACKGROUND: Selective fetal growth restriction (sFGR), a serious complication of monochorionic diamniotic (MCDA) twin pregnancies, typically results from unequal placental sharing, especially in type II sFGR. This condition is associated with an increased risk of intrauterine death and adverse neurological outcomes in one of the twin fetuses. Mitochondria play an important role in fetal development and mitochondrial DNA (mtDNA) mutations have been implicated in the pathogenesis of singleton FGR. Mutations in the mtDNA can lead tomitochondrial dysfunction, oxidative stress, and apoptosis. Howev…

Use of the angiogenic biomarker profile to risk stratify patients with fetal growth restriction.

BACKGROUND: Novel angiogenic biomarker profiles have demonstrated emerging evidence for predicting preeclampsia onset, severity, and adverse outcomes. Limited data exist in screening patients with fetal growth restriction for preeclampsia development using angiogenic biomarkers. OBJECTIVE: The objective of this study was to risk stratify patients with fetal growth restriction using a soluble fms-like tyrosine kinase-1 to placental growth factor ratio. Previously published cutoff of 38 was used to predict preeclampsia development and severity as well as adverse maternal or neonatal outcomes wi

Pravastatin plus L-arginine prevents adverse pregnancy outcomes in women with uteroplacental vascular dysfunction.

BACKGROUND: Uteroplacental vascular dysfunction, characterized by diminished uterine artery (UtA) blood flow in the second trimester is a clinically useful predictor of the further development of preeclampsia, fetal growth restriction and stillbirth. Efforts to develop effective treatments to protect pregnancies with abnormal UtA Dopplers would be of significant clinical benefit for mothers and their fetuses. OBJECTIVE: The aim of this pilot non randomized control study was to use pravastatin +L-arginine to improve uteroplacental haemodynamics and prevent adverse maternal and neonatal outcome

Nanoplastics at the Placenta-Fetal Interface: Emerging Chemical Toxicology Concerns.

The interaction of nanoplastics with trophoblasts can potentially disrupt fetal development. The additives associated with nanoplastics, their surface chemistry, and protein corona may lead to oxidative stress and affect the cellular functions. Detailed studies revealing a clear mechanism using an advanced placental model is essential to assess developmental toxicity and guide risk assessment.

[Ganglioneuroma in a newborn with early inborn syphilis].

A case is reported of a retroperitoneal sympathic nodes ganglioneuroma in a newborn with classic early inborn syphilis involving practically all systems of the viscera, skin, placenta. Some peculiar morphological features of the tumour are described. Possible teratogenic and oncogenic effects of treponema pallidus on embryonal fetal structures during intrauterine development are suggested.

[Origin of Hematopoietic Stem Cells in Bone Marrow--Endothelial to Hematopoietic Transition (EHT)?].

In contrast to primitive hematopoiesis, during embryonic definitive hematopoiesis, it has been demonstrated that multilineage hematopoietic stem/progenitor cells (HSPCs) arise from hemogenic endothelium, and the endothelial to hematopoietic transition (EHT) exists within the yolk sac, placenta, AGM, mouse head vascular and extraembryonic vessels. However, whether hemogenic endothelial cells contribute to blood cell development at other sites of definitive hematopoiesis, including fetal liver and bone marrow, remains largely unknown. Recently, more and more researches showed that hematopoiesis

[Risks of surgery and anesthesia and optimization of anesthesiological maintenance of abdominal delivery].

The paper deals with abdominal labor pain relief in pregnant women at high risk for impaired development of the general adaptation syndrome: significant sympathicotonia, placental ischemia-reperfusion syndrome, and systemic inflammatory response syndrome. All these syndromes are attended by either impaired oxygen transport or a high risk for the occurrence of such at surgery. The detection and correction of these impairments in development of the general adaptation syndrome in pregnancy, which are provided by the author's modified rating scale for risks of surgery and anesthesia, contribute to

The recognition and management of intrapartum fetal heart rate emergencies: beyond definitions and classification.

The ongoing and well-documented debate about the value of electronic fetal monitoring has focused primarily on the fact that most variant fetal heart rate (FHR) patterns are poor predictors of fetal acid-base status. Most recently, much of this attention has been focused on the implications for clinical management of FHR patterns that the National Institute of Child Health and Human Development has classified as indeterminate: neither normal nor abnormal. Given that a majority of fetuses will have an FHR pattern considered indeterminate at some point in labor, this is an important and worthwhi

[Toxoplasmosis in pregnancy. Prevention, diagnosis, and therapy].

Toxoplasmosis is a worldwide health problem. Infection of a pregnant woman can result in severe fetal morbidity or in subclinical neonatal infection; most subclinical cases will develop ocular and neurological sequelae. Fetal infection and clinical outcome is related to when in pregnancy toxoplasmosis was acquired. The risk of transmission increases from 14% in the first trimester to 29% in the second and 59% in the third. Conversely, clinical damage decreases from about 80% in the first to 10% in the third trimester, but up to 50% of patients with subclinical congenital toxoplasmosis will dev

Consequences of congenital Zika virus infection.

The 2015 Zika virus (ZIKV) epidemic in the Americas led to the discovery that ZIKV causes congenital abnormalities including microcephaly, intrauterine growth restriction, and eye disease that can result in blindness. Studies in animal models and human organoid cultures, together with human epidemiological studies, have shown that ZIKV crosses the placenta and subsequently replicates within fetal tissues including the developing brain. Preferential infection of neural cell precursors causes damage to the developing fetal brain. However, a majority of congenitally infected humans do not develop

2,3,7,8-Tetrachlorodibenzo-p-dioxin in pregnant Long Evans rats: disposition to maternal and embryo/fetal tissues.

Prenatal exposure to 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) interferes with fetal development at doses lower than those causing overt toxicity in adult animals. In a multigeneration study (Murray et al., 1979), female rats that were administered 0.01 microgram TCDD/kg/day in their diet did not experience reduced fertility; however, reduced fertility was seen in the F1 and F2 generations. Exposure to TCDD during development produces alterations in the reproductive system of the developing pups, such as delayed puberty and reduced sperm counts in males (Mably et al., 1992a; Gray et al., 1995…
