BACKGROUND: Regression of left ventricular hypertrophy is an important and well-recognized salutary effect of aortic valve replacement. The earliest evidence of left ventricular mass regression after aortic valve replacement and the influence of prosthesis type are not well known, and were the focus of this study.
METHODS: Transthoracic echocardiography was used to measure left ventricular mass index preoperatively and before discharge in 57 consecutive patients undergoing isolated aortic valve replacement (with or without coronary artery bypass grafting).

Balloon mitral valvulotomy is used in treating mitral stenosis. We stabilized a near-moribund patient who had combined aortic and mitral valve stenoses with this technique on an emergency basis. Following valvulotomy, mitral valve area increased from 1.1 to 2.2 cm2, pulmonary capillary wedge pressure decreased from 31 to 21 mmHg, and cardiac index increased from 1.9 to 2.8 L/min/m2. When the patient's condition improved, we replaced the aortic and mitral valves with St. Jude Medical bileaflet prostheses. Technical factors in mitral valve surgery (MVS) which may influence neurological complications, trauma to the left atrium and formation of atrial mural thrombi have not previously been described in detail. We have reviewed the records of 146 patients (pts) undergoing MVS through the superior approach between January 1974 and May 1981. The series consisted of 97 females and 49 males with a mean age of 57 +/- 18 years. All but 4 pts were in New York Heart Association functional class III or IV. Early postoperative intermittent malfunction in a St. Jude mitral valve prosthesis is described. The patient underwent reoperation and had an uneventful recovery. After explantation, the prosthesis underwent laboratory examination. The findings included a minimal disproportion between leaflets and valve ring causing intermittent inhibition of leaflet motion. The unpredictable irregularity of the malfunction phenomenon in vivo, confirmed by laboratory investigations, is explained as a result of a changing myocardial tension status sufficient to alter the ring to leaflet proportion of the valve

A 66-year-old man had an acute inferior wall myocardial infarction complicated by hypotension, high-grade atrioventricular block, and distended neck veins, suggesting associated right ventricular infarction. He failed to respond to volume loading, intra-aortic balloon counterpulsation, and pharmacologic afterload reduction. Cardiac catheterization five days after the infarction disclosed severe tricuspid regurgitation and a hemodynamic pattern resembling pericardial constriction. Emergency tricuspid valve replacement was dramatically successful.

A series of 24 cases of paraprosthetic valvular regurgitation, 10 mitral and 14 aortic, in 19 out of a group of 634 operated patients with a total of 822 prosthetic heart valves is reported. This study included 40 patients over 60 years of age with echocardiographic findings of mitral valve prolapse (MVP). Most of these patients were unaware of any cardiac disorder until the time of echocardiography. In the majority, the clinical manifestations were benign, and the duration of symptoms variable. Real-time, two-dimensional echocardiography was used to document the presence and assess the size and location of vegetative lesions of the cardiac valves and chambers in seven patients with bacterial endocarditis. Anatomic correlation (surgical or autopsy) was accurate in all patients. We report a case of midterm restenosis of freestyle valved conduit implanted between the right ventricle and the pulmonary artery. A 69-year-old woman visited our hospital with dyspnea and general fatigue and was diagnosed as tetralogy of Fallot.
