Peripheral prism glasses: effects of dominance, suppression, and background.

PURPOSE: Unilateral peripheral prisms for homonymous hemianopia (HH) place different images on corresponding peripheral retinal points, a rivalrous situation in which local suppression of the prism image could occur and thus limit device functionality. Detection with peripheral prisms has primarily been evaluated using conventional perimetry, where binocular rivalry is unlikely to occur. We quantified detection over more visually complex backgrounds and examined the effects of ocular dominance. METHODS: Detection rates of eight participants with HH or quadranopia and normal binocularity weari

[The role of circulating endothelial progenitor cells in the progression of retinopathy of prematurity--a prospective study].

PURPOSE: Retinopathy of prematurity (ROP) is the primary cause of visual impairment in preterm infants. There are available data confirming that circulating endothelial progenitor cells (EPCs) are involved in forming the growing network of blood vessels in the developing retina. In this study we sought to explore potential relationship between concentration of circulating bone marrow-derived EPCs and development of ROP in prospective study. MATERIAL AND METHODS: The study groups consisted of 90 preterm (23-36 weeks of gestational age), and 52 full-term control infants. EPCs were analyzed in c

[Steele-Richardson-Olszewski syndrome].

BACKGROUND: Basal ganglia diseases with an akinetic-rigid symptom manifest like a Parkinson disease or an atypical Parkinson syndrome. Their symptoms are similar and therefore a diagnosis often is only possible after long-standing care of the patient. In the class of the atypical Parkinson syndromes is the progressive supranuclear palsy (PSP), which is also known as Steele-Richardson-Olszewski syndrome. CASE REPORT: A seventy-year-old patient who reported unaccountable sudden drops and disturbances of balance as well as diplopia in near vision was examined and a retrospective analysis of the

[The endothelin-1 level in blood serum of patients with primary open angle glaucoma abd its influence on the static perimetry abd Gdx changes].

PURPOSE: The aim of the work was to define the relationships between the endothelin-1 level in blood serum of glaucoma patients concerning the visual field defects and the number of nerve fibers. MATERIAL AND METHODS: 24 women and 14 men, aged 38-74 year (mean 53.4 year) with primary open angle glaucoma, without any systemic diseases. The control group consisted of 20 healthy women and men, aged 34-70 year (mean 52.6 year). The endothelin-1 level was tested with the enzymo-immunologic method from the blood serum. The visual field defects were examined with the Humphrey static perimetry. The n

Correlation of blue-on-yellow visual fields with scanning confocal laser optic disc measurements.

PURPOSE: Visual field defects and changes in the optic nerve head are signs of glaucoma. It has been shown that blue-on-yellow (B-Y) perimetry can reveal visual field defects earlier and shows them larger than does white-on-white (W-W) perimetry. The Heidelberg retina tomograph (HRT) can produce three-dimensional images of the optic disc. The aim of this study was to find out how B-Y perimetry results correlate with optic disc parameters in comparison with W-W perimetry results. METHODS: One randomly chosen eye was evaluated in each of 40 normal subjects and 37 patients with ocular hypertensi

Relative dispersion analysis enhances perimetric sensitivity.

Objective identification of minor visual field defects is problematic. A possible solution is to examine spatial correlations by means of relative dispersion analysis, a tool of fractal analysis. We studied patients with glaucoma, previous optic neuritis, chiasmal compression and lesions of the brain hemispheres, using high-pass resolution perimetry. One-hundred visual field records were drawn consecutively for each category and ranked according to severity of defects. Records with scores ranking below the 35th percentile, i.e. those with the smallest field defects, were analysed. Relative dis

Short-wavelength automated perimetry in neuro-ophthalmologic disorders.

OBJECTIVE: To evaluate the efficacy of short-wavelength automated perimetry (SWAP) in the assessment of patients with neuro-ophthalmologic disorders, especially optic neuropathies. METHODS: A modified Humphrey field analyzer was used to perform standard automated perimetry and SWAP, a technique that isolates the activity of short-wavelength-sensitive ("blue") mechanisms. Forty patients (80 eyes) were evaluated by SWAP and standard automated perimetry. Thirteen patients (26 eyes) had recovered from optic neuritis and/or multiple sclerosis, 15 (30 eyes) were in various stages of treatment for p

Neurologic and developmental outcome in treated congenital toxoplasmosis.

BACKGROUND: Earlier studies have shown that infants with untreated congenital toxoplasmosis and generalized or neurologic abnormalities at presentation almost uniformly develop mental retardation, seizures, and spasticity. Children with untreated subclinical disease at birth have developed seizures, significant cognitive and motor deficits, and diminution in cognitive function over time. OBJECTIVE: To determine neurologic, cognitive, and motor outcomes for children with congenital toxoplasmosis who were treated for approximately 1 year with pyrimethamine and sulfadiazine. DESIGN AND METHODS:

[Complicated oculomotor disorders evaluated by the medical expert].

The association of sensory and motor defects may lead the medical expert to a false estimation of a patient's impairment, especially (1) if he simply adds to the sensory impairment of one eye the motor impairment of the other, without thinking of the eyes as a paired organ; (2) if he underestimates a cumulation of sensory and motor defects in the case of partial adaptation, and (3) if he judges from dyskinetic behavioral characteristics that the patient is blind. Several clinical cases illustrating these problems and allowing the following recommendations are described: (1) Prior to simply sum

Presbyopia correction and the accommodation in reserve.

One method of determining the additional correction for presbyopia suggests leaving a percentage of the amplitude of accommodation in reserve. The rationale for this assumption seems logical because using all of the available accommodation is not sustainable without discomfort. However there is no empirical evidence indicating what percentage of the amplitude of accommodation should actually be left in reserve. Common figures adopted have been one-half and one-third. In this investigation the percentage of accommodation used is deduced mathematically after having determined the following: 1. T

Visual field defects in relation to head injury severity. A neuropsychological study.

A total of 159 head-injured patients were classified into four groups based on two acute categories of injury severity (minor-to-moderate and severe) and the subsequent presence or absence of visual field defects (VFDs). They were assessed neuropsychologically within ten to 19 months after injury. Within both minor-to-moderate and severe injury classifications, patients with VFDs were more impaired neuropsychologically and had more frequent acute secondary intracranial complications (brain swelling, intracranial hypertension, and cerebral hyperemia) than patients without VFDs. The findings sug

[Chiasmatic syndrome--clinical value of the visual evoked responses (VER) to the diagnosis (author's transl)].

The VER is compared with that of other neuroophthalmological examinations in 25 patients presenting lesions of the anterior optic pathways (of which 18 were of an expansive nature). The authors believe that visual field plotting and VER are indispensable complementary examinations, giving results which orientate subsequent investigations towards a chiasmatic or prechiasmatic syndrome. In 15 cases of chiasmatic syndrome of tumoral origin (thus in 30 optic nerves), visual fields and VER showed early pathological changes. In originally unilateral forms, VER changes are usually bilateral even when
