STUDY OBJECTIVES: Sleep quality in patients studied with laboratory-based polysomnography may differ from sleep quality in patients studied at home but remains clinically relevant and important to describe. We assessed objective sleep quality and explored factors associated with poor sleep in patients undergoing laboratory-based polysomnography.
METHODS: We reviewed diagnostic polysomnography studies from a 10-year period at a single sleep center. Total sleep time (TST) and sleep efficiency (SE) were assessed as markers of sleep quality. Poor sleep was defined as TST ≤ 4 hours or SE ≤ 50%. Mu

The pharyngeal airway obstruction during sleep in the obstructive sleep apnea syndrome (OSAS) can be improved after treatment by laser-assisted uvulopalatoplasty (LAUP), a new technique performed under local anesthesia. It permits surgery to be performed without hospitalization or general anesthetic. LAUP has many advantages over the traditional uvulopalatopharyngoplasty (UPPP). It is simple, reliable, hemostatic, sterilizing, and relatively painless. Our experience with LAUP in sleep apnea syndrome is described in 46 patients from December 1988 to May 1993. Among 40 patients classified as suc

Obstructive sleep apnea/hypopnea syndrome (OSAHS) is characterized by repeated airflow partial reduction or complete cessation due to upper airway collapse during sleep. OSAHS can induce frequent awake and intermittent hypoxia that is associated with hypertension and cardiovascular events. Full-channel Polysomnography (PSG) is the gold standard for diagnosing OSAHS; however, this PSG evaluation process is unsuitable for home screening. To solve this problem, a measuring module integrating abdominal and thoracic triaxial accelerometers, a pulsed oximeter (SpO2) and an electrocardiogram sensor w

Rapid assessment of breathing patterns is important for several emergency medical situations. In this research, we developed a non-invasive breathing analysis system that automatically detects different types of breathing patterns of clinical significance. Accelerometer and gyroscopic data were collected from light-weight wireless sensors placed on the chest and abdomen of 100 normal volunteers who simulated various breathing events (central sleep apnea, coughing, obstructive sleep apnea, sighing, and yawning). We then constructed synthetic datasets by injecting annotated examples of the vario

BACKGROUND: Obstructive sleep apnea (OSA) is more common in patients with atrial fibrillation (AFib). Recently, an additional association between central sleep apnea/Cheyne-Stokes respiration (CSA/CSR) and AFib has been described. The aim of this study was to investigate the prevalence and type of sleep-disordered breathing in patients with AFib and normal systolic left ventricular function.
METHODS: 150 patients (110 men and 40 women, aged 66.1 +/- 1.7 years) underwent cardiorespiratory polygraphy, capillary blood gas analysis, measurement of NT-proBNP, and echocardiography to determine the

Continuous positive airway pressure (CPAP) is an accepted and highly effective method used in the treatment of obstructive sleep apnea (OSA). We have explored the potential utility of CPAP in a patient with sleep apnea without a major contribution of airway obstruction as measured by the usual criteria. The patient showed dramatic clinical and laboratory improvement in central apnea when treated with CPAP. A low level of CPAP (7.5 cm H2O) resulted in the reduction of central apneas from 281 to 32 per night with no significant change in obstructive apneas, which increased from 43 to 59 per nigh

In this work, a detection and classification method for sleep apnea and hypopnea, using photopletysmography (PPG) and peripheral oxygen saturation (SpO 2) signals, is proposed. The detector consists of two parts: one that detects reductions in amplitude fluctuation of PPG (DAP)and one that detects oxygen desaturations. To further differentiate among sleep disordered breathing events (SDBE), the pulse rate variability (PRV) was extracted from the PPG signal, and then used to extract features that enhance the sympatho-vagal arousals during apneas and hypopneas. A classification was performed to

Sexsomnia has been reported and is well described in 115 prior cases in the literature. There have been associations with other sleep disorders serving as triggers for confusional arousals, thereby worsening sexsomnia episodes. We present a case of an adolescent boy with a history of resected and treated pineoblastoma who later developed sexsomnia marked by multiple episodes of masturbatory events per night. He had additional suspicions of obstructive sleep apnea. Polysomnography confirmed severe obstructive sleep apnea and captured multiple episodes of sexsomnia from both REM and NREM sleep.

Obstructive sleep apnea syndrome leads to an increased risk of motor vehicle accidents through multiple pathways. Sleep apnea and motor vehicle crashes are common, and motor vehicle collisions are the leading causes of death amongst accidents. Therefore, the clinician charged with the care of apneic patients must address the issue of safe driving. Some recent evidence reveals that driving simulator technology may discriminate apneic drivers from control drivers as well as those apneics at a lesser risk for automobile crashes. This same tool demonstrates an improvement in driving capabilities t

Children with Prader-Willi Syndrome (PWS) may present with a malocclusion and have a high propensity of developing obstructive sleep apnea (OSA). Obstructive sleep apnea is associated with short- and long-term adverse effects that negatively impact children with PWS. A case of a 15-year-old male with PWS, OSA, and a debilitating malocclusion is presented who underwent a combination of Le Fort 1 osteotomy, genioplasty, and tongue reduction to successfully treat his OSA and malocclusion. In select cases, orthognathic correction and other surgical therapies should be considered in patients with P

Sleep apnea syndrome (SAS) is a sleep-related breathing disorder characterized by repetitive breathing interruptions during sleep, resulting in daytime drowsiness, concentration difficulties, and increased risk of cardiovascular diseases. SAS is diagnosed in specialized sleep laboratories via polysomnography (PSG). PSG involves a high number of contact-based sensors, and it may cause patient discomfort and bias in measurement results. Therefore, contactless alternatives to PSG are a promising way to overcome these issues. This work introduces a novel feature-based method for detecting and clas

Sleep apnea may interact endocrine rhythms by a number of mechanisms. Repetitive apneas will cause sleep fragmentation and disorganization of sleep stages and cycles. How this compares with hormonal changes secondary to sleep deprivation is unclear. Hypoxia may have direct central effects on neurotransmitters, which in turn will affect hypothalamic-pituitary hormone production. And, sudden arousal from sleep may produce a central stress response leading to hormonal changes. These factors may interact and lead to changes in the central of sleep and endocrine rhythms. We describe Growth hormone

Excessive daytime sleepiness (EDS) is a frequently reported and not well-understood symptom in patients with Fabry disease (FD). Sleep-disordered breathing (SDB) is a possible factor. As deposition of glycosphingolipids in the upper airway muscles is likely, we hypothesized that obstructive sleep apnoea (OSA) is highly prevalent in FD and positively associated with its severity. All patients with FD who are followed in the Fabry cohort of the University Hospital Zurich (n = 62) were asked to participate in this prospective cohort study. Eligible patients were prospectively investigated by asses

UNLABELLED: Hypoglossal nerve stimulation (HGNS) has evolved as a novel and effective therapy for patients with moderate-to-severe obstructive sleep apnea. Despite positive published outcomes of HGNS, there exist uncertainties regarding proper patient selection, surgical technique, and the reporting of outcomes and individual factors that impact therapy effectiveness. According to current guidelines, this therapy is indicated for select patients, and recommendations are based on the Stimulation Therapy for Apnea Reduction or STAR trial. Ongoing research and physician experiences continuously i
