Interstitial lung disease (ILD) with dermatomyositis often requires intensive immunosuppressive therapy. Here, we report two cases of pulmonary alveolar proteinosis (PAP) in dermatomyositis with ILD. This case report describes agranulocytosis immediately after oral administration of cibenzoline and dabigatran in a 70-year-old woman with paroxysmal atrial fibrillation (AF). No blasts were found in peripheral blood and bone marrow, and the white blood cell count increased abruptly by intravenous administration of granulocyte colony-stimulation factor, suggesting an allergic response caused by cibenzoline or dabigatran, or both. Though antiarrhythmic drugs with anticoagulation therapy are commonly used to treat paroxysmal AF, caution has to be paid to drug-induced agranulocytosis.

INTRODUCTION: Phialophora sp. dematiaceous fungus frequently isolated from soil and organic debris from animals and plants. There have been no reports of nail involvement caused by this fungus. We report the case of a 77-year-old male with blackish-brown pigmentation involving a single finger for one year. The identification of the fungus was determined by macromorphological and micromorphological study of the colony and we discuss the clinical, microbiological, differential diagnosis, and treatment.
COMMENT: We describe the first case of nail infection caused by Phialophora sp.

Two variants of Listeria monocytogenes were isolated from blood cultures from each of two patients with listeriosis. Each variant displayed a two-band difference in DNA profile from the other by pulsed-field gel electrophoresis. Although this difference in profile is insufficient to distinguish clearly between the variants, the possibility of co-infection with different strains of L. monocytogenes needs to be considered. We suggest that more than one colony should be selected for molecular typing to aid interpretation during investigation of the sources and routes of Listeria infection.

The paper generalizes experience gained in performing 5800 courses of multidrug therapy (MDT) in patients with malignant tumors at various sites, complicated in 68 cases of severe neutropenia (neutrophils less than 500 per microl). A case of enteropathy associated T-cell lymphoma (EATCL) in a 62-year-old female with a previous history of coeliac disease, complicated during the clinical course by massive blood and tissue eosinophilia is described. The patient's serum contained a factor capable of stimulating the in vitro growth of eosinophilic colonies (CFU-Eo), that was absent in the serum of normal donors. We suggest that such factor was Interleukin-5 (IL-5), as indicated by the presence in the monoclonal tumor T cells of IL-5 encoding mRNA, usually absent in the normal enterocytes of the jejunum.

The pathogenesis of the Guillain-Barre syndrome (GBS) is felt to involve alterations in cellular and humoral immune responses. We report the case of a 44-year-old female with stage IV breast cancer who developed GBS 2 days after receiving high dose chemotherapy with bone marrow and peripheral progenitor cell support, and granulocyte-macrophage colony stimulating factor (GM-CSF). Her clinical status stabilized with plasmapheresis. This report lends further support to hypotheses regarding cellular immune alterations in the pathogenesis of GBS.

A 12 year old boy was found to be deficient in immunoglobulins (Ig) A, G2 and G4, and common variable immunodeficiency was diagnosed. He also had cyclic thrombocytopenia at intervals of approximately 28-30 days. His bone marrow revealed normocellular with slightly decreased megakaryocytes. In vitro colony assays showed markedly imparied megakaryocytopoiesis, erythropoiesis and granulopoiesis. Platelet-associated IgG was elevated at his thrombocytopenic phase. Direct Coombs' test was repeatedly positive. Although not defined at present, we suggest the autoimmune nature of the disease.

By means of histochemical demonstration of dihydrofolate reductase activity and chromosomal analysis, peripheral white blood cells from patients with chronic granulocytic leukemia were studied in vitro cultures grown on normal leucocytes feeder-layers. After 14 days of incubation, two different types of colonies were found corresponding to the leukemic and normal leucocytic populations: (a) made up of Ph'-positive leucocytes which have a high dihydrofolate reductase activity; (b) made up of Ph'-negative leucocytes, slightly positive for dihydrofolate reductase activity.

A 6-year-old male blue-and-gold macaw (Ara ararauna) was presented with severe weakness, anorexia, and weight loss of 2 weeks duration. Cardiac auscultation revealed a soft systolic murmur. Blood cultures collected both antemortem and postmortem yielded pure isolates of Enterobacter cloacae. At necropsy, vegetative endocarditis was found involving the left atrioventricular valve. Microscopically, the lesion on the valve was characterized by a mixture of necrotic material, colonies of gram-negative bacteria, fibrosis, and inflammatory infiltrate consisting primarily of heterophils.

Accidental trauma associated with an epileptic convulsion in a 10-year-old, male chimpanzee (Pan troglodytes) resulted in incomplete paraplegia from fracture and subluxation of T4. Dorsal laminectomy and segmental spinal fixation were used in treatment. The segmental spinal fixation consisted of sublaminar wires attached to a contoured 316L stainless steel U-rod. The chimpanzee recovered sufficient function to allow reintroduction into the chimpanzee colony at a zoological park during the 12 months after surgery and continues to do well 24 months after surgery.

Cells from a salivary gland pleomorphic adenoma with a reciprocal translocation, t(8;12)(q22;q14), were transformed by transfection with a plasmid containing the SV40 "early region." Compared with nontransformed adenoma cells, the cells expressed large-T antigen, had a higher proliferative capacity, showed a drastically increased or even unlimited growth potential in vitro, and formed colonies in soft agar. The amount of cellular material produced in this way should make possible many experiments at the molecular level that could not be performed before.

Nervous pointer dogs have been suggested as an animal model for certain human pathological anxiety states. Work done in humans and in animals suggests a role for noradrenergic overactivity in the manifestations of anxiety. We studied alpha 2 adrenergic receptor binding as determined by [3H]-yohimbine in platelets and brains of the nervous and normal dogs in our colony. Our findings indicate that alpha 2 adrenergic receptor density and affinity are similar in platelets and frontal cortex, but we did not observe significant differences in binding between the two groups of dogs.

The effect of peripheral mononuclear cells (MNCs), lymphocytes and sera of a patient with typical cyclic neutropenia (CN) upon the normal colony forming unit in culture (CFU-C) growth was examined. MNCs and lymphocytes obtained during the recovery phase of the cycles inhibited CFU-C growth. In contrast, these cells obtained during the neutropenic phase revealed no suppressive effect.
