Mortality and cause of death postoperatively in patients with a hip fracture: a national cohort longitudinal follow-up study.

AIMS: The aim of this study was to compare the rate of mortality and causes of death in Korean patients who undergo surgery for a fracture of the hip, up to 11 years after the injury, with a control group from the general population. MATERIALS AND METHODS: National cohort data from Korean Health Insurance Review and Assessment Service - National Sample Cohort were used. A ratio of 1:4 matched patients with a fracture who underwent surgery (3383, fracture group) between 2003 and 2012, and controls (13 532) were included. The matches were processed for age, gender, income, and region of residen

Health concerns of veterans with high-level lower extremity amputations.

BACKGROUND: The aim of the study was to identify health concerns of veterans with high-level lower extremity amputations. METHODS: Through a cross-sectional study, general practitioners, an orthopedic specialist, psychologists, psychiatrists, physiotherapist and prosthetists examined 100 veterans using a short-form health-related quality of life questionnaire (SF-36) that assessed their ability to perform activities of daily living (ADL), instrumental activities of daily living (IADL) and life satisfaction (SWLS) after hip disarticulation or hemi-pelvectomy amputations. The assessment tool wa

Control of standing balance at leaning postures with functional neuromuscular stimulation following spinal cord injury.

This study systematically explored the potential of applying feedback control of functional neuromuscular stimulation (FNS) for stabilizing various erect and leaning standing postures after spinal cord injury (SCI). Perturbations ranging from 2 to 6% body weight were applied to two subjects with motor complete thoracic level SCI who were proficient at standing with implanted multichannel neural stimulators to activate the ankle, knee, hip and trunk muscles. The subjects stood with four different postures: erect, forward, forward-right and forward-left. Repeatable and controlled perturbations w

Anterior Column Realignment (ACR) in Adult Sagittal Deformity Correction: Technique and Review of the Literature.

STUDY DESIGN: The anterior column realignment (ACR) procedure was retrospectively reviewed. OBJECTIVE: To review surgical technique, complication avoidance, case examples, and published results on ACR. SUMMARY OF BACKGROUND DATA: For surgical correction of sagittal imbalance, three column osteotomies (3CO) have traditionally been employed for large degrees of correction at a single segment. However, 3CO procedures are technically challenging and carry high morbidity rates. ACR was developed as a less invasive procedure for restoring segmental lordosis. METHODS: The ACR surgical technique is

An Integrated Musculoskeletal-Finite-Element Model to Evaluate Effects of Load Carriage on the Tibia During Walking.

Prior studies have assessed the effects of load carriage on the tibia. Here, we expand on these studies and investigate the effects of load carriage on joint reaction forces (JRFs) and the resulting spatiotemporal stress/strain distributions in the tibia. Using full-body motion and ground reaction forces from a female subject, we computed joint and muscle forces during walking for four load carriage conditions. We applied these forces as physiological loading conditions in a finite-element (FE) analysis to compute strain and stress. We derived material properties from computed tomography (CT)

Fractures of the femoral head: a long-term follow-up study.

The femoral head fracture has become an increasingly frequent injury, usually sustained by individuals during high-energy trauma. Regardless of the type of treatment, long-term consequences, as avascular necrosis, post-traumatic arthritis, and heterotopic ossification, may complicate the clinical outcome leading to variable degree of disability. The aim of this study was to review the clinical and radiological long-term follow-up of patients with a fracture of the femoral head. Between January 1985 and January 2002, twenty-one patients with mean age 42.0 ± 15.9 years (range, 21-70 years) with

Serum FGF21 levels are elevated in association with lipodystrophy, insulin resistance and biomarkers of liver injury in HIV-1-infected patients.

OBJECTIVE: HIV-1-infected patients with lipodystrophy show insulin resistance, dyslipidemia and other signs of metabolic syndrome. Fibroblast growth factor-21 (FGF21) is a novel metabolic regulator that has been suggested to exert beneficial effects on metabolic homeostasis and insulin sensitivity. Our goal was to determine the relationship between FGF21 levels and metabolic alterations in these patients. RESEARCH DESIGN AND METHODS: Serum FGF21 levels were analyzed in 179 individuals belonging to four groups: HIV-1-infected, antiretroviral-treated patients that have developed lipodystrophy (

The effects of muscle damage on walking biomechanics are speed-dependent.

The purpose of the present study was to examine the effects of muscle damage on walking biomechanics at different speeds. Seventeen young women completed a muscle damage protocol of 5 × 15 maximal eccentric actions of the knee extensors and flexors of both legs at 60°/s. Lower body kinematics and swing-phase kinetics were assessed on a horizontal treadmill pre- and 48 h post-muscle damaging exercise at four walking speeds. Evaluated muscle damage indices included isometric torque, delayed onset muscle soreness, and serum creatine kinase. All muscle damage indices changed significantly after ex

Trauma related fat embolism syndrome in forensic practice.

The fat embolism syndrome (FES) in forensic practice is observed usually in cases of polytrauma related deaths. FES is rare, but serious complication after trauma. The most cases of post traumatic fat embolism are not fatal and it's very likely that many cases of mild fat embolism are overlooked. We describe a case of fat embolism syndrome in a young man after high energy injury. Upon concrete ceiling fell on his shoulders he had open fractures of both tibias and massive haematoma of the left hip joint. Use of antimicrobial prophylaxis for major surgery: baseline results from the National Surgical Infection Prevention Project.

HYPOTHESIS: Surgical site infections (SSIs) are a major contributor to patient injury, mortality, and health care costs. Despite evidence of effectiveness of antimicrobials to prevent SSIs, previous studies have demonstrated inappropriate timing, selection, and excess duration of administration of antimicrobial prophylaxis. We herein describe the use of antimicrobial prophylaxis for Medicare patients undergoing major surgery. DESIGN: National retrospective cohort study with medical record review. SETTING: Two thousand nine hundred sixty-five acute-care US hospitals. PATIENTS: A systematic r

Current concepts in sports medicine: the use of COX-2 specific inhibitors and the emerging trends in arthroscopic surgery.

Arthritis does not escape the athlete. From the recreational athlete to the professional athlete, arthritis can be a common and perplexing problem. In the typical orthopedic sports medicine practice, it is no longer uncommon to see relatively young patients suffering from arthritis. It can affect the major joints such as the knee, hip, ankle, shoulder, and elbow. One of the most common problems is shoulder arthritis secondary to injury in recreational athletes. Athletes at risk for shoulder arthritis typically include overhead athletes and weight lifters. The clinical presentation is usually s

Bone density and local growth factors in generalized osteoarthritis.

Osteoarthritis is usually considered to be a primary disorder of chondrocyte function with secondary changes in bones. However, a defect in the subchondral bone resulting in loss of its shock absorbing capacity could transfer the stress of loading directly to the articular cartilage with secondary changes in the cartilage. Review of histomorphometric and bone densitometric studies at sites of osteoarthritis at the hip or knee revealed that cartilage fibrillation could not be dissociated from bony changes even in the earliest stages of osteoarthritis and that subchondral trabeculae are thickene

Acceptable outcomes after anterior subcutaneous pelvic Internal Fixation (INFIX) for Fragility Fractures of the Pelvis (FFP) type III in elderly patients: a retrospective study.

BACKGROUND AND PURPOSE: For elderly with fragility fractures of the pelvis (FFP) type III, minimally invasive surgery may reduce perioperative morbidity and facilitate mobilization, but evidence guiding implant selection remains limited. We evaluated postoperative hip function, recovery of independence in activities of daily living, and safety of the anterior subcutaneous pelvic internal fixator (INFIX) in elderly patients with FFP type III fractures, including surgery-related complications and fixation failure. METHODS: We retrospectively reviewed all elderly patients (≥ 65 years) with FFP ty

Genetic Correlation and Causal Inference Between Female Fat Distribution and Preeclampsia: An Integrative Genomic Study.

Preeclampsia (PE) is a major cause of maternal and perinatal morbidity. Because abnormal fat distribution is closely related to metabolic dysfunction, vascular injury, and hypertensive disorders during pregnancy, clarifying its genetic relationship with PE may improve our understanding of adverse pregnancy outcomes. Here, we investigated the shared genetic architecture between PE and waist-hip ratio adjusted for body mass index (WHRadjBMI) by integrating large-scale genome-wide association study (GWAS) summary statistics for female WHRadjBMI from the GIANT consortium (n ≈ 700 000) and PE from
