Factors affecting cancer recurrence after microvascular flap reconstruction of the head and neck.

OBJECTIVE: Our objective was to determine which factors are associated with cancer recurrence after microvascular reconstruction of the head and neck for squamous cell carcinoma (SCC). STUDY DESIGN: A cohort of patients who fit the inclusion/exclusion criteria were identified retrospectively. METHODS: A group of 184 patients who underwent successful surgical resection and simultaneous microvascular reconstruction of the head and neck for treatment of SCC were identified. The mean age was 60 (range 23-90) years, there were 115 males and 69 females, and mean follow-up was 26.2 (range 1-99) mon

Urothelial carcinoma of the bladder with prostatic stromal invasion and early postoperative bone metastases: a case report.

BACKGROUND: Prostatic involvement by bladder urothelial carcinoma is uncommon, and prostatic stromal invasion usually indicates aggressive tumor biology and poor prognosis. Such patients may present with prostatic enlargement or a mass at the bladder neck-prostate junction, while serum prostate-specific antigen (PSA) may remain within the normal range. This diagnostic overlap may lead to clinical confusion with benign prostatic hyperplasia or primary prostatic tumors. CASE PRESENTATION: A 54-year-old man was admitted with urinary frequency, urgency, dysuria, and intermittent gross hematuria.

Can displaced extra-articular fractures of the scapula be stabilized through a direct lateral-column approach.

BACKGROUND: Displaced extraarticular fractures of the scapula are uncommon, and rarely require operative fixation. When managed operatively, a posterior Judet approach with detachment of the deltoid muscle from the scapular spine and elevation of the infraspinatus from its fossa, is often performed. This approach is invasive and involves extensive soft tissue dissection. This paper describes the utility of single-column fixation with a direct lateral column approach, with mobilisation of the deltoid, and elevation of the interval between the infraspinatus and teres minor. Our aim is to assess

Free Flap Transfer for Head and Neck Reconstruction Using Local Anesthesia in Elderly Patients.

BACKGROUND: The incidence of skin cancer appearing on the head and neck areas is higher in elderly patients. Although free flap reconstruction is the mainstay after tumor excision, it is challenging to complete in elderly patients, owing to the high risk of complications and/or mortality rates associated with the use of general anesthesia. In this study, we used only local anesthesia in free tissue reconstruction of the head and neck in five elderly patients. MATERIALS AND METHODS: From 2013 to 2016, 5 elderly patients with high risk of general anesthesia underwent reconstruction with either

Exploring the Significance of the Obturator Externus Groove as a Marker for Bipedalism: A Cadaveric Study.

Background Bipedalism, defined as locomotion using only two feet, is a distinctive trait that sets humans apart from most other creatures. This study explores the significance of the obturator externus groove (OEG) as a marker for bipedalism. This topic is important due to its implications in evolutionary history. Previous studies have identified various anatomical features indicative of bipedalism, including an S-shaped spine, a centrally positioned foramen magnum, and unique pelvic and femoral structures. Among these, the morphology of the femur, particularly the presence of the OEG, has bee

Factors Influencing the Relationship of the External Branch of the Superior Laryngeal Nerve with the Superior Pole Vessels of the Thyroid Gland.

OBJECTIVES: In a thyroidectomy, the external branch of the superior laryngeal nerve (EBSLN) is a potential risk during the superior pole dissection due to its close anatomical relationship with the superior thyroid artery and its highly variable anatomy. In this study, we aimed to evaluate the relationship of EBSLN with the superior pole considering Cernea classification and the factors affecting this relationship. METHODS: The data of thyroidectomized 126 patients (95 female, 31 male) with 200 neck sides (mean age of 45.6±12.1 years) using intraoperative neuromonitoring (IONM) for the EBSLN

Development of a New Outcome Prediction Model in Early-stage Squamous Cell Carcinoma of the Oral Cavity Based on Histopathologic Parameters With Multivariate Analysis: The Aditi-Nuzhat Lymph-node Prediction Score (ANLPS) System.

The aim of this study was to evaluate the histopathologic parameters that predict lymph node metastasis in patients with oral squamous cell carcinoma (OSCC) and to design a new assessment score on the basis of these parameters that could ultimately allow for changes in treatment decisions or aid clinicians in deciding whether there is a need for close follow-up or to perform early lymph node dissection. Histopathologic parameters of 336 cases of OSCC with stage cT1/T2 N0M0 disease were analyzed. The location of the tumor and the type of surgery used for the management of the tumor were recorde

Total aortic endovascular repair.

Endovascular repair has an established role in aortic treatment and has with evolving technologies expanded its applicability to the whole aorta. Whereas open repair is still the standard treatment for aortic arch and ascending aortic pathology, in high-risk patients, endovascular treatment has gained an increasing role. Endovascular options range from hybrid procedures to total endovascular repair using chimney grafts, branched and fenestrated stent grafts. Thoracic endovascular repair (TEVAR) for descending aortic pathologies including aneurysms, traumatic aortic transection, and complicated
