Review of the current methods in the diagnosis and treatment of scaphoid fractures.

If neglected or misdiagnosed, non-union of a scaphoid fracture will almost inevitably progress to radiographic and symptomatic osteoarthritis of the wrist with subsequent morbidity and lifelong disability, especially in young males in which the fracture is more common. Fractures of the scaphoid bone are the most common fractures of the carpus and second in occurrence among fractures of the wrist. The diagnosis and treatment are not simple. Familiarity with different imaging methods and treatment options is required. The treatment in most cases is conservative and will lead to uneventful union,

Ipsilateral intertrochanteric and diaphyseal femoral fractures. Four patients treated by one technique.

Concomitant intertrochanteric and diaphyseal fracture of the femur is an uncommon injury that is probably best managed by open reduction and internal fixation as soon as the patient is hemodynamically stable. Four cases are presented in which rigid fixation of both fractures allowed early mobilization of the patient, facilitating nursing care and allowing rapid rehabilitation. Use of the sliding compression screw with a long side plate, although technically demanding, achieved rigid fixation with compression at both fracture sites. All fractures in the present series united, but in two of four

The geriatric distal femur fracture: nail, plate or both?

Surgical fixation of distal femur fractures in geriatric patients is an evolving topic. Unlike hip fractures, treatment strategies for distal femur fractures are ill-defined and lack substantive high-quality evidence. With an increasing incidence and an association with significant morbidity and mortality, it is essential to understand existing treatment options and their supporting evidence. Current fixation methods include the use of either retrograde intramedullary nails, or plate and screw constructs. Due to the variability in fracture patterns, the unique anatomy of the distal femur, and

A Novel Anatomical Locking Guide Plate for Treating Acetabular Transverse Posterior Wall Fracture: A Finite Element Analysis Study.

OBJECTIVE: To improve the treatment of the acetabular transverse posterior wall fracture (ATPWF), a novel anatomical locking guidance plate (NALGP) was designed and compared with traditional fixations using finite element analysis. METHODS: The ATPWF model was constructed using the three-dimensional finite element model of the half pelvis via the Mimics software and three internal devices were used to fix this model: the posterior-column locking plate with anterior-column screws (PCLP), double-column locking plates (DCLP), and NALGP. Next, mesh division was conducted by solid 187 tetrahedral

[Bio-derived bone transplantation with tissue engineering technique: preliminary clinical trial].

OBJECTIVE: To sum up the clinical results of bio-derived bone transplantation in orthopedics with tissue engineering technique. METHODS: From January 2000 to May 2002, 52 cases with various types of bone defect were treated with tissue engineered bone, which was constructed in vitro by allogeneous osteoblasts from periosteum (1 x 10(6)/ml) with bio-derived bone scaffold following 3 to 7 days co-culture. Among them, there were 7 cases of bone cyst, 22 cases of non-union or malunion of old fracture, 15 cases of fresh comminuted fracture of bone defect, 4 cases of spinal fracture and posterior r

Bone Health in Women.

Osteoporosis is a significant chronic condition worldwide, causing morbidity and mortality that may be prevented with mitigation of risk factors, screening, and treatment of low bone density to prevent fractures. Adequate intake of calcium and vitamin D as well as regular weight-bearing exercise are integral for optimal bone health. Decreased bone density on dual-energy x-ray absorptiometry screening or evidence of fragility fractures are qualifiers for treatment. Treatment options beyond the first-line treatment of oral bisphosphonates exist for those who do not tolerate these medications or

The Child with Multiple Fractures, What Next?

Fractures are common during childhood; however, they can also be the presenting symptom of primary or secondary causes of bone fragility. The challenge is to identify those children who warrant further investigation. In children who present with multiple fractures that are not commonly associated with mild to moderate trauma or whose fracture count is greater than what is typically seen for their age, an initial evaluation, including history, physical examination, biochemistry, and spinal radiography, should be performed. In children with bone pain or evidence of more significant bone fragilit

[Prevention of bone loss in a patient receiving androgen deprivation therapy].

Bone loss is a frequent side effect of androgen deprivation therapy, which leads to a significant number of pathologic fractures. Prevention of osteoporosis aims at reducing skeletal-related events. The RANK ligand antibody denosumab and bisphosphonates are used for this indication. Denosumab has been shown to increase bone density and to reduce the incidence of fractures at any location by 72%. Bone density can also be increased by bisphosphonates; however, there is no conclusive evidence so far that the fracture rate can be reduced significantly. Therefore guidelines do not recommend use of

[Bone metastases].

Bone metastases are a frequent condition leading to fractures, hypercalcaemia and spinal compression. The treatment will depend on the possibility to treat the cancer. Secondly local treatment should be considered. To relief pain radiotherapy, given as 1 fraction with 8 Gy, is an effective treatment. Radiotherapy can also be used to treat complications from the metastasis. The use of bisphosphonates is now established in the treatment of breast cancer with bone metastasis to prevent complications from bone metastasis. Surgery is a growing possibility both as treatment of manifest fractures and

The natural history of suspected scaphoid fractures.

Clinically suspected scaphoid fractures are common injuries. The authors prospectively followed up, by serial radiography and bone scanning, 23 patients who had a history of trauma to the carpus and associated localized tenderness to the anatomic "snuff box": in 15 patients the radiographs and bone scans were negative; in 5 the radiograph was equivocal and the bone scan positive. The authors concluded that patients with clinically suspected scaphoid fractures and negative radiographs can be treated symptomatically without immobilization in a cast. Patients with equivocal radiographs should be

Segmental fracture of the scaphoid: report of a rare fracture pattern.

INTRODUCTION: To the best of our knowledge, there are no reports in the orthopaedic and trauma literature of true segmental fracture of the scaphoid bone. We present such a case with a brief discussion of the morphology and mechanisms of injury of scaphoid fractures and the problems they present, particularly in diagnosis. CASE HISTORY: A 43-year-old male with polytrauma sustained in a motorcycle road traffic accident was treated at our hospital. His injuries included a fracture initially thought to involve the waist of the scaphoid. Because he had bilateral upper limb injuries, we elected to

Determinants of mortality risk following osteoporotic fractures.

PURPOSE OF REVIEW: Increased mortality risk is accepted for hip and vertebral fracture. Recent data suggest that other fracture types have also been linked to excess mortality. This article reviews the existing evidence on the pattern and determinants of postfracture mortality. RECENT FINDINGS: The pattern of mortality over time following hip and vertebral fractures has recently been clarified. Nonhip nonvertebral fractures at major, and even minor sites in older individuals have also been associated with excess mortality. Studies have revealed the higher excess mortality in men and in younge

Effective management of pregnancy- and lactation-associated osteoporosis using romosozumab: a clinical case report.

Pregnancy- and lactation-related osteoporosis (PLO) is a rare condition characterized by reduced bone density and increased fracture risk during late pregnancy or early postpartum. Because of its rarity, no well-established treatment exists for PLO. Here, we report a case demonstrating significant improvements in bone density and the absence of new fractures after 12 mo of romosozumab (ROMO) treatment following weaning. This report suggests that ROMO could effectively increase bone density and reduce fracture risk in patients with PLO.

Simulation study on the mechanical mechanism of humeral fracture during throwing.

The purpose of this study was to understand the mechanical mechanisms behind throwing fractures and their relation to bone degeneration. Finite models were built based on three bone degeneration evaluation indexes. All models showed that the crack initiated at the morphological transition junction of the distal 1/3 of the humeral shaft. The initial crack line was lateral to the humerus and extended spirally at 45° with the humerus axis. The fracture torque of healthy humerus was about 63 N·m, with a fracture torque degree of about 12°, both decreasing with increasing bone degeneration.

[Secondary osteoporosis. Disordered bone metabolism in chronic kidney disease.].

In patients with chronic kidney disease(CKD), mineral metabolism abnormalities such as hyperphosphatemia, decreased 1,25-dihydroxyvitamin D, and elevated parathyroid hormone develop as kidney function declines, which lead to vascular calcification and a variety of skeletal abnormalities, collectively termed renal osteodystrophy. Because CKD patients have increased risk of bone fractures, it is important to assess fracture risk by measuring bone mineral density and bone metabolism markers. In addition to management of secondary hyperparathyroidism, medications for osteoporosis could be a reason

Scintigraphy in the evaluation of the "clinical" scaphoid fracture.

In a prospective study we performed bone scans on 84 patients who had sustained a wrist injury and in whom a scaphoid fracture was clinically suspected but could not be confirmed on the original set of five routine "scaphoid view" radiographs. In 40 patients the bone scan was normal and in 25 there was increased uptake in areas other than the scaphoid. In 19 there was localized increased uptake in the scaphoid bone. In seven of these a scaphoid fracture was subsequently demonstrated on repeat radiographs (five cases) or a CT scan (two cases). In the remaining 12 no fracture could be demonstrat

Age and Female Sex Are Important Risk Factors for Stress Fractures: A Nationwide Database Analysis.

BACKGROUND: Stress fractures are caused by micro-trauma due to repetitive stress on bone, common in active individuals and athletes. Previous studies demonstrate that the weightbearing bones of the lower extremities incur stress fractures most often, especially in women and older adults. HYPOTHESIS: Prior literature does not quantify the difference in frequency of stress fractures among different genders, age groups, or body mass indices (BMIs). We hypothesized that older female patients would have higher rates of lower extremity stress fractures than male patients. STUDY DESIGN: Epidemiolog

Influence of lateral hinge fractures on biplanar medial closing-wedge distal femoral osteotomy for valgus knee: a new classification of lateral hinge fracture.

BACKGROUND: The purpose of this study was to examine the influence of lateral hinge fractures in medial closing-wedge distal femoral osteotomy (MCWDFO) on bone union. METHODS: Twenty-one patients were followed-up for more than 1 year after MCWDFO. The incidence and type of hinge fracture, as well as the course of bone healing, were investigated. Slow healing was defined as bone union was not obtained until 3 months after surgery. RESULTS: Hinge fractures were observed in 12 cases (57%). There were three types of hinge fractures. Type 1: the lateral cortex was completely cut through (4 cases)
