Safety and Efficacy of BCMA CAR-T Cell Therapy in Older Patients With Multiple Myeloma.

Risks of B-cell maturation antigen (BCMA) chimeric antigen receptor T-cell (CAR-T) therapy for patients with multiple myeloma (MM) include cytokine release syndrome (CRS), immune effector cell-associated neurotoxicity syndrome (ICANS), cytopenias, and infections. The efficacy and safety of BCMA CAR-T therapy in the geriatric setting, including complications such as falls and delirium, which may be more prevalent in older patients, have not been fully analyzed. We wanted to analyze the efficacy and safety of BCMA CAR-T therapy among older patients (age ≥70 at infusion) versus younger patients w

Osteosarcopenia: epidemiology, diagnosis, and treatment-facts and numbers.

BACKGROUND: Osteosarcopenia, the presence of osteopenia/osteoporosis and sarcopenia, is an emerging geriatric giant, which poses a serious global health burden. METHODS AND RESULTS: The prevalence of osteosarcopenia ranges in community-dwelling older adults [5-37% (≥65 years)] with the highest rates observed in those with fractures (low-trauma fracture: ~46%; hip fracture: 17.1-96.3%). Among 2353 community-dwelling adults, risk factors associated with osteosarcopenia include older age [men: 14.3% (60-64 years) to 59.4% (≥75 years); women: 20.3% (60-64 years) to 48.3% (≥75 years), P < 0.05], p

Predicting ideal spinopelvic balance in adult spinal deformity.

OBJECTIVE: Spinopelvic balance is based on the theory that adjacent segments of the spine are related and influenced by one another. By understanding the correlation between the thoracolumbar spine and the pelvis, a concept of spinopelvic balance can be applied to adult deformity. The purpose of this study was to develop a mathematical relationship between the pelvis and spine and apply it to a population of adults who had undergone spinal deformity surgery to determine whether patients in spinopelvic balance have improved health measures. METHODS: Using values published in the literature, a

Frailty as a Predictor of COVID-19 Mortality in the South Indian Population: An Observational Study.

Background Frailty is a clinical syndrome characterized by diminished strength, endurance, and physiological function that significantly increases vulnerability to adverse health outcomes, including infections. In the context of COVID-19, frailty has emerged as a critical risk factor for severe disease, complications, and mortality, particularly in older adults. The severity and fatality rates among the geriatric group were notably high, as the virus's pathogenesis, marked by prolonged inflammation, contributed to increased morbidity and mortality in this age group. The study was conducted to

Oral Health and Undernutrition in the Frail Elderly Persons.

INTRODUCTION: Although many studies associate a poor state of oral health with the risk of undernutrition in both the autonomous and dependent elderly, very few of them consider the frail elderly person. However, the identification of the frail elderly is one of major issues of modern geriatrics. It is at this stage that preventive strategies are the most effective against the risks of dependency. The main objective of our study is to explore the nature of the association between oral health and undernutrition in the frail elderly patient, and to identify the associated factors. MATERIALS AND…

Treatment of bimalleolar fractures in elderly.

Geriatrics continues to evolve as a specialty by adapting itself to increasingly older patients. Musculoskeletal injuries are common in these patients, who can maintain their physical capacities and autonomy for a long time, but whose bone solidness is frequently reduced by osteoporosis. Falls increase with age and because of certain medical conditions. Trauma in the geriatric population involves specific risks; thus, the treatment must be adapted not only to the fracture, but to the local conditions and the patient. Ankle injuries are particularly frequent in general traumatology but even mor…

Phytotherapy for Cachexia: Where Do We Stand?

BACKGROUND: In contrast to Western medicine which currently offers no approved pharmacotherapy options for cachexia, in Japan multi-component extracts of medicinal plants are used with coverage by the national health insurance. This so called "Kampo" medicine is an example of the modern concept of multi-component/multi-target therapy. For the three traditional preparations Hochuekkito (), Juzentaihoto (), and Rikkunshito (), a multitude of clinical research data relating to cachexia has been published. These preparations are also referred to as "Hozai" (). A similar concept is found in Russian

[The practice of restraint in a psychiatric emergency unit].

The practice of physical restraint is relatively frequent in medical emergency and geriatric units. Its use in psychiatry is controversial. Although distinct, it is often associated with seclusion, as a response to or prevention of agitated mentally ill patients'behavior. A detailed review of the literature shows the scarceness of work defining the exclusive use of restraint without seclusion. We report a naturalistic study over 6 Months, covering 76 cases having required restraint. The study of the international literature concerns nursing care, geriatric, child-adolescent psychiatric and adu

Dysphagia and aspiration pneumonia in older adults.

PURPOSE: To review the risks, diagnosis, management, and prevention of aspiration pneumonia in adults over age 65, and discuss the major role of dysphagia as a risk factor in this population. DATA SOURCES: A comprehensive search of the literature using PubMed (MedLine) and CINAHL. CONCLUSIONS: Aspiration pneumonia is a common diagnosis in older adults and is associated with significant morbidity and mortality. Oropharyngeal and esophageal dysphagia, often related to stroke, dementia, poor oral hygiene, or multiple chronic illnesses, increases the risk of aspiration. Nurse practitioners (NPs)

Rational dental care: part 2. A case history.

Treatment planning and managing the care of an elderly, frail patient can be complicated by a variety of modifying factors. To understand the patient"s needs, one must understand the environment in which the patient functions. This second in a series of 2 articles presents a case history of a frail, older adult to illustrate some of the social, medical and community problems that are involved in caring for elderly persons.

Health professionals need to be able to identify depression in older people.

Healthcare literature shows that depression in older people, as a mental health problem, is often under-recognized and under-managed. This may partly be the result of multiple pathology, a common feature in later life, masking the features of depressive illness. It may also be the consequence of inadequate use of mental health specialists' skills. As the population is ageing rapidly, healthcare professionals will encounter more cases of depression among the older population. Some therapeutic approaches have been suggested for nurses to adopt. More importantly, nurses and other care workers hav

Behavioral approaches for reducing agitation in residents of long-term-care facilities: critical review and suggestions for future research.

Many residents in long-term-care facilities demonstrate agitated behaviors. Research on behavioral approaches for reducing agitation in nursing home residents has been conducted during the past 25 years. This research is critically reviewed in this article and suggestions for future research are offered. Empirical evidence suggests that behavioral approaches are effective. Antecedent control strategies have been shown to reduce physically nonaggressive behaviors. Both aggressive and verbally agitated behaviors have been successfully treated by manipulating reinforcing consequences of these beh

Contribution of cognitive impairment, depression, and psychosis to the outcome of agitated geropsychiatric inpatients with dementia.

This study investigated the correlates of change in behavioral disturbance in geropsychiatric inpatients with dementia. It was hypothesized that improvement in specific psychiatric symptoms, such as psychosis and depression, contribute to the improvement of specific behavioral disturbances. All admissions between October 1993 and May 1995 were reviewed to identify those patients admitted to the Houston VA geropsychiatry unit with a diagnosis of dementia; 233 patients were included in the study. Improvement in behavioral disturbance symptoms was associated with decreases in depression, thought

Staging and Grading in Gerodontology-An Introduction.

INTRODUCTION: Gerodontology lacks a unified framework to describe case complexity and assess the multidimensional determinants shaping oral health. METHODS: An international initiative developed an evidence-based case definition for staging and grading older adults' oral health. RESULTS: Staging includes three domains: person, oral function (clinical and subjective), and oral disease (caries, periodontal disease, mucosal conditions) level staging. Grading incorporates risk factors at the oral, systemic, and social levels, including caries and periodontal risk, prosthetic maintenance, xerosto

Remote assessment and management of physical function in older adults (RAMP): An international expert Delphi process.

BACKGROUND: Remote methods may help older adults improve their physical function, but understanding the perspectives of experts who deliver this care is critical. This modified Delphi process aimed to develop expert consensus on effectiveness, feasibility and implementation of remote assessment and management of physical function in older adults. METHODS: This online Delphi process included experts involved in supporting older adults to maintain or improve their health. The Round 1 survey presented 33 Delphi statements assessed using an 11-point Likert scale (0-10; ≥7 indicated agreement). St

Validated Measures of Agitation in Neurocognitive Disorders: A Systematic Review of 41 Scales and Their Alignment With International Psychogeriatric Association Criteria.

The International Psychogeriatric Association (IPA) developed a consensus syndromic definition of agitation in neurocognitive disorders. To facilitate adoption of the IPA criteria, we systematically reviewed validated measures of agitation and evaluated alignment with IPA criteria. This review was pre-registered on PROSPERO (CRD42023429494). We searched MEDLINE, EMBASE, and PsycINFO from inception to June 30, 2023 (updated September 9, 2025) using search clusters for 1) neurocognitive disorders; 2) agitation; and 3) psychometric outcomes. Title/abstract screening identified validation studies
