Enteral nutrition in patients with dementia and stroke.

PURPOSE OF REVIEW: Patients suffering from dementia or significant cognitive impairment (SCI) due to neurologic injury routinely receive percutaneous endoscopic gastrostomy (PEG) due to swallowing difficulty or lack of appetite. This review discusses current data and opinion regarding the risks and benefits of PEG in these populations. RECENT FINDINGS: The current data regarding PEG placement in patients with dementia or SCI due to neurologic injury do not confirm either improvement or worsening of survival. Significant risk factors for poor prognosis after PEG include sex, hypoalbuminemia, a

ESPEN Guidelines on Parenteral Nutrition: surgery.

In modern surgical practice it is advisable to manage patients within an enhanced recovery protocol and thereby have them eating normal food within 1-3 days. Consequently, there is little room for routine perioperative artificial nutrition. Only a minority of patients may benefit from such therapy. These are predominantly patients who are at risk of developing complications after surgery. The main goals of perioperative nutritional support are to minimize negative protein balance by avoiding starvation, with the purpose of maintaining muscle, immune, and cognitive function and to enhance posto

Consumption of vitamin A by breastfeeding children in rural Kenya.

Vitamin A deficiency remains a significant health risk in developing countries, affecting infants and children in particular. To counter child malnutrition, mothers are encouraged to breastfeed to ensure that their children receive adequate macro- and micronutrients, including vitamin A. However, this assumes that the mother has sufficient vitamin A intake to provide enough vitamin A to her child. This study investigates maternal and infant intakes of locally available foods of high vitamin A content in a rural agricultural community in Kenya. The study aims to establish the community risk for

Old and alone: barriers to healthy eating in older men living on their own.

Ageing is associated with reduced energy intake and loss of appetite. Older men tend to have poorer dietary intakes including consumption of fewer fruits and vegetables in comparison to older women. Living and eating alone further diminishes food consumption and dietary quality. The aim of the present study was to explore food choice and energy intake in older men living alone using both quantitative and qualitative methods. 39 older men were interviewed and completed questionnaires on health, food choice, dietary patterns and appetite. Few men managed to consume recommended levels of energy,

Detection of vitamin A deficiency in Brazilian preschool children using the serum 30-day dose-response test.

BACKGROUND: Vitamin A deficiency (VAD) is endemic in Brazil and health professionals have difficulty in recognizing its subclinical form. In addition, serum retinol concentrations do not always represent vitamin A status in the organism. OBJECTIVE: To identify VAD in preschool children by the serum 30-day dose-response test (+S30DR) and to examine its potential as a tool for the assessment of vitamin A status in the community. DESIGN: A prospective transverse study in which blood samples were obtained from 188 preschool children for the determination of serum retinol concentrations and the c

Summary of the evidence for perioperative nutritional management of TIPS in patients with cirrhotic portal hypertension.

This study aims to search, evaluate, and summarize the existing evidence on perioperative nutritional management of transjugular intrahepatic portosystemic shunt (TIPS) in patients with cirrhotic portal hypertension (CPH) and provide the basis for evidence-based clinical practice. According to the "6S" model of evidence-based sources, the evidence on perioperative nutrition management of TIPS in patients with CPH from relevant databases and websites was systematically searched, including clinical decisions, guidelines, expert consensus, systematic review, and evidence summary.
