Equity in the Provision of Diabetes Self-Management Education and Support.

Diabetes self-management education and support (DSMES) interventions must be accessible to all people with diabetes. To address equity in the delivery of DSMES, interventions should consider the unique needs of various populations. This article outlines the needs of a wide range of populations, including people with diabetes who are racially or ethnically diverse; have limited English proficiency or literacy; are deaf or hard of hearing; are blind or have low vision; are neurodiverse; live with learning disabilities or intellectual or developmental disabilities; have dementia or cognitive impa

A new light chain for myosin-7.

Recent research has revealed that an adhesion complex based on cadherins and the motor protein myosin-7b (MYO7B) links the tips of intestinal microvilli. Choi et al. now report that a largely uncharacterized protein known as calmodulin-like protein 4 (CALML4) is a component of this adhesion complex and functions as a light chain for myosin-7b. Because the intermicrovillar adhesion complex is homologous to the myosin-7a (MYO7A)-based Usher syndrome complex and Choi et al. also report that CALML4 can bind to myosin-7a, this work also has important implications for research on myosin-7a and hered

[Osteogenesis imperfecta and dentinogenesis imperfecta: diagnostic frontiers and importance in dentofacial orthopedics].

Osteogenesis imperfecta is a genetic disease that varies in severity and is characterized by fragile bones that fracture easily. Many extra-skeletal manifestations can be noted such as blue sclerotic markings, dentinogenesis imperfecta and impaired hearing or deafness. In most cases, an anomaly of collagen is the cause. It is usually accompanied by a specific Class III type cranio-facial morphology with open bite and increased incidence of impacted permanent molars. Orthodontists called upon to treat the dental aspects of this malady, should be careful to protect their patients against bacteri…

First Croatian auditory brainstem implantation.

A deaf female patient was diagnosed with bilateral acoustic neurinomas. Diagnosis incorporated the standard audiological battery for sensorineural hearing loss, computed tomography and magnetic resonance imaging. The left side had been operated on four years previously in another clinic using the suboccipital approach. The auditory brainstem implant surgery was performed on the 'second side' using the same approach. It was an uneventful operation with good anatomy and no serious post-operative complications. Post-operatively, the patient performed exceptionally well, with up to 50 per cent of

Neurological complications of local anaesthetics in dentistry.

Neurological complications following the administration of a local anaesthetic can be alarming. By reading reports of such incidents, dentis … Horner's syndrome, transient paralysis of combined cranial nerves III, IV and VI, sudden unilateral deafness and abducens nerve palsy. A thorough knowledge of the relevant anatomy pertinent to the various injections used in dental surgery is essential and is highlighted in the text.

Explantation of a nucleus multichannel cochlear implant and re-implantation into the contralateral ear. A case report of a new strategy.

We present a unique case in which a multichannel cochlear implant device was explanted and the same device was re-implanted into the contralateral ear. A patient with bilateral total deafness secondary to head injury received an implant in his left ear but developed severe facial nerve stimulation. Because this stimulation could not be eliminated effectively with change of programming, it was decided to implant the contralateral ear. Since the device itself was functioning well, it was explanted from the left ear and re-implanted successfully into the right ear. Facial nerve stimulation was mi…

[CT examination of the temporal bones for cochlear implantation].

CT examinations of temporal bones were performed in 31 deaf patients from qualify diagnostic stage prepared for cochlear implants. Essential for surgical treatment anatomic details were estimated, especially canalis spiralis cochleae was paid to attention. In our group, 8 patients had some CT changes (abnormal findings) which corresponded to different stages of obliteration the cochlea. The majority of patients lost the hearing because of meningitis. In 6 implanted patients CT scan was compared with surgical findings. During the operation 1 patient with patent cochlea on CT scan had partially

Postural control and vestibular function in patients selected for cochlear implantation.

Postural control and vestibular and eye motor function were evaluated in 7 postlingually deaf patients before cochlear implantation as vibration toward the calf muscles or galvanic electrical stimulation of the vestibular nerves perturbed stance. The patients were compared with 21 control subjects. Vibration-induced bodysway was increased in the patients compared to the normals. Galvanic stimulation induced a bodysway that was not significantly different from that of the control group suggesting that the patients selected for cochlear implantation, and with otherwise reduced postural control,

Advances in otology and neurotology.

During the last few years many advances occurred in the field of Otolaryngology. In this article we will present significant advances in the fields of otology and neurotology. The adaptation of laser energy to surgery is a significant event in the evolution of ear microsurgery. Lasers in otology reduce trauma and increase accuracy. Implantable hearing devices are used to rehabilitate patients with mild to moderately severe hearing loss. Cochlear implants have secured a permanent role in the management of selected profoundly deaf patients. Intraoperative cranial nerve monitoring is a major adva

Hearing-impaired autistic children.

The charts of 46 children diagnosed as deaf and autistic were reviewed. Nearly one-fifth had normal or near-normal non-verbal intelligence and only one-fifth had severe mental deficiency. The severity of the autistic behavior was related to the severity of the mental deficiency, but not to that of the hearing loss. In 11 of the 46 children, autism went unrecognized for over four years after the diagnosis of hearing loss, and in 10 the hearing loss went unrecognized for several years after the diagnosis of autism. The educational experience of some children was generally disastrous because of t

[Early treatment. An overview].

"Early training" for deaf children was introduced some twenty years ago. The idea rapidly gained ground for using the same principles to help as early as possible all children suffering from a handicap the diagnosis of which could be made at birth or very early in life, as well as those children who are retarded or language-risk children. Such interventions take account of the parents, the child, and are aimed at encouraging or furthering everything in the field of arousal, communication and language. In many respects speech therapists are the best trained to meet the needs of multidirectional

Demyelinating radiculopathy in the Kearns-Sayre syndrome: a clinicopathological study.

In the few previously autopsied patients with the Kearns-Sayre form of progressive ophthalmoplegia, the most prominent abnormalities have been in muscle, with less conspicuous changes in the central nervous system, primarily in the brainstem. Similar findings were present in the case reported here, but in addition there was severe demyelination in the initial few millimeters of the cranial and spinal motor roots distal to the glial-Schwann cell junction. Milder demyelination was observed in the dorsal spinal and afferent cranial nerve roots, including the eighth nerve, but deafness was due to …

AUDITORY NERVE: ELECTRICAL STIMULATION IN MAN.

Auditory perceptions produced in a person deaf to acoustic stimulation were studied by electrically exciting the auditory nerve through permanently implanted electrodes. Pulsed current as small as 1 microampere peak-to-peak could be perceived. Pitch, as reported by the subject, varied with electrode selection, current amplitude, and pulse repetition rate from about 70 to at least 300 pulses per second. Loudness increased with amplitude and duration of pulse stimuli, and to a lesser extent with repetition rate. The total range in amplitude of the stimulus, from threshold to an uncomfortable lou

IoT-driven smart assistive communication system for the hearing impaired with hybrid deep learning models for sign language recognition.

Deaf and hard-of-hearing people utilize sign language recognition (SLR) to interconnect. Sign language (SL) is vital for hard-of-hearing and deaf individuals to communicate. SL uses varied hand gestures to speak words, sentences, or letters. It aids in linking the gap of communication between individuals with hearing loss and other persons. Also, it creates comfortable for individuals with hearing loss to convey their feelings. The Internet of Things (IoTs) can help persons with disabilities sustain their desire to attain a good quality of life and permit them to contribute to their economic a
