Negatively charged phospholipid liposomes were labelled with technetium 99m and used as a radioisotopic tracer to detect focal inflammatory lesions in deep-seated sepsis and rheumatoid arthritis. Hepatobiliary studies were performed over a three-year period on 139 patients suspected of having cystic duct obstruction. Each patient was infused intravenously with sincalide, a C-terminal octapeptide of CCK, 15 minutes prior to the administration of the hepatobiliary imaging agent Tc-99m paraisopropyl iminodiacetic acid (PIPIDA). Condylar hyperplasia poses a problem in planning treatment because it is a self-limiting process for some but not all patients. Serum thyroglobulin (Tg) level was determined in 34 biologically euthyroid patients having benign hypofunctioning nodule, before and after surgery. An unusual position of the spleen may cause diagnostic difficulties and may mimic disease. We report two patients with an unusual position of the spleen. In one, congenital absence of the lieno-renal ligament allowed unusual mobility of a bilobed spleen with intermittent torsion. In the other the spleen descended into the left renal fossa following left adrenalectomy and nephrectomy.

The relationship between bone pain and stress fractures diagnosed by bone scintigraphy was investigated in military recruits during active training. In three patients pain appeared in the site of abnormal uptake 7-14 days after the bone scan in a previously asymptomatic site. A highly motivated 18-year-old man continued vigorous military training in spite of the presence of 11 stress fractures and subsequently developed two additional stress fractures. Scintigraphy over 27 weeks illustrated simultaneous regression and progression in his individual multiple foci. When stress fracture foci were not in anatomic locations prone to evolve to gross fractures, healing occurred despite vigorous activities and pain.

50 patients with complaints in the region of the sacroiliacal joints and the hip, but negative radiographs following injury to the pelvis were investigated by bone scan. Scintigraphic diagnosis always showed involvement of the pelvis ring other than the apparent fractures. Together with the clinical symptoms scintigraphic findings are interpreted as sacroiliacal strain or subluxation. Additional injuries to the acetabulum and to the lumbosacral joints may be present. The patients' complaints are explained, the injuries are localized and documented.

A case of metastatic phaeochromocytoma is presented. Meta-iodobenzylguanidine was used to locate the tumour but failed to show two functioning abdominal metastatic deposits. Although meta-iodobenzylguanidine appears to be highly specific for phaeochromocytoma, a negative scan does not exclude a functioning tumour with confidence.
