Integrating qualitative and quantitative MRI analysis for optimizing DBS candidate selection in patients with disorders of consciousness.

INTRODUCTION: Disorders of consciousness (DoC) encompass a spectrum of clinical conditions with often indistinct boundaries, making accurate diagnosis and therapeutic decision-making particularly challenging. While advanced imaging techniques such as fMRI and PET reduce misdiagnosis risk, their limited availability in routine clinical settings underscores the need for alternative approaches. This study investigates whether the integration of qualitative and quantitative parameters derived from conventional MRI can improve diagnostic precision and support more accurate deep brain stimulation (D

Interpreting liver function markers in children on continuous renal replacement therapy for acute liver failure.

Children with acute liver failure (ALF) often experience hyperammonaemia, fluid overload, metabolic derangements, and multi-organ failure. C … ecular clearance during CRRT is crucial for accurate interpretation of liver biochemistry to inform clinical decision-making. By considering the kinetic properties of hepatic markers, including bilirubin, bile acids, liver enzymes, metabolites, waste products, inflammatory markers, albumin, and coagulation proteins, we describe the impact of CRRT on the plasma levels of t …

Evaluating the use of electromyography in UK and European gait laboratories for the assessment of cerebral palsy and other neurological and musculoskeletal conditions.

BACKGROUND: Electromyography (EMG) can estimate the magnitude and timing of muscle activation during walking in those with gait disorders. Despite the potential of EMG use in assessment and clinical decision-making, there are reports of declining use of EMG within gait laboratories. Technical and educational barriers to EMG usage in clinics in Italy were recently suggested. RESEARCH QUESTION: What is the current EMG practice and associated knowledge and barriers to EMG usage in UK and European clinical gait labs? METHODS: Semi-structured interviews were conducted online with 16 participants …

Are Clinical Practice Guidelines Representative of Patients With Distal Radius Fractures? A Review of Patient Demographics and Patient-Reported Outcome Measures Used to Inform Guidelines.

PURPOSE: Clinical practice guidelines (CPGs) are recommendations developed for broad application to optimize high-quality care and decision-making. The composition of patients and outcome measures used in studies informing CPGs; however, has not been rigorously evaluated. With growing evidence that outcomes in musculoskeletal surgery vary by sociocultural factors, we aimed to: (1) review the linguistic, racial, and ethnic representation of the patients in the studies informing CPGs for distal radius fractures and (2) assess their use of linguistically and culturally adapted patient-reported ou

DISENGAGE Registry.

BACKGROUND: During fractional flow reserve (FFR) measurement, the simple presence of the guiding catheter (GC) within the coronary ostium might create artificial ostial stenosis, affecting the hyperemic flow. We aimed to investigate whether selective GC engagement of the coronary ostium might impede hyperemic flow, and therefore impact FFR measurements and related clinical decision-making. METHODS: In the DISENGAGE (Determination of Fractional Flow Reserve in Intermediate Coronary Stenosis With Guiding Catheter Disengagement) registry, FFR was prospectively measured twice (with GC engaged [FF…

[The value of in vitro diagnostic testing in the provision of medical care to patients with cardiovascular diseases.].

In vitro diagnostics are used at all stages of patient care. The aim of this study was to assess the impact of laboratory examination on clinical decision-making in providing medical care to patients with a cardiovascular profile. We also took into account the level of financing for the laboratory industry in the Russian Federation. We divided our study on three sequential steps: literature review, survey of clinicians and test-survey of clinicians. The share of costs for the laboratory tests in 2017 amounted to about 8% of the total funding for Russian health care. About 80% (70; 90) of the v…

Why Drugs Fail in Late Stages of Development: Case Study Analyses from the Last Decade and Recommendations.

New drug development is both resource and time intensive, where later clinical stages result in significant costs. We analyze recent late-stage failures to identify drugs where failures result from inadequate scientific advances as well as drugs where we believe pitfalls could have been avoided. These can be broadly classified into two categories: 1) where science is mature and the failures can be avoided through rigorous and prospectively determined decision-making criteria, scientific curiosity, and discipline to follow up on emerging findings; and 2) where problems encountered in Phase 3 fa

Fracture risk assessment without bone density measurement in routine clinical practice.

UNLABELLED: Fracture probability assessed without bone mineral density (BMD) could potentially be sufficient for clinical decision making in many individuals categorized as low or high fracture risk. For individuals falling in a moderate risk range, there is incremental value in using BMD in the probability calculation as this appropriately reclassifies risk in over one third of the individuals. INTRODUCTION: A new fracture risk assessment tool from the World Health Organization (FRAX) estimates 10-year major osteoporotic and hip fracture probabilities from multiple clinical risk factors with

The development of a decision aid to support Hodgkin lymphoma survivors considering lung cancer screening.

BACKGROUND: Decisions aids (DA) can support patients to make informed decisions about screening tests. This study describes the development and initial evaluation of a lung cancer screening (LCS) DA targeted towards survivors of Hodgkin lymphoma (HL). METHODS: A prototype decision aid booklet was developed and subsequently reviewed by a steering group who provided feedback. Revisions were made to produce the DA tested in this study. HL survivors were recruited to an online survey and/or focus groups. Lymphoma practitioners were invited to an interview study. In the online survey, decisional c

Aspartate transaminase to platelet ratio index and Model for End-Stage Liver Disease scores are associated with morbidity and mortality after endovascular aneurysm repair among patients with liver dysfunction.

BACKGROUND: Liver cirrhosis dramatically increases morbidity and mortality after open surgical procedures and is often a contraindication to open repair of abdominal aortic aneurysms. However, limited data have evaluated the effect of liver disease on outcomes after endovascular repair of aortic aneurysms. METHODS: The National Surgical Quality Improvement Program was used to evaluate all nonemergent endovascular aneurysm repairs (EVARs) from 2005 to 2016. The aspartate transaminase to platelet ratio index is a sensitive, noninvasive screening tool used to screen for liver disease and was cal

Protocolized Based Management of Cerebrospinal Fluid Drains in Thoracic Endovascular Aortic Aneurysm Repair Procedures.

BACKGROUND: Spinal cord ischemia (SCI) resulting in paraplegia is a devastating complication associated with thoracic endovascular aortic aneurysm repair (TEVAR) whose incidence has significantly declined over time. In this review, we present our experience with a multidisciplinary clinical protocol for cerebrospinal fluid (CSF) drain management in patients undergoing TEVAR. Furthermore, we aimed to characterize complications of CSF drain placement in a large, single center experience of patients who underwent TEVAR. METHODS: This retrospective review is of patients undergoing TEVAR with and

Comparative effectiveness of antihypertensive drugs in nondiabetic patients with hypertension: A population-based study.

The authors compared the effectiveness of thiazide diuretic (TD), angiotensin-converting enzyme inhibitor (ACEI), angiotensin receptor blocker (ARB), and calcium channel blocker (CCB) monotherapies for the treatment of nondiabetic hypertension using MarketScan Databases 2010-2014. Multivariable Cox regression models assessed whether the addition of a new antihypertensive drug, treatment discontinuation, or switch and major cardiovascular or cerebrovascular events varied across groups. A total of 565 009 patients started monotherapy with ACEIs (43.6%), CCBs (23.6%), TDs (18.8%), or ARBs (14.0%)

AI is already in the theatre of war just not on the side of the patient: clinical chatbots, telemedicine, and an agenda for "AI for medical humanity".

Artificial intelligence (AI) is already embedded in modern warfare, mainly through surveillance, targeting, and military decision-making. However, its potential to support clinicians and patients in conflict settings remains underdeveloped. Frontline medical teams must make time-critical, high-stakes decisions with scarce diagnostics, disrupted referrals, insecure communications, changing injury patterns, and irregular access to specialist expertise. Humanitarian medicine needs a proactive clinician-facing AI agenda that strengthens clinical judgment, not replaces it. We propose a two-stage mo

The Role of Therapeutic Drug Monitoring in the Treatment of Childhood Leukemia.

Therapeutic drug monitoring (TDM) is critical for optimizing chemotherapy in pediatric leukemia, where high interpatient variability and narrow therapeutic windows increase the risk of toxicity. This review examines TDM for key cytotoxic agents as well as tyrosine kinase inhibitors. TDM enables individualized dosing, guides supportive care decisions, facilitates adherence monitoring, and minimizes treatment-related adverse effects. Advances in TDM may enable clinicians to incorporate real-time monitoring, predictive modeling, and standardized guidelines into clinical decision making to achieve
