Oral breathing and oxygenation stability during wakefulness: Identifying an unresolved question in respiratory physiology.

Arterial oxygen saturation is traditionally interpreted using threshold-based paradigms in which physiological significance is primarily attributed to overt hypoxemia. However, evidence from sleep physiology and integrative respiratory research suggests that oxygenation dynamics, including the cumulative burden of transient oxygen desaturation, may provide complementary physiological information beyond average oxygen saturation alone. Whether similar concepts are relevant during wakefulness remains largely unexplored. Among the factors that could influence oxygenation dynamics, breathing route

Characterization of vascular tortuosity throughout the murine oxygen-induced retinopathy model of ischemic retinopathy.

Vascular tortuosity (VT) is a critical biomarker of disease progression that informs the decision to treat ischemic retinal disorders, particularly retinopathy of prematurity (ROP). The murine oxygen-induced retinopathy (OIR) model is the most widely used model of ischemic retinopathy. Although VT has been described in OIR, its temporal dynamics have not been systematically defined. In this study, a semiautomated artificial intelligence-based (AI-based) pipeline was used to quantify VT throughout OIR. Retinal flat mounts from age-matched normoxic and OIR mice (P10-P56) underwent vessel segment

Subtherapeutic Pentobarbital Levels in a Patient With Intracranial Pressure Crisis on Venoarterial Extracorporeal Membrane Oxygenation.

Elevations in intracranial pressure (ICP) may result after neurologic injury. As ICP increases, cerebral perfusion pressure (CPP) decreases and may lead to further neurologic damage. Management of an ICP crisis may involve several different therapies, including hyperosmolar therapy, cerebrospinal fluid diversion, or surgical craniectomy. Pentobarbital is a barbiturate that has been shown to decrease cerebral blood flow and may be used in severe cases of ICP crisis. The literature has not described management strategies for ICP crisis in patients requiring extracorporeal membrane oxygenation (E

Self‑oxygenating nanoparticle-mediated photodynamic therapy for biofilm control and macrophage immunomodulation in vitro.

Periodontitis is a chronic biofilm-induced inflammatory disease that is increasingly linked to systemic conditions. Effective treatment requires removing pathogenic biofilms and modulating the host inflammatory response. Antibacterial photodynamic therapy (aPDT) shows promising potential in the field of antibacterial treatment. However, issues such as the hypoxic microenvironment within periodontal pockets and biofilm structures, as well as the poor stability of traditional photosensitizers, pose challenges to the efficacy of aPDT. Here, we developed a multifunctional nanoparticle MB-MnO₂@PLGA

Nonocclusive Balloon Dilation in Severe Tracheobronchial Stenosis: Preserving Ventilation and Oxygenation Throughout the Procedure.

BACKGROUND: Balloon tracheobronchial dilation is a key technique for managing central airway stenosis. Conventional occlusive balloons interrupt ventilation and oxygenation, limiting the duration and safety of dilation. The objective of this study was to evaluate the clinical outcomes of tracheobronchial stenosis dilation using a nonocclusive balloon and to describe relevant technical aspects of its application. METHODS: A retrospective cross‑sectional study was conducted, including all patients with central airway stenosis who underwent dilation with a nonocclusive balloon at a tertiary cent

Intraoperative monitoring of myocardial oxygenation during open-heart surgery: feasibility and initial performance.

Myocardial protection during cardiopulmonary bypass relies on intermittent administrations of cardioplegia without continuous, tissue‑level feedback on myocardial oxygen supply and mitochondrial metabolic status. Extended near‑infrared spectroscopy (eNIRS) can enable real-time intramyocardial monitoring of hemoglobin/myoglobin oxygen saturation (SHb/Mb) and cytochrome c oxidase oxidation (OCcO) for assessment of cardiac oxygen availability and utilization during surgery. In this prospective, single-center feasibility study, myocardial eNIRS monitoring was carried out in 15 adults undergoing ao

Hypoxia/reoxygenation cycles, reactive oxygen species and succinate: A mechanistic framework for apnea of prematurity?

We review clinical observations and experimental data to examine the pathophysiological mechanisms by which apnea of prematurity and intermittent hypoxemia destabilizes respiratory control and might lead to neurodevelopmental disorders. Clinically, both apnea and hypoxemia are characterized by marked heterogeneity in definitions, monitoring approaches, and temporal patterns, complicating comparisons across studies. Clinical evidence suggests that cumulative hypoxemic burden, rather than apnea duration alone, is a key determinant of respiratory and neurodevelopmental morbidity in preterm neonat

Oxygen-Refractory Platypnea-Orthodeoxia Syndrome Requiring Urgent Surgical Closure: A Dynamic Assessment.

BACKGROUND: Platypnea-orthodeoxia syndrome (POS) causes posture-dependent hypoxemia due to right-to-left shunt, and it may be underestimated by routine evaluation. CASE SUMMARY: An 84-year-old woman developed oxygen-refractory, posture-dependent hypoxemia during facial cellulitis and after contracting COVID-19. Transesophageal echocardiography identified a large patent foramen ovale with dynamic right-to-left shunt. Provocative assessment using invasive oximetry under a semi-sitting position and abdominal compression demonstrated a decrease in pulmonary-to-systemic flow ratio from 0.66 to 0.5

Idiopathic pneumoperitoneum, a possible complication of pure oxygen administration in a neonate with laryngomalacia: a case report.

INTRODUCTION AND CLINICAL IMPORTANCE: Idiopathic pneumoperitoneum can occur in a full-term neonate due to the administration of pure oxygen. Recognition of this rare mechanism is necessary to avoid unnecessary surgical exploration. CASE PRESENTATION: We report the case of a six-day-old male neonate with a history of respiratory distress and non-invasive pure oxygen support (FiO2 = 1.00) via a nasal cannula. He progressively developed abdominal distension, but had no fever, abdominal tenderness, or redness. A massive pneumoperitoneum was noted on a plain X-ray. While preparing for surgical exp

Effects of hyperbaric oxygen therapy on EEG and sEMG signals after low-load exercise.

OBJECTIVE: To evaluate the effect of hyperbaric oxygen therapy (HBOT) on acute exercise-induced fatigue via electroencephalogram (EEG) signal and surface electromyography (sEMG) signal, with the aim of providing a theoretical basis and methodological reference for promoting fatigue recovery. We hypothesized that HBOT for 60 min and natural recovery would reverse the changes in the indicators related to blood flow restriction (BFR) intervention, with HBOT demonstrating a superior restorative effect. METHODS: College students (n = 34) majoring in physical education were randomly assigned to an

Protist oxygen-sensing and the role of prolyl hydroxylase-regulation of E3(SCF) ubiquitin ligases.

A primary mechanism for sensing O2 in animals involves the O2-dependent prolyl hydroxylase PHD2, which generates a degron on the transcription co-factor HIF-α. In protists, its apparent evolutionary predecessor, PhyA, is dedicated to the modification of Skp1. Skp1 is a subunit of the E3(SCF) family of ubiquitin ligases whose impact on the proteome is mediated by multiple F-box protein (FBP) substrate receptors. In the social amoeba Dictyostelium, genetic manipulations of PhyA and Skp1 levels lead to reciprocal effects on an O2-dependent developmental transition. PhyA is also required for fitne

A Rare Case Report of Penile Necrosis Successfully Treated With Early Hyperbaric Oxygen Treatment.

INTRODUCTION: Warfarin is a widely used oral anticoagulant for the treatment of thromboembolic disorders. Warfarin-induced skin necrosis (WISN), a serious but rare complication, typically affects the breasts, hips, and abdomen. Penile necrosis, however, has been reported only in exceptionally rare cases. CASE REPORT: In this report, we present the successful outcome of early hyperbaric oxygen treatment (HBO₂) in a patient who developed glans penis necrosis during warfarin treatment and was subsequently diagnosed with protein C deficiency. The patient underwent nine HBO₂ sessions initiated ear

Mismatch between oxygen delivery and consumption in the cerebral watershed during subacute global hypoperfusion.

Hemodynamically significant carotid artery stenosis is a common clinical condition that can lead to chronic cerebral hypoperfusion. Despite the well-recognized pivotal role of pial collaterals in maintaining cerebral perfusion during focal arterial occlusions, regulation of microvascular blood flow and oxygenation in the cerebral watershed "pial-collateral territory" during chronic hypoperfusion remains unexplored. To answer this question, we applied 2-photon microscopy and Doppler optical coherence tomography to assess the changes in cerebral blood flow, capillary red-blood-cell (RBC) flux, a

Lung ultrasound aeration score in pediatric ARDS: associations with oxygenation impairment and RALE score.

UNLABELLED: We evaluated associations of the lung ultrasound aeration score (LUS Aeration Score) and Radiographic Assessment of Lung Edema (RALE) score with oxygenation impairment in pediatric acute respiratory distress syndrome (PARDS) and explored whether either imaging score added incremental discrimination for ICU mortality. This single-center prospective observational study included 42 invasively ventilated children aged 2 months to 16 years with PARDS. LUS Aeration Score, RALE, and oxygenation index (OI) were assessed within 48 h of diagnosis (T1; n = 42) and 48 h later (T2; n = 35). Ass

Examining Obstructive Sleep Apnea Risk and Changes in Oxygen Saturation in an Outpatient Endoscopy Center.

As obesity becomes more prevalent in the United States, the incidence of undiagnosed or untreated obstructive sleep apnea (OSA) and the associated comorbidities present challenges when providing sedation to office-based gastroenterology patients. This study examined changes between pre-post procedural oxygen saturations among patients with moderate to high OSA risk. Patients (N = 316) in the preprocedural area were screened using the STOP-BANG questionnaire prior to receiving anesthesia. The patients' STOP-BANG score (SBS) and pre-post procedural oxygen saturations were obtained and documented
