The causes and management of nonunion of femoral subtrochanteric shortening osteotomy in a THA patient: a case report.

BACKGROUND: Total hip arthroplasty (THA) is considerably difficult to perform in patients with Crowe type IV developmental dysplasia of the hip (DDH). Some Crowe type IV DDH patients require a femoral subtrochanteric shortening osteotomy to equalize the length of the lower extremities and decrease the difficulty of intraoperative reduction. Subtrochanteric transverse osteotomy has been proven to have superior clinical efficacy, but some cases of nonunion occur. CASE PRESENTATION: We present the case of a 62-year-old male patient who underwent right THA with femoral subtrochanteric transverse

Corpectomy with titanium cage reconstruction in the cervical spine.

Multi-level anterior cervical discectomy and fusion (ACDF) has been utilized by many surgeons to address multi-level cervical disease with the use of autograft or allograft. High failure rates have been reported in three-level decompressions when autograft is used, which increases with the use of allograft. This has led many surgeons to advocate anterior cervical corpectomy with strut grafting where only two surfaces are needed for fusion compared to the multiple surfaces required for multi-level ACDF. We retrospectively evaluated the efficacy of titanium cage use with autologous bone graft (r

Long-term follow-up radiologic and clinical evaluation of cylindrical cage for anterior interbody fusion in degenerative cervical disc disease.

OBJECTIVE: Various procedures have been introduced for anterior interbody fusion in degenerative cervical disc disease including plate systems with autologous iliac bone, carbon cages, and cylindrical cages. However, except for plate systems, the long-term results of other methods have not been established. In the present study, we evaluated radiologic findings for cylindrical cervical cages over long-term follow up periods. METHODS: During 4 year period, radiologic findings of 138 patients who underwent anterior cervical fusion with cylindrical cage were evaluated at 6, 12, 24, and 36 postop

Intramedullary fixation of forearm fractures with new locked nail.

BACKGROUND: Lack of availability of interlocked nails made plate osteosynthesis the first choice of treatment of forearm fractures inspite of more surgical exposure, periosteal stripping and big skin incision subsequent scar along with higher risk of refracture on implant removal. We hereby report the first 12 cases with 19 forearm bone fractures internally fixed by indegenous interlocked nail. MATERIALS AND METHODS: Existing square nails were modified to have a broad proximal end of 5.5 mm with a hole for locking screw of 2.5 mm. The nail has a distal hole of 1/1.2/1.5 mm in 2.5/3/3.5 mm dia

Vitamin D and human health: lessons from vitamin D receptor null mice.

The vitamin D endocrine system is essential for calcium and bone homeostasis. The precise mode of action and the full spectrum of activities of the vitamin D hormone, 1,25-dihydroxyvitamin D [1,25-(OH)(2)D], can now be better evaluated by critical analysis of mice with engineered deletion of the vitamin D receptor (VDR). Absence of a functional VDR or the key activating enzyme, 25-OHD-1alpha-hydroxylase (CYP27B1), in mice creates a bone and growth plate phenotype that mimics humans with the same congenital disease or severe vitamin D deficiency. The intestine is the key target for the VDR beca

[Osteochondroma of the clavicle and pain syndrome of the shoulder. Apropos of a case. Review of the literature].

INTRODUCTION: Osteochondroma, a frequent benign tumor of the bone, is in most cases asymptomatic. Localization in the clavicle is extremely rare. The authors report an isolated case occurring in the coraco-clavicular area responsible for a painful shoulder syndrome evoking rotator cuff tendinitis. METHODS: A 47 years female patient consulted for painful shoulder syndrome. Pain occurred following an effort and at night. On clinical examination, the area around the coracoid process was tender as was the supraspinatus fossa. Passive anterior elevation of the shoulder was limited to 150 degrees.

Pin vs plate fixation for metacarpal fractures: a meta-analysis.

BACKGROUND: The differences in the clinical and functional outcomes of closed reduction and percutaneous pin fixation and open reduction with internal fixation (ORIF) using plate and screws have been systematically synthesized by one meta-analysis. With newer studies being published, an effort to update the earlier meta-analysis is necessary. METHODS: Comprehensive searches were done systematically through PubMed, Scopus, CENTRAL (Cochrane Central Register of Controlled Trials), and Google scholar databases. Randomized controlled trials, quasi-experimental studies, prospective comparative non

Minimally invasive anterior pelvic internal fixation: An anatomic study comparing Pelvic Bridge to INFIX.

OBJECTIVES: Anterior external fixation for pelvic ring fractures has shown to effectively improve stability and reduce mortality. However, these fixators can be associated with substantial morbidity such as pin tract infection, premature loss of fixation, and decreased quality of life in patients. Recently, two new methods of subcutaneous anterior pelvic internal fixation have been developed; the INFIX and the Pelvic Bridge. These methods have the purported advantages of lower wound complications, less surgical site pain, and improved quality of life. We sought to investigate the measured dist

Anterior subcutaneous internal fixation of the pelvis - what rod-to-bone distance is anatomically optimal?

INTRODUCTION: Anterior fixation of the pelvis using subcutaneous supra-acetabular pedicle screw internal fixation (INFIX) has proven to be a useful tool by avoiding the downsides of external fixation in patients where open fixation is not suited. The purpose of this study was to find a rod-to-bone distance for the INFIX that allows for minimal hazard to the inguinal neuro-vascular structures and, at the same time, as little as possible interference with the soft tissues of the proximal thigh when the patient is sitting. METHODS: An INFIX was applied to 10 soft-embalmed cadaver pelvises with t

Optimal internal fixation of anatomically shaped synthetic bone grafts for massive segmental defects of long bones.

BACKGROUND: Large segmental bone defects following tumor resection, high-energy civilian trauma, and military blast injuries present significant clinical challenges. Tissue engineering strategies using scaffolds are being considered as a treatment, but there is little research into optimal fixation of such scaffolds. METHODS: Twelve fresh-frozen paired cadaveric legs were utilized to simulate a critical sized intercalary defect in the tibia. Poly-ε-caprolactone and hydroxyapatite composite scaffolds 5 cm in length with a geometry representative of the mid-diaphysis of an adult human tibia wer

Fracture fixation in the mutilated hand.

Early anatomic (or near anatomic; lingers do not impinge or overlap during flexion or extension) stable fracture fixation provides the foundation for successful wound management and for the repair, reconstruction, and healing of all damaged tissues in a mutilating hand injury. It also plays an instrumental role in pain control and affords an optimal opportunity for timely and favorable rehabilitation of and recovery from mutilating injuries of the hand. Kirschner or other wiring systems or mini external fixators may be used for simple fractures, in children, when rapid fracture fixation is nec

Heavy metal: beware.

A 19-year-old Crouzon's syndrome patient with a history of multiple craniofacial procedures presented with severe bilateral temporal and frontal depressions and metal implants protruding through the scalp in multiple locations. Preoperative analysis revealed an extensive cranial defect that had been reconstructed with multiple large metallic mesh implants. The mesh required complete removal with an autograft cranial reconstruction. We present this case to urge that caution and forethought be exercised when contemplating the use of metallic alloplasts for major craniofacial reconstructions.

Internal fixation of supracondylar fractures after condylar total knee arthroplasty.

Condylar buttress plates were applied in six patients with total knee arthroplasties and supracondylar fractures. According to the criteria of Cain, 50% of the patients had a satisfactory result with an average follow-up period of 17 months. Time to union (full weight bearing) averaged 14 weeks. The final coronal femorotibial alignment averaged 5 degrees of valgus, whereas preoperative valgus had averaged 8 degrees. Four patients were pain free and the two others had minimal pain that did not interfere with daily activity. Knee motion averaged 97 degrees. All patients were ambulating; five of

Dual Fixation in Complex Long Bone Fractures: A Case Series of 30 Patients.

INTRODUCTION: Complex long bone fractures characterized by comminution, segmental patterns, metaphyseal extension, or associated joint instability pose significant challenges to stable fixation. While intramedullary nailing remains the standard treatment for diaphyseal fractures, certain fracture configurations are prone to mechanical failure, delayed union, or non-union when treated with a single fixation modality. Dual fixation using a combination of intramedullary nailing and plate augmentation may provide superior biomechanical stability and improve healing outcomes. OBJECTIVES: To evalua

An unusual case of neural palatal swelling.

Neural tumors in the oral cavity occur both in the soft tissues and in the jaw bones. They occur as painless, smooth surface swelling in the soft tissues of the mouth, exhibiting a slow rate of growth and mild expansion of the cortical plates. Here, we report a rare case of a cellular variant of schwannoma in a young Indian female patient who presented with an asymptomatic nodule in the mid-palatine raphe region of the hard palate, mimicking a fibroma, thus creating a dilemma in the diagnosis.

Extension of the frontal sinus into the roof of the optic canal: a cadaveric case report.

This case reports a bilateral asymmetrical posterior extension of the frontal sinuses into the orbital roof with an unusual expansion into the roof of the optic canal in a 55-year-old male cadaver. The posterior extensions of the sinus were lined by mucoperiosteum and were separated from the underlying orbital contents and optic nerve by a thin plate of bone. This knowledge of an unusual anatomic variation of the frontal sinus may help understand better the ocular and intracranial complications associated with frontal sinus pathologies.
