The effect of foot placement on sit to stand in healthy young subjects and patients with hemiplegia.

OBJECTIVE: To determine the effect of altering the foot placement of the dominant limb in young healthy subjects and the uninvolved limb of subjects with hemiplegia on their ability to perform sit to stand (STS). DESIGN: Controlled biomechanical experiment. SETTING: Research laboratory of a university health science center. PARTICIPANTS: Nonrandom convenience sample of 10 healthy and 10 subjects with hemiplegia. Respective mean ages were 26 and 59 years. All patients with hemiplegia could ambulate and STS independently. The mean time since the stroke was 3.6 years. INTERVENTIONS: Subjects …

Single stage correction with external fixation of the ulcerated foot in individuals with Charcot neuroarthropathy.

The ulcerated foot in individuals with Charcot neuroarthropathy presents a complex problem when correction of the deformity is necessary but the presence of infection precludes the use of internal fixation. We reviewed 11 patients with midfoot Charcot neuroarthropathy, collapse, and ulceration who were at risk for amputation. These patients underwent operative debridement, corrective osteotomy, external skeletal fixation and culture-directed antibiotic therapy as a limb salvage procedure. Patients were transitioned from the external fixator (average 57 days) to total contact casting (average 1…

The paper grip test for screening on intrinsic muscle paralysis in the foot of leprosy patients.

Plantar intrinsic foot muscles provide structure to the foot during walking and thus regulate mechanical foot sole stresses. When paralyzed, for instance in leprosy patients with neuropathy of the distal part of the tibial nerve, there is a high prevalence of plantar ulceration and deformities, especially when muscle weakness goes together with loss of foot sole sensibility. These patients should get immediate care involving education, special footwear and reconstructive surgery before further foot impairment and deformity becomes manifest. Thus far, in leprosy patients little attention is pai…

Integration of plantar soft tissue stiffness measurements in routine MRI of the diabetic foot.

OBJECTIVE: A method of measuring mechanical properties of plantar soft tissues in vivo by integrating a photoelastic technique into an MRI system is presented. BACKGROUND: Plantar tissue stiffening in the diabetic foot is associated with development of ulcers at the highest-pressure sites. MRI is used as an effective means of demonstrating tissue damage caused by diabetic ulceration. DESIGN: The new measurement method uses the indentation of a sphere into the plantar tissue, which is simultaneously scanned by MRI. The indentation force is obtained using the photoelastic technique while tissu…

Flat and cavus foot, indexes of obesity and overweight in a population of primary-school children.

BACKGROUND: Obesity and overweight during the developmental years are related to certain dysmorphisms of the foot, in particular with flat foot. METHODS: The authors evaluated the incidence of this dysmorphism in a population of 243 primary-school pupils, (3rd and 4th class - age range 8-10 years) of ULSS no. 16 in Padua and, at the same time, determined the index of overweight and obesity and the extent and quality of exercise practised. From the anthropometric data collected, it was calculated that the Cole index (ideal ratio between the ideal BMI at the 50 degrees percentile of weight and …

[Optimization of surgical treatment of suppurative-necrotic lesions of the foot in patients with diabetes mellitus].

The analysis of treatment results in 559 patients with diabetes mellitus complicated by foot pyonecrotic lesions was carried out. Surgical treatment was performed in 448 patients considering route of infection spread (basic group), conventional methods of treatment were used in 111 patients (control group). It is shown that one of the causes of unsuccessful treatment results was the infection spread from foot to leg by toes tendons and synovial sheaths. The study of microbial contamination in wound exudate, distal and proximal sites of tendons revealed a significant difference in proximal-dist…

Onychomycosis and other superficial fungal infections of the foot in the elderly: a pan-European survey.

BACKGROUND: The escalating ageing population in the western world has led to an increased incidence of superficial fungal infections. The most common infections include onychomycosis, tinea pedis and tinea cruris. With the increasing life expectancy, the prevalence of onychomycosis and other superficial fungal infections is likely to increase further without adequate prevention and treatment. OBJECTIVE: To study the prevalence of foot mycoses in Europe. METHOD: The Achilles project represents a survey of 90,085 subjects from 16 European countries. RESULTS: Approximately half of the total sc…

Pathologic studies of fatal cases in outbreak of hand, foot, and mouth disease, Taiwan.

In 1998, an outbreak of enterovirus 71-associated hand, foot, and mouth disease occurred in Taiwan. Pathologic studies of two fatal cases with similar clinical features revealed two different causative agents, emphasizing the need for postmortem examinations and modern pathologic techniques in an outbreak investigation.

Delineation of a complex karyotypic rearrangement by microdissection and CGH in a family affected with split foot.

We report on a male patient and members of his family with additional material in chromosome 3. This derivative chromosome 3 was transmitted from his mother who had a complex rearrangement between chromosomes 2, 3, and 7. It was possible to delineate her chromosomal rearrangement by microdissection and reverse painting and to exclude these aberrations from being responsible for neonatal deaths and several abortions in this family. Two members of this family suffer from ectrodactyly or split hand/foot malformations (SHFM) of the feet which possibly correlates with the derivative chromosome 7 co…

Risk factors that may adversely modify the natural history of the pediatric pronated foot.

Flatfoot is one of the most common conditions seen in pediatric podiatry practice. There is no universally accepted definition for flatfoot. Flatfoot is a term used to describe a recognizable clinical deformity created by malalignment at several adjacent joints. Clinically, a flatfoot is one that has a low or absent longitudinal arch. Determining flexibility (physiologic) or rigidity (pathologic) is the first step in management. A flexible flat foot will have an arch that is present in open kinetic chain (off weight-bearing) and lost in closed kinetic chain (weight-bearing). A rigid flatfoot h…

Infection of foot ulcers with Staphylococcus aureus associated with increased mortality in diabetic patients.

Diabetic patients with foot ulceration have a poorer prognosis than those without ulceration. The reason for this is unclear, but there is considerable interest in the putative links between infection and atherogenesis, and it is notable that diabetic foot ulcers (DFU) are often infected with Staphylococcus aureus and the main cause of death in DFU patients is ischaemic heart disease. We examined the 5 year survival of 71 diabetic patients who presented with foot ulcers that were newly infected (Sa group, n = 56) or not infected at all during the study period (non-Sa group, n = 15) with S. aur…

Os vesalianum as a cause of lateral foot pain: a familial case and its treatment.

This is a rare case of bilateral symptomatic os vesalianum in a 13-year-old girl whose mother had the same condition unilaterally. We performed osteosynthesis and bone grafting instead of a simple resection to preserve the peroneus brevis tendon, with excellent results.

[Transfer of posterior tibial muscle to the back of the foot: an original procedure for fixing the transplant].

PURPOSE OF THE STUDY: The purpose of the study was to adjust a palliative transfer using the Tibialis Posterior (TP) tendon in case of foot dorsiflexion palsy. The surgical procedure emphasized some functional features, in an attempt to lessen parasitical motion of the foot in abduction or in adduction when patients bear the ankle joint in dorsiflexion, and to strengthen the transfer. MATERIAL AND METHODS: The surgical technique was carried out four times in patients, with a follow-up of six to eighteen months. One 20 cm long leg incision, on the antero-lateral part of tibia, up to the retina…

Biomedical gait evaluation of the immediate effect of orthotic treatment for flexible flat foot.

Flexible flat foot subjects attending the prosthetics and orthotics units come with prescriptions from orthopaedic surgeons for arch supports. Usually a pair of thermoformed plastic inserts are fabricated and fitted to treat the patients. However the effect of the orthotic treatment is not yet clear. A motion analysis system with two video cameras placed on the lateral and rear sides of the subject together with one force platform was used to investigate the immediate effects of the orthotic treatment. The force platform collected force data and the two cameras captured two-dimensional displac…

Foot and ankle reconstruction using the radial forearm flap: a review of 25 cases.

Twenty-five patients underwent soft-tissue reconstruction of the different anatomic regions of the foot and ankle using the microvascular radial forearm free flap. The patients, 19 men and 6 women, ranged in age from 3 to 80 years (mean, 48.4 years). Indications for the surgery included diabetes and/or vascular insufficiency (10 patients), trauma (9 patients), tumor (3 patients), gunshot wound (2 patients), and burn (1 patient). Osteomyelitis occurred in patients with traumatic (3 patients) and diabetic (3 patients) wounds. The weight-bearing surface of the foot was involved in eight patients.…
