BACKGROUND: Atrial fibrillation (AF) is more prevalent in endurance athletes (EAs) than in the general population, but identifying athletes at greatest risk remains challenging.
OBJECTIVES: This study sought to develop an AF risk assessment tool from associations between AF and electrophysiologic, imaging, and genetic markers of atrial cardiomyopathy in EAs.
METHODS: EAs and nonathletic control subjects (NAs) underwent comprehensive phenotyping with the use of cardiopulmonary exercise testing, echocardiography, 12-lead electrocardiography (ECG), Holter monitoring for premature atrial contrac…

BACKGROUND: Occluding the left atrial appendage (LAA) during cardiac surgery reduces the risk of ischemic stroke; nonetheless, it is currently only softly recommended with "may be considered" by the current guidelines. We aimed to assess thromboembolic risk after LAA amputation in patients with atrial fibrillation (AF) and aortic stenosis undergoing biological aortic valve replacement (AVR) as primary cardiac surgery.
METHODS: Two cohorts were generated retrospectively: patients with AF undergoing AVR alone or combined with revascularization either with LAA amputation or without. Data were co…

OBJECTIVES: This study sought to identify whether atrial strain could be used as an imaging biomarker to predict atrial fibrillation (AF). … ƐR ≤21.4%, ƐCd >10.4%, and CHARGE-AF (Cohorts for Heart and Aging Research in Genomic Epidemiology Atrial Fibrillation) score >7.8% as discriminatory for AF, with a 13-fold greater hazard of AF (p < 0.001) in patients with increased clinical risk and reduced ƐR. However, validation is needed for these strain cutoffs for detection of AF.
CONCLUSIONS: Left atrial str …

• Massive LA thrombus is rare in the absence of anticoagulation and mitral stenosis. • Direct-acting oral anticoagulants are widely used in nonvalvular AF. • Inadequate dosing of direct-acting anticoagulants may be difficult to recognize. • Consequences of inadequate dosing may be severe.

BACKGROUND: Epicardial adipose tissue located close to the atrial wall can change the electric conduction of the left atrium, potentially leading to atrial fibrillation (AF). The aim of this study was to assess whether an increased atrial adipose tissue mass posterior to the left atrium is related to AF independent of demographical and cardiovascular risk factors.
METHODS AND RESULTS: Two hundred patients with AF and 200 patients without AF who underwent computed tomographic angiography were included. The posterior left atrial adipose tissue mass was quantified on computed tomographic angiogr…

AIMS: The low efficacy rates reported for conventional catheter ablation of longstanding persistent atrial fibrillation (LPAF) have led to the development of alternative techniques such as minimal invasive surgical ablation, aiming for durable and contiguous transmural lesions. The aim was to evaluate the efficacy and safety of total thoracoscopic epicardial left atrial ablation (TELA-AF) procedures in a prospective study of severely symptomatic patients with either drug-resistant AF and/or failed attempts of catheter ablation.
METHODS AND RESULTS: The TELA-AF surgical technique includes pulm…

BACKGROUND: Transient receptor potential (TRP) family plays important roles in cardiovascular system. We investigated the relationship between transient receptor potential channel subfamily M6 (TRPM6) and atrial fibrosis in rheumatic heart disease patients with atrial fibrillation (AF).
METHODS: The right atrial tissue samples were obtained from 64 patients with rheumatic heart diseases who underwent heart valve replacement surgery, and composed of 34 sinus rhythm (SR) patients and 30 AF patients. Hematoxylin and Eosin (HE) staining was used to observe cross-sectional area (CSA) of myocardial…

BACKGROUND: Atrial fibrillation (AF) is common among adult patients with an atrial septal defect (ASD). Catheter ablation or the Maze procedure can be considered for AF before or concurrently with ASD closure. However, the fate of preoperative AF is not well established. This study examined the postoperative course of patients with AF before undergoing ASD correction.
METHODS AND RESULTS: The 471 patients (131 men, 42 ± 14 years) underwent transcatheter closure (n=237, 50%) or surgical repair (n=234, 50%) of an ASD. ECG and Holter monitoring were used to document preoperative and postoperativ…

BACKGROUND: Atrial tachycardia (AT) is commonly encountered after atrial fibrillation (AF) ablation. But no study exclusively on noncavotricuspid isthmus-dependent right AT (NCTI-RAT) post-AF ablation has been reported. The present study aims to describe its prevalence, electrophysiological mechanisms, and ablation strategy and to further discuss its relationship with AF.
METHODS: From July 2006 to November 2009, 350 consecutive patients underwent catheter ablation for paroxysmal AF. A total of seven patients (2.0%) developed NCTI-RAT after left atrium ablation for AF. In these highly selecte…

BACKGROUND: The total atrial conduction time (TACT) is an independent predictor of atrial fibrillation (AF). A new transthoracic echocardiographic tool to determine TACT by tissue Doppler imaging (PA-TDI (the time from the initiation of the P wave on the ECG (lead II) to the A' wave on the lateral left atrial tissue Doppler tracing)) has been developed recently.
OBJECTIVE: To test the hypothesis that measurement of PA-TDI enables prediction of new-onset AF.
METHODS: 249 Patients without a history of AF were studied. All patients underwent an echocardiogram and the PA-TDI interval was measure…

BACKGROUND: Surgical ablation is one of the most effective techniques for treatment of atrial fibrillation. Stable sinus rhythm is achieved and effective atrial contractions are restored in the majority of patients. The aim of the prospective study is to assess the presence of atrial transport function after surgical ablation of atrial fibrillation using cryoenergy and to evaluate predictors of the procedural success.
METHODS AND RESULTS: Between January 2005 and September 2006, 100 consecutive patients underwent left atrium cryoablation as a concomitant cardiac procedure. Their mean age was …

This report describes a fatal case of left atrial-esophageal fistula occurring in a 72-year-old man after a radiofrequency catheter ablation of paroxysmal atrial fibrillation. Catheter ablation was performed around the pulmonary vein using an 8-mm-tip electrode (60 W or 55 degrees C) guided by a 25-mm circular catheter. On day 22 of follow-up, the patient presented with seizures followed by hematemesis due to left atrial-esophageal fistula. His clinical condition deteriorated, and he died of speticemia. Thus, left atrial-esophageal fistula is a sever complication of radiofrequency catheter abl…

Computer (robotic) enhancement has emerged as a facilitator of minimally invasive cardiac surgery and has been used to perform portions of intracardiac procedures via thoracotomy incisions. This report describes the use of the da Vinci surgical system in two totally endoscopic ("closed chest") cardiac operations: atrial septal defect closure and pulmonary vein isolation of atrial fibrillation. ASD closure: Fifteen patients underwent repair of a secundum-type atrial septal defect or patent foramen ovale by a totally endoscopic approach, utilizing the da Vinci robotic system. We report an arrhythmic complication in two patients in whom a procedure directed at isolating one or two pulmonary veins had been performed. The complication was related to pulmonary vein disconnection scars after ablation. Both patients developed new clinical tachycardia (atypical atrial flutter) secondary to a reentrant phenomena in the vicinity of a previously ablated pulmonary vein.

A patient with a surgically repaired double outlet right ventricle developed AF 6 months after successful RF catheter ablation of typical atrial flutter. Guided by a 64-electrode basket catheter, the patient's AF was found to be dependent on an atypical atrial flutter circuit rotating around the fossa ovalis. Successful RF catheter ablation was performed by creating a line of conduction block from the superior vena cava to the fossa ovalis.
