Fifty Years of Care and Carcerality.

AbstractNancy Dubler founded the Montefiore Bioethics Consultation Service in the late 1970s on the cusp of two epidemics: AIDS and mass incarceration. In New York these two phenomena intersected with devastating consequences. By the late 1980s, the HIV infection rate among incarcerated people in New York State was the highest in the nation. Hospitals across the state built locked units staffed by correctional officers to manage the influx of incarcerated patients dying from AIDS-units that persist to this day. Dubler's earliest scholarship reflects this ethically fraught context. In this arti…

It changes your mind: doing gender by proxy and gender fluidity in the context of dementia.

People with dementia progressively lose the ability to act and make decisions independently, requiring a prolonged period of supported decision making and possibly complete decision making by proxy in the final stages of the disease. Sometimes, the decision-making process is complicated by the fact that people with dementia have different personalities, preferences, desires, and values compared to earlier in their lives, creating a conflict between earlier values and preferences and currently expressed ones. Most examinations of this process examine caregivers' proxy decisions around medical c

Integrating Artificial Intelligence Competencies Across the Six ACGME Core Competencies: A Proposed Framework for Graduate Medical Education.

Although Artificial Intelligence (AI) is increasingly embedded in clinical practice and Graduate Medical Education (GME), AI-specific competencies are not yet explicitly represented as a distinct domain within the Accreditation Council for Graduate Medical Education's (ACGME) six core competency framework. The ACGME's ongoing consideration of AI in its Common Program Requirements revision process signals an emerging recognition of the need to address this gap. Over 1200 FDA-authorized AI medical devices, unregulated generative AI tools in clinical workflows, and a proliferating state legislati

Development and pilot validation of a European ethical framework for decision-making in dementia care.

BACKGROUND: Dementia care involves ethically complex situations across diverse clinical, social care, and policy contexts, yet practice-oriented ethical guidance remains limited. Existing ethical frameworks rarely capture the full complexity of practice across European contexts. This article presents the development and pilot external validation of an Ethical Framework for Decision-Making in Dementia Care, designed for professional caregivers. METHODS: Within COST Action CA21137 (Ethics in Dementia - EDEM), a multidisciplinary European consortium developed the framework through an iterative,

Brain-Hype and For-Profit Medical Devices.

Research into the functioning of the human brain is essential both for the biological assessment of the species and for our self-comprehension. However, following the "neuroscientific turn" in several areas of research, such as psychology, philosophy, and AI, interdisciplinary collaboration in this area gave birth to an important phenomenon that is still active today: "neuro-hype" or "brain-hype." In this paper, we will inquire about one particular aspect of this widespread phenomenon: for-profit neuro-related products. Due to the lack of a vast and robust literature on the topic, the first as

Relationship between susceptibility to neurodegenerative diseases and genetic factors in populations exposed to mercury in Medellín, Colombia.

INTRODUCTION: Neurodegenerative diseases such as Alzheimer’s and Parkinson’s are associated with genetic and environmental factors, including exposure to mercury, a heavy metal with neurotoxic effects. OBJECTIVES: To identify and analyze alleles and genes linked to neurodegenerative diseases in relation to mercury exposure in Colombia. MATERIALS AND METHODS: Scientific literature and population genotype data from public databases were reviewed, covering 94 Colombian adults genotyped under the CLM (Colombians from Medellín) reference. Cancer prevention in people experiencing homelessness: ethical considerations and experiences from the CANCERLESS project.

The incidence of cancer in Europe has been increasing in recent years. Despite this, cancer prevention has remained a low priority in health … of the most disadvantaged groups of the population. Hence, it is necessary to resort to relational bioethics that includes the ethics of care and solidarity and that recognizes the moral identity of socially excluded persons, reaffirming their position of equality in society. Relational autonomy therefore provides a broader conception by including the infl …

Human rights violations in normalizing procedures on intersex children.

Intersex children in Brazil are still subjected to "normalizing" surgical procedures and subsequent bodily interventions to make their bodies conform to binary views of sex. Resolution n. 1,664/2003 of the Brazilian Federal Council of Medicine legitimizes interventions upon intersex bodies, being the only national normative instrument that address the subject. However, the demands of international intersex political activism have denounced how early childhood interventions for sex designation mutilate children's bodies and violate a number of human rights. This research discusses how early, ir

Fatherlessness, sperm donors and 'so what?' parentage: arguing against the immorality of donor conception through 'world literature'.

Is biology and knowing biological ancestral information essential to the construction of identity? Bioethicist James David Velleman believes this is the case and argues that donor gamete conception is immoral because a portion of genetic heritage will be unknown. Velleman is critical of sperm donation and the absence of a biological father in donor-assisted families. His bioethical work, specifically the 2005 article 'Family History', is oft-cited in articles debating the ethics surrounding gamete donations and diverse family formations. However, I wonder to what extent Velleman's ethical stan

Fear of Illness and Convenient Access to Vaccines Appear to Be the Missing Keys to Successful Vaccination Campaigns: Analysis of the Factors Influencing the Decisions of Hospital Staff in Poland concerning Vaccination against Influenza and COVID-19.

The COVID-19 pandemic has lasted for two years as of 2022, and it is common knowledge that vaccines are an essential tool to mitigate the health, economic, and social fallout. Unfortunately, vaccine hesitancy is still a serious global problem, both in the general population and among healthcare workers. The authors used an original questionnaire to conduct an anonymous survey study in the University Clinical Hospital in Wrocław, Poland, in April and May of 2021 after acquiring consent from the Medical University of Wrocław, Poland Bioethical Committee. The study results demonstrate that, to a

Fiftieth Anniversary of Uncovering the Tuskegee Syphilis Study: The Story and Timeless Lessons.

This year marks the 50th anniversary of the uncovering of the Tuskegee syphilis study, when the public learned that the Public Health Servic … haracters convey the principles of research ethics more vividly than a dry account in a textbook of bioethics. The study spurred reforms leading to fundamental changes in the infrastructure of research ethics. The reason people fail to take steps to halt behavior that in retrospect everyone judges reprehensible is complex. Lack of imagination, rationalizati …

Challenging the dogma of the healthy heterozygote: Implications for newborn screening policies and practices.

Heterozygous (carrier) status for an autosomal recessive condition is traditionally considered to lack significance for an individual's health, but this assumption has been challenged by a growing body of evidence. Carriers of several autosomal recessive disorders and some X-linked diseases are potentially at risk for the pathology manifest in homozygotes. This minireview provides an overview of the literature regarding health risks to carriers of two common autosomal recessive conditions on the Recommended Uniform Screening Panel: sickle cell disease [sickle cell trait (SCT)] and cystic fibro

Inclusion of children in the initial conversation about their cancer diagnosis: impact on parent experiences of the communication process.

INTRODUCTION: Including children in medical conversations is considered the standard of care for children with cancer. However, previous qualitative research has raised concerns about how the child's presence impacts the parent's communication experience. The current study examines the frequency and impact of child presence during a serious medical conversation on the parent's communication experience in pediatric oncology. METHODS: Three hundred sixty parents of children newly diagnosed with cancer completed questionnaires assessing the child's presence during the initial conversation with t

Targeted Muscle Reinnervation: Considerations for Future Implementation in Adolescents and Younger Children.

Prosthetic options for patients with proximal upper limb absence are limited. Current above-elbow prostheses may restore basic motor functions for crucial activities, but they are cumbersome to operate, lack sensory feedback, and are often abandoned. Targeted muscle reinnervation is a novel surgical procedure that enhances the ability of patients with above-elbow amputations to intuitively control a myoelectric prosthesis. By transferring multiple severed peripheral nerves to a robust target muscle, targeted muscle reinnervation restores physiologic continuity and enables more intuitive prosth

Ethical Approach to Prevention of Schizophrenia - Concepts and Challenges.

Patients with schizophrenia, nowadays chronic, frequently disabling mental disorder, get initial treatment after detection of a psychotic episode, seemingly late, potentially preventable stage of illness. As our knowledge about the nature of schizophrenia and other diseases of the spectrum is growing, so are the early interventions becoming more possible, and it is important to conceptualize the clinical, legal and moral issues emerging with new preventive treatments. Every intervention, especially in pre-clinical population, demands a careful risk-benefit assessment and having basic bioethica

The argumentation draws upon recommendations by the working group "Bioethics and Intersex" within the German Network DSD/Intersex, which are presented in detail.
