Bronchopulmonary foregut malformation presenting as extralobar pulmonary sequestration associated with a bronchogenic cyst: an unusual clinical and radiological feature in an adolescent patient.

We present a case of symptomatic complex bronchopulmonary foregut malformation (BPFM), including extralobar pulmonary sequestration and a bronchogenic cyst, in the left anterior mediastinum of a 15-year-old boy. Preoperative computed tomography showed a cystic mass with heterogeneous enhancement of adjacent soft tissue components and pleural effusion. We suggested the infected bronchogenic cyst as the first impression. However, pathological examination after surgical resection revealed extralobar pulmonary sequestration and a bronchogenic cyst with unusual manifestation, which was located in t…

Complete resection of an intrapericardial bronchogenic cyst with median sternotomy showing high CA19-9 level fluid sampling by EBUS-TBNA: a case report.

BACKGROUND: An intrapericardial bronchogenic cyst (IBC) is rare and compresses the surrounding organs, causing symptoms. Resection of an IBC leads to an improvement in symptoms. We completely resected an IBC by performing median sternotomy and the symptom improved, and we described the clinical course. CASE PRESENTATION: A 48-year-old man with palpitation and chest discomfort was referred to our institution. Chest computed tomography (CT) revealed a 42 × 35 × 32-mm cystic mass attached to the right pulmonary artery and right bronchus. Chest CT also revealed that this cystic mass compressed th…

A 4-Month-Old Girl with Persistent Respiratory Distress and Multiple Admissions to the Pediatric Intensive Care Unit Due to a Congenital Bronchogenic Cyst.

BACKGROUND Bronchogenic cysts are foregut duplication cysts that result in congenital cysts of the tracheobronchial tree. They can be fatal especially, if they present in early infancy. Persistent respiratory distress is the most frequent reported presentation. Histologically, bronchogenic cysts are unilocular fluid-filled lesions that are composed of respiratory epithelium. This report presents the case of a 4-month-old girl with persistent respiratory distress and multiple admissions to the Pediatric Intensive Care Unit (PICU) due to a congenital bronchogenic cyst. CASE REPORT We describe a …

Single-port robotic resection of a superior mediastinal bronchogenic cyst in a 6-year-old child using the da Vinci SP system: a case report.

BACKGROUND: Bronchogenic cysts often require surgical excision in children because of the risk of progressive enlargement and complications. Although minimally invasive surgery is preferred, pediatric thoracic procedures remain technically challenging because of limited intrathoracic working space. CASE DESCRIPTION: A 6-year-old girl with an asymptomatic mediastinal mass demonstrated interval growth of a cystic lesion in the right superior mediastinum on serial imaging. Single-port robotic resection was performed using the da Vinci SP system through a single intercostal incision with low-pres…

Four cases of developmental foregut cysts.

Four patients with developmental foregut cysts were seen in the Singapore General Hospital between 1991 and 1994. Three had bronchogenic cysts while one had features consistent with both enteric and bronchogenic origin. Among those patients with bronchogenic cysts, only one was symptomatic, having presented with cough and dysphagia. Two other patients presented incidentally on chest radiographs done for pre-National Service enlistment while one patient was picked up when a repeat chest radiograph was done following a course of antibiotics. The diagnosis of our first patient with the symptomati…

Laryngeal cysts in infants and children--a pathologist's perspective (with review of literature).

OBJECTIVES: To expose the rarity of the phenomena of congenital laryngeal cysts. Additionally, a discussion is presented in support of the basic similarities between laryngeal duplication cyst and bronchogenic cyst and a proposal to use the term 'bronchogenic cyst' (appended by the location) is put forth. MATERIALS AND METHODS: Three cases of cysts of the larynx, two in the neonate and one, in a child were discovered in the pediatric age group from the archives of the Department of Pathology, Kasturba Medical College, Mangalore. OBSERVATIONS: Laryngeal duplication cyst/bronchogenic cyst in a…

[Clinical and pathological features of congenital bronchial cyst].

OBJECTIVE: To study the clinical and pathological features of congenital bronchial cysts. METHODS: The clinical, X-ray, CT and histopathologic manifestations were retrospectively reviewed in 30 patients with congenital bronchial cysts. RESULTS: There were 23 patients with intrapulmonary bronchogenic cysts, 5 patients with mediastinal bronchogenic cysts, and 2 patients with ectopic bronchogenic cysts. Of the 30 patients, 18 were liquid cysts, 7 air cysts, and 5 multiple pulmonary cysts. Intrapulmonary congenital bronchial cysts were diagnosed always due to infection. Nineteen of 23 patients (…

Role of Endobronchial Ultrasound-guided Transbronchial Needle Aspiration in the Diagnosis and Management of Mediastinal Cyst.

BACKGROUND: Foregut cysts account for >50% of cystic lesions in the mediastinum, of which bronchogenic cysts are most common. Surgical resection is the most definitive approach for its diagnosis and treatment. A recent systematic review, however, suggests that endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) has a role in the management of bronchogenic cyst. We report our experience with EBUS-TBNA in the diagnosis and management of bronchogenic cysts. METHODS: Medical records of patients with evidence of mediastinal cysts who underwent EBUS-TBNA between 2008 and 20…

Bronchogenic cancer: It still exists.

OBJECTIVES/HYPOTHESIS: Branchial cleft cysts rank among the most common differential diagnoses of cystic cervical masses. Rarely, classic cystic structures exhibit a squamous cell carcinoma differentiation that represents a bronchogenic carcinoma. The existence of bronchogenic carcinoma is controversial due to the lack of systematic immunohistologic workup. The present study aimed to identify the clinical and immunohistologic features of bronchogenic carcinoma to clearly distinguish this entity from other cystic cervical masses. METHODS: Immunohistologic (epidermal growth factor receptor; cyt

[Vestigial cysts of the anterior intestine of unusual localization. Report of two cases].

Bronchogenic cysts and enteric cysts both result from an aberration of development of the anterior gut. Their usual location is the mediastinum. The abdominal or retroperitoneal location of such cysts is rare and raises problems in terminology and pathogenesis. We report two cases of an unusual location of bronchogenic and enteric cysts. We also recall the criteria of diagnosis and the pathogenesis.

Benign developmental cysts of the mediastinum.

The examination of 40 benign developmental cysts of the mediastinum from the surgical pathology files supports the previous conclusions in the literature about these lesions. In distinguishing bronchogenic from esophageal cysts, the lining epithelium is not helpful, but the presence of two muscle layers in esophageal cysts and bronchial glands or bronchial cartilage in bronchogenic cysts enables categorization in the majority of cases. Gastroenteric cysts are lined by intestinal epithelium and are further distinguished by their location in the posterior mediastinum and association with other d…

In-flight seizures and fatal air embolism: the importance of a chest radiograph.

OBJECTIVE: To describe for the first time, to our knowledge, a case of recurrent in-flight-dependent seizures related to commercial airline flight in which the patient experienced a fatal air embolism secondary to a giant bronchogenic cyst. DESIGN: Case report. SETTING: University hospital. PATIENT: A female airline passenger presented with a seizure, then unconsciousness and death. The patient had experienced 2 previous episodes of in-flight seizures without any sequelae. RESULTS: The patient had an air embolism proved by findings on cranial computed tomography (CT) and a CT perfusion stu…

[A case report of pericardial diverticulum in the upper mediastinum: surgical treatment of two cases].

Pericardial cyst and diverticulum are commonly found in areas of cardiophrenic angles. In this article, we reported on two operated cases (54-year-old female, 43-year-old male) with pericardial diverticula in upper mediastinum. On their chest CT films, well circumscribed cystic lesions were seen in the right (case 1) and left (case 2) upper mediastinum. Based on findings of imaging modalities such as CT, MRI and transesophageal ultrasonography, the lesions were suspected of benign cystic tumors such as bronchogenic cyst, lymphatic cyst, esophageal cyst and pericardial cyst. However, definite d…

Two concomitant congenital lung malformations mimicking a paravertebral mass.

We report the case of a 47-year-old woman who presented with the unique asymptomatic malformation of an extralobar pulmonary sequestration communicating with a bronchogenic cyst. Despite computed tomography and magnetic resonance imaging of the chest, the diagnosis could not be established before a left thoracotomy was performed. The sequestrated lobe and bronchogenic cyst were then successfully resected.

[Posterior Mediastinal Cyst Resection during Cardiac Surgery through Median Sternotomy:Report of a Case].

A 47-year-old man presented with hematuria. Computed tomography( CT) showed a posterior mediastinal cyst. Chest magnetic resonance imaging s … full sternotomy under cardiac arrest. Pathological examination revealed that cyst was bronchogenic. Bronchogenic cyst has malignant potential and it is very difficult to resect after presenting symptoms. We need to consider the differential diagnosis, the timing of operation and operative strategy.

Challenge in preoperative diagnosis of retroperitoneal mucinous cyst in a pediatric patient.

Mucinous cystic lesions of the retroperitoneum can be either neoplastic or non-neoplastic. It is very important to make a correct diagnosis, or at least, an accurate classification, to proceed with an optimal treatment strategy. In spite of advantage of ultrasound and X-ray image examinations, it is still a challenge to make differential diagnosis of retroperitoneal mucinous cyst from gangliocytoma because both tumors have similar density under the image assessment. In this article, we reported an asymptomatic 8-year-old boy with multiple bronchogenic cysts in both lung and adrenal area on the…
