OBJECTIVES: The aim of this study was to evaluate prospectively the effect of late atrial septal defect closure on cardiac output and oxygen delivery in patients who have undergone the Fontan procedure.
BACKGROUND: An adjustable atrial septal defect is incorporated in patients undergoing the Fontan procedure who have increased pulmonary vascular resistance or poor ventricular function, or both. After the Fontan procedure, the atrial septal defect is test occluded. Patients with mean right atrial and pulmonary artery pressures > 15 mm Hg are discharged with the atrial septal defect open.

Iatrogenic femoral arteriovenous fistulas (AVFs) are an uncommon but potentially significant vascular complication following catheter-based cardiac procedures, particularly those requiring femoral access. Although most AVFs remain small and hemodynamically insignificant, high-flow fistulas may result in substantial arteriovenous shunting, leading to increased cardiac preload, reduced systemic vascular resistance, and high-output heart failure. Early recognition is essential to prevent progressive hemodynamic deterioration and cardiovascular compromise. The clinical presentation of femoral AVFs

BACKGROUND: Many heart diseases can result in reduced pumping capacity of the heart muscle. A mismatch between ATP demand and ATP production of cardiomyocytes is one of the possible causes. Assessment of the relation between myocardial ATP production (MVATP) and cardiac workload is important for better understanding disease development and choice of nutritional or pharmacologic treatment strategies. Because there is no method for measuring MVATP in vivo, the use of physiology-based metabolic models in conjunction with protein abundance data is an attractive approach.
METHOD: We developed a co

BACKGROUND: Cardiac arrest and cardiogenic shock are medical emergencies with poor survival rates despite improvements in advanced life support and resuscitation techniques.
CASE SUMMARY: We present the case of a 44-year-old woman with ischaemic cardiomyopathy who suffered a prolonged out-of-hospital cardiac arrest due to ventricular tachycardia. Despite high-dose catecholamines and vasopressin, the patient had persistent hypotension and elevated vasoactive-inotropic requirements. Haemodynamic monitoring revealed features consistent with post-arrest vasoplegia. Two low-dose boluses of methyle

BACKGROUND: Left ventricular (LV) septal myocardial pacing (LVSP) is present in the areas where left bundle branch (LBB) capture is not feasible with high pacing output (general LVSP), or in LV subendocardial locations, where it transits from LBB pacing (LBBP) during the decremental output pacing (LV subendocardial pacing [LVSeP]).
OBJECTIVE: This study aimed to compare electrical ventricular synchrony and acute hemodynamic response between LBBP and LVSeP.
METHODS: Consecutive cardiac resynchronization therapy (CRT) patients with transition from nonselective left bundle branch capture to LVS

INTRODUCTION: Cardiovascular disease is the most frequent cause of death in kidney transplant patients. High-flow arteriovenous fistula (AVF) increases cardiac output and may contribute to hyperkinetic heart failure. AVF follow-up is not implemented in kidney transplant patients. The aim of this study was to reduce AVF blood flow in a group of patients with a high-flow AVF following kidney transplantation to reduce cardiac strain.
METHODS: This prospective study was performed in kidney transplant patients who had a vascular access created before transplantation. The AVF of these patients was

It has been long recognized that there are 2 methods for inducing VF (ventricular fibrillation) with electrical currents‥ These are: (1) delivering a high-charge shock into the cardiac T-wave, and (2) delivering lower level currents for 1-5 seconds. Present electrical safety standards are based on this understanding. We present new data showing a 3(rd) mechanism of inducing VF which involves the steps of delivering sufficient current to cause high-rate cardiac capture, causing cardiac output collapse, leading to ischemia, for sufficiently long duration, which then lowers the VFT (VF threshold)

BACKGROUND: Sudden Infant Death Syndrome (SIDS) mechanisms of death remains obscured. SIDS' Triple Risk Model assumed coexistence of individual subtle vulnerability, critical developmental period and stressors. Prone sleeping is a major risk factor but provide no clues regarding the mechanism of death. The leading assumed mechanisms of death are either an acute respiratory crisis or arrhythmias but neither one is supported with evidence, hence both are eventually speculations. Postmortem findings do exist but are inconclusive to identify the mechanism of death. WHAT DOES THE PROPOSED HYPOTHESI

Fenpropathrin, a pyrethroid insecticide, has been widely used for many years in agricultural fields. It works by disturbing the voltage-gated sodium channel, leading to paralysis and the death of the target animal. While past studies have focused on neurodegeneration following fenpropathrin poisoning in humans, relatively few pieces of research have examined its effect on other peripheral organs. This study successfully investigated the potential toxicity of fenpropathrin on the cardiovascular system using zebrafish as an animal model. Zebrafish larvae exposed to varying doses of fenpropathrin

Exercise-induced pulmonary haemorrhage (EIPH) is a common occurrence in race horses. Although blood in cases of EIPH has been suspected to originate from the bronchial circulation, which receives approximately 1% of the left ventricular output, physiological evidence has recently emerged to indicate that the pulmonary circulation, which receives the entire output of the right ventricle, is a more likely source. High transmural pulmonary capillary pressures have been shown to cause breaks in the capillary endothelium, basement membrane as well as in the alveolar epithelium. Blood constituents e

AIM: The aim of the study was to find out whether dilatation of the left atrium (LA) influences the intra-thoracic distribution of thermodilution pulmonary artery (TPA) catheter in either branch of pulmonary artery and compare the measured cardiac output.
METHODS: In this prospective study of 132 consecutive patients in a university hospital setting, LA size and ejection fraction was assessed by echocardiography, in the preoperative period. In 66 patients posted for coronary artery bypass grafting (Group 1), a standard anaesthesia regimen was used and TPA catheter was floated through the righ

BACKGROUND: Ventricular arrhythmias (VAs) are most frequently the onset of sudden cardiac death, and early detection is key for survival. Limited studies suggest that wrist-worn photoplethysmography (PPG) sensors can detect VA. Ring-type wearable PPG sensors may be more accurate; however, they have not yet been investigated.
OBJECTIVE: We aimed to show the sensitivity of a ring-type wearable PPG sensor to detect induced VA.
METHODS: Patients undergoing a procedure involving VA induction were included. The CART-I was positioned on the finger during the procedure, and PPG and reference electro

Neonatal cardiac surgery in severe hemophilia A remains rarely reported, with limited data guiding perioperative coagulation management during cardiopulmonary bypass. This report describes neonatal repair of supracardiac total anomalous pulmonary venous connection in genetically confirmed severe hemophilia A using a structured, assay-guided intermittent-bolus recombinant factor VIII strategy rather than continuous infusion. Perioperative management targeted supraphysiologic factor VIII activity, with serial monitoring of factor levels, activated clotting time, and rotational thromboelastometry

Although plasma norepinephrine concentration often is elevated in patients with essential hypertension, the relevance of this to the pathogenesis of the blood pressure elevation remains uncertain. If the high concentration of plasma norepinephrine truly indicates sympathetic nervous system overactivity, it should be accompanied by hemodynamic features suggestive of sympathetic cardiovascular stimulation. This proposition was tested, and in part confirmed, in a study performed on 21 patients who had mild essential hypertension and on 11 normal subjects. Among hypertensive patients, plasma norep

High-volume shunt flow after arteriovenous fistula (AVF) creation for hemodialysis can cause high-output heart failure. We used the Frame™ (Vascular Graft Solutions Ltd., Tel Aviv, Israel) external support, a stent, to limit vein dilatation and consecutive high-volume shunt in a 62-old female who underwent brachial-basilic upper arm transposition. After maturation, the shunt was used for dialysis and showed a plateauing flow volume 3 months after the operation. This case illustrates the safety and feasibility of this intervention when performed during AVF formation.

BACKGROUND/AIM: The impact of myocardial stressors such as high-fat diet (HFD) and pressure overload has been extensively studied. Toll-like receptor 4 (TLR4) deficiency has been suggested to have a protective role in response to these stressors, although some conflicting data exist. Furthermore, there is limited information about the role of TLR4 on cardiac remodeling in response to long-term exposure to stressors. This study aims to investigate the effects of TLR4 deficiency on cardiac histology and physiology in response to chronic stressors.
METHODS: TLR4-deficient (TLR4-/-) and wild-type

Constant cerebral blood flow (CBF) is vital to human survival. Originally thought to receive steady blood flow, the brain has shown to experience increases in blood flow during exercise. Although increases have not consistently been documented, the overwhelming evidence supporting an increase may be a result of an increase in brain metabolism. While an increase in metabolism may be the underlying causative factor for the increase in CBF during exercise, there are many modulating variables. Arterial blood gas tensions, most specifically the partial pressure of carbon dioxide, strongly regulate

Satisfactory hemodialysis access flow (Qa) is necessary for dialysis adequacy. However, high access flows are postulated to increase cardiac output (CO). The relationship between Qa and CO is not well defined. The purpose of this study was to observe the relationship between Qa and CO and to evaluate the effect of blood volume change (BVdelta) on Qa and CO during hemodialysis (HD). Measurements of Qa and CO (ultrasound dilution; Transonics Monitor, Ithaca, NY) were performed sequentially at baseline in 18 patients (13 forearm arteriovenous fistulae, 5 Gore-Tex grafts) and after an intervention

OBJECTIVE: To review current concepts in the management of patients with heart failure.
DATA SOURCES: A review of articles reported on acute and chronic heart failure.
SUMMARY OF REVIEW: Heart failure has been defined as that state which occurs when the heart fails to maintain the needs of the body despite a satisfactory venous return. While it has been divided functionally into high-output failure and low-output failure, it is often used to describe patients with left ventricular low-output failure and is divided into systolic or diastolic heart failure, depending on left ventricular ejecti
