Implementation of case-based collaborative learning in radiology residency: experience report of an international educational experience adapted to the Brazilian context.

BACKGROUND: Contemporary medical education increasingly incorporates active learning strategies to promote integration between foundational sciences and clinical practice and to support the development of diagnostic reasoning. Case-Based Collaborative Learning (CBCL) combines principles of team-based learning, problem-based learning, and case-based learning through structured preparation and collaborative discussion of clinical cases. In this context, this experience report describes the adaptation and implementation of a CBCL-based educational activity focused on clinical-radiological reasoni

Preventable diagnostic errors of lower gastrointestinal perforation: a secondary analysis of a large-scale multicenter retrospective study.

BACKGROUND: Lower gastrointestinal perforation (LGP) is an acute abdominal condition associated with a high mortality rate. Timely and accurate diagnosis is crucial. Nevertheless, a diagnostic delay has been estimated to occur in approximately one-third of the cases, and the factors contributing to this delay are yet to be clearly understood. This study aimed to evaluate the diagnostic process for appropriate clinical reasoning and availability of image interpretation in cases of delayed diagnosis of LGP. METHODS: A secondary data analysis of a large multicenter retrospective study was conduc…

A decision support system for upper limb rehabilitation robot based on hybrid reasoning with RBR and CBR.

The rehabilitation robot can assist hemiplegic patients to complete the training program effectively, but it only focuses on helping the patient's training process and requires the rehabilitation therapists to manually adjust the training parameters according to the patient's condition. Therefore, there is an urgent need for intelligent training prescription research of rehabilitation robots to promote the clinical applications. This study proposed a decision support system for the training of upper limb rehabilitation robot based on hybrid reasoning with rule-based reasoning (RBR) and case-ba

Probabilistic reasoning in obsessive-compulsive and delusional disorders.

BACKGROUND: Delusional disorder (DD) and obsessive-compulsive disorder (OCD) have been investigated in previous studies using probabilistic reasoning paradigms and abnormalities in each group have been reported. No study to date has compared results between these groups. This study compares patients with these disorders with those who have both phenomena. METHODS: Thirty subjects with DD, 29 with OCD and 16 with obsessive and delusional features were compared with 30 normal controls in a study of probabilistic reasoning using two different computer-based tasks involving a Bayesian paradigm.

I want to believe: delusion, motivated reasoning, and Bayesian decision theory.

Introduction: Several arguments suggest that motivated reasoning (occurring when beliefs are not solely shaped by accuracy, but also by other motives such as promoting self-esteem or self-protection) is important in delusions. However, classical theories of delusion disregard the role of motivated reasoning. Thus, this role remains poorly understood. Methods: To explore the role of motivated reasoning in delusions, here we propose a computational model of delusion based on a Bayesian decision framework. This proposes that beliefs are not only evaluated based on their accuracy (as in classical t

Quality assessment of large language model-generated prior authorization letters in nephrology.

BACKGROUND: Prior authorization (PA) is a major source of administrative burden, treatment delay, and clinician burnout. Artificial intelligence (AI), particularly large language models (LLMs), is increasingly used to assist with clinical documentation, yet its reliability for payer-facing administrative tasks remains uncertain. OBJECTIVE: To evaluate the quality of PA letters drafted by ChatGPT-5 for commonly used medications requiring PA in nephrology. Quality was evaluated based on correctness and strength of clinical reasoning. METHODS: We created a single standardized prompt and applied…

Comparison among children with children with autism spectrum disorder, nonverbal learning disorder and typically developing children on measures of executive functioning.

It has been suggested that children with nonverbal learning disabilities (NLD) or Asperger's Syndrome (AS) may show difficulties with executive functioning. There were 3 groups in this study who completed a neuropsychological battery of visual-spatial, executive functioning, and reasoning tasks; AS (n = 37), NLD (n = 31), and controls (n = 40). Results indicated that children in both clinical groups scored within average limits on measures of spatial reasoning and verbal ability. Fluid reasoning was also found to be within average ranges for all groups. The AS group experienced significant pro

A Practical Guide to Becoming a Clinical Problem Solving Discussant.

Clinical problem solving (CPS) conferences provide a real-time view into diagnostic reasoning, yet faculty who serve as discussants often do so without formal guidance. This article offers a practical framework for clinicians preparing to take on the CPS discussant role, emphasizing that success depends less on reaching the correct diagnosis and more on structured reasoning. Preparatory steps include strengthening clinical analytical skills, observing experienced discussants, and practicing in low-stakes settings that foster comfort with speaking and thinking aloud. During the CPS session, dis

LungGPT: A unified multimodal system for interpretable diagnosis and clinical decision support of respiratory diseases.

Respiratory diseases cause significant morbidity, yet diagnosis remains labor intensive and dependent on physician expertise. Here, we present LungGPT, a unified multimodal system trained on 147 million tokens of domain-specific electronic health records from 125,917 participants. LungGPT comprises two modules: LungGPT-Dx for respiratory disease diagnosis and early warning of critical illness, and LungGPT-Ex for interpretable diagnostic reasoning and treatment recommendations. In large-scale evaluations, LungGPT-Dx achieves a macro-average area under the curve (AUC) of 0.852 (95% confidence in

Metabolic Syndrome in Musculoskeletal Practice: Clinical and Social Features.

INTRODUCTION: Metabolic syndrome is a well-established cluster of cardiovascular risk factors with increasing relevance to musculoskeletal pain, function, and rehabilitation. Beyond its recognised effects on cardiovascular health, its systemic biological consequences may influence pain mechanisms, tissue health, recovery, and rehabilitation outcomes. Understanding these effects may enhance clinicians' assessment, clinical reasoning, and management of individuals presenting with musculoskeletal disorders. METHOD: This narrative conceptual paper explores the potential biological mechanisms thro…

Cognition, prior aggression, and psychopathic traits in relation to impaired multimodal emotion recognition in psychotic spectrum disorders.

BACKGROUND: Psychopathic traits have been associated with impaired emotion recognition in criminal, clinical and community samples. A recent study however, suggested that cognitive impairment reduced the relationship between psychopathy and emotion recognition. We therefore investigated if reasoning ability and psychomotor speed were impacting emotion recognition in individuals with psychotic spectrum disorders (PSD) with and without a history of aggression, as well as in healthy individuals, more than self-rated psychopathy ratings on the Triarchic Psychopathy Measure (TriPM). METHODS: Eight

Implementation of problem-based learning pedagogy with integration of tamsulosin management in traditional urology residency training under the multidisciplinary team model.

BACKGROUND: Traditional urology residency programs often depend heavily on lectures, which can limit resident engagement and teamwork. Problem-based learning (PBL) in a multidisciplinary team setting has been shown to enhance clinical reasoning skills and improve knowledge retention. This article examines the effectiveness of a PBL approach, specifically focused on a tamsulosin module related to benign prostatic hyperplasia (BPH) in urology training. OBJECTIVES: To evaluate whether a PBL-based educational intervention improves pharmacology knowledge, clinical reasoning, and interprofessional …

Should we prioritize knowledge gain or process improvement when teaching diagnostic reasoning? Insights from how medical students engage with deliberate reflection.

INTRODUCTION: The complexity of diagnostic reasoning ensures there can be no singular educational intervention to perfect its teaching. The need to flexibly adapt to needs of trainees and learning environments demands deeper understanding of available strategies. Deliberate reflection offers one technique that is central to debate in this area, but the literature remains insufficiently clear about why it can be effective and, hence, what purposes it can serve. By better understanding how deliberate reflection works we can better design learning exercises that guide students toward clinical exp

AI-Mediated Assessment of Continuing Medical Education: The Case-based Learning Intelligence Credit System (CLICS) Framework.

Continuing medical education (CME) and continuing professional development (CPD) systems have traditionally relied on time-based credit allocation, using participation duration as a proxy for professional learning. Although administratively simple and scalable, this model does not reliably demonstrate whether physicians have engaged in meaningful learning, improved clinical reasoning, or critically appraised evidence. The emergence of generative AI creates an opportunity to rethink how physician learning is documented, assessed, and credited. This viewpoint proposes the Case-based Learning Int…
