Hyperdominant left anterior descending artery (LAD) is a rare anomaly in which the LAD gives rise to the posterior descending artery. Our case report describes an extreme case of hyperdominant LAD supplying the anterior, inferior, and partially the lateral walls of the left ventricle. It is crucial that physicians be aware of the possibility of extreme LAD dominance. A proximal occlusion of such a vessel might have catastrophic consequences with atypical presentation on electrocardiogram.

The coronary sinus (CS) can be imaged echocardiographically as a small tubular sonolucency in the posterior atrioventricular groove. To date, its importance to echocardiographers has been that CS dilatation usually signifies a persistent left superior vena cava. In this case report, we describe a rare patient who experienced complete fracture of a sirolimus-eluting stent that was implanted for a focally calcified lesion of the right coronary artery ostium. Although the fractured fragment migrated somewhere in the systemic circulation, fortunately there were no complications. Implantation for ostial lesions of the right coronary artery should be performed with great care.

We present unusual coronary-pulmonary collaterals in a 65-year-old CTEPH patient. Perfusion mapping of a dual-energy computed tomography (DECT) study revealed areas of right lung that were minimally perfused despite unilateral occlusion of the right pulmonary artery, leading to the discovery of coronary-pulmonary collaterals via invasive coronary angiography. A 42-year-old woman with an anomalous left coronary artery originating from the pulmonary artery and a fistula from the left coronary artery to the left ventricle was treated by aortocoronary bypass grafting of an autologous saphenous vein. The presence of an abundant collateral circulation and of fistulous communications between the left coronary artery and the left ventricle, in our opinion, was the reason that this patient remained asymptomatic until the age of 40 years. An anomalous origin of the entire coronary circulation from the right pulmonary artery is a very rare congenital malformation. We report on a male newborn presenting with hypoplastic aortic arch and coarctation of the aorta in whom a previously undetected single coronary artery originating from the right pulmonary artery was detected at the time of corrective surgery. The patient underwent a resection of the coarctation with a patch enlargement of the aortic arch and a successful reimplantation of the coronary artery into the aorta.

Cardiac haemodynamics have been studied radiocardiographically in 56 men at the age of 40-60 including 15 healthy persons, 24 patients with chronic coronary disease and 17 patients associating coronary disease with chronic cholecystitis. This association was shown to reduce minute heart volume thus playing the role of a factor compromising cardiac blood circulation in such patients. Neuroreflex influence on the coronary arteries and autoimmune damage of myocardium due to similarity of myocardial and gallbladder antigenic determinants may be probable mechanisms of such an effect.
