Intestinal failure-associated liver disease: what do we know today?

Intestinal failure-associated liver disease develops in 40% to 60% of infants who require long-term total parenteral nutrition (TPN) for intestinal failure and 15% to 40% of adults on home parenteral nutrition. The clinical spectrum includes hepatic steatosis, cholestasis, cholelithiasis, and hepatic fibrosis. Progression to biliary cirrhosis and the development of portal hypertension and liver failure occurs in a minority but is more common in infants and neonates than in adults. The pathogenesis is multifactorial. In infants it is related to prematurity, low birth weight, duration of PN, sho…

Acute liver failure: a critical appraisal of available animal models.

The availability of adequate experimental models of acute liver failure (ALF) is of prime importance to provide a better understanding of this condition and allow the development and testing of new therapeutic approaches for patients with ALF. However, the numerous etiologies and complications of ALF contribute to the complexity of this condition and render the development of an ideal experimental model of ALF more difficult than expected. Instead, a number of different models that may be used for the study of specific aspects of ALF have been developed. The most common approaches used to indu…

Acute-on-chronic liver failure: pathophysiological basis of therapeutic options.

The vast majority of patients that are referred to a specialist hepatological centre suffer from acute deterioration of their chronic liver disease. Yet, this entity of acute-on-chronic liver failure remains poorly defined. With the emergence of newer liver support strategies, it has become necessary to define this entity, its pathophysiology and the short- and long-term prognosis. This review focusses upon how a precipitant such as an episode of gastrointestinal bleeding or sepsis may start a cascade of events that culminate in end-organ dysfunction and liver failure. We briefly review the pa…

Brain edema in liver failure: basic physiologic principles and management.

In patients with severe liver failure, brain edema is a frequent and serious complication that may result in high intracranial pressure and brain damage. This short article focuses on basic physiologic principles that determine water flux across the blood-brain barrier. Using the Starling equation, it is evident that both the osmotic and hydrostatic pressure gradients are imbalanced across the blood-brain barrier in patients with acute liver failure. This combination will tend to favor cerebral capillary water influx to the brain. In contrast, the disequilibration of the Starling forces seems …

Liver failure caused by hepatic angiodysplasia in hereditary hemorrhagic telangiectasia.

Hereditary hemorrhagic telangiectasia is a systemic vascular disease with autosomal dominant inheritance that results in telangiectasia, arteriovenous malformations, and hemangiomas. The liver is one of the organs commonly affected in hereditary hemorrhagic telangiectasia, and hepatic lesions consist of angiodysplasia and fibrosis. A patient with hereditary hemorrhagic telangiectasia and significant impairment of synthetic liver function is reported. Dynamic computed tomography revealed marked enlargement of the common hepatic and intrahepatic arteries, heterogeneous parenchymography, and earl

Reversal of liver failure in sickle cell vaso-occlusive crisis.

A severe but unusual complication of sickle cell vaso-occlusive crisis is acute liver failure related to intrahepatic cholestasis. The outcome is usually fatal in adults. A case of reversible acute liver failure in a patient with s/beta+ thalassemia is reported. The patient was admitted to the intensive care unit because of major organ failure related to vascular occlusion phenomena. After blood-plasma transfusion and supportive therapy for acute liver failure, complete recovery was noted. A liver biopsy performed at the stage of recovery was compatible with intrahepatic cholestasis and sickli…

Acute liver failure: results of a 5-year clinical protocol.

This investigation summarizes and evaluates the results of a clinical protocol that we designed to care for patients with acute liver failure (ALF). Adult patients with ALF were enrolled in the protocol. Grade II portal-systemic encephalopathy prompted admission to the intensive care unit (ICU). Patients who met the clinical criterion were activated for liver transplantation. Intracranial pressure (ICP) was monitored in patients with grade III encephalopathy. An increase in ICP was treated with hyperventilation, diuretics, barbiturates, or a combination thereof. Survival was considered to have…

Intracranial pressure monitoring and liver transplantation for fulminant hepatic failure.

Cerebral edema and intracranial hypertension, commonly present in fulminant hepatic failure, may lead to brainstem herniation and limit the survival of comatose patients awaiting liver transplantation before a donor organ becomes available. Also, they are likely responsible for postoperative neurological morbidity and mortality. Although intracranial pressure monitoring has been proposed to aid clinical decision making in this setting, its use in the prevention of brainstem herniation preoperatively, in the selection of patients for liver transplantation who have the potential for neurological

Antibiotic exposure exacerbates acute-on-chronic liver failure via gut microbiota imbalance and secondary liver lesion.

Introduction. The correlation between antibiotic exposure and adverse outcomes in patients with acute-on-chronic liver failure (ACLF) remains controversial, and the underlying mechanism is unclear.Hypothesis/Gap Statement. This study hypothesizes that antibiotic exposure in ACLF patients alters gut microbiota, which affects the outcome of ACLF.Aim. To explore the effect of antibiotic exposure on gut microbiota that affects the outcome of ACLF.Methodology. A retrospective matched study of ACLF patients and the ACLF rat model was used to assess adverse outcomes associated with antibiotic exposur…

Emergency ABO-Incompatible Living-Donor Liver Transplantation for Acute Liver Failure due to Intentional Iron Poisoning.

BACKGROUND: Iron supplements are widely available over-the-counter, and an intentional overdose is a recognized cause of self-harm, particularly among adolescents. Severe iron poisoning can lead to acute liver failure (ALF) with high mortality, for which optimal management strategies have yet to be established. CASE PRESENTATION: A 14-year-old girl presented after ingestion of iron supplements (54 mg/kg elemental iron). Her peak serum iron concentration was 908 μg/dL and deferoxamine therapy was initiated. Despite the management, the patient progressed to hepatic encephalopathy with worsening…

Evaluation of available scoring systems in liver transplantation for severe acute-on-chronic liver failure (ACLF).

BACKGROUND & AIMS: The Transplantation for ACLF-3 Model (TAM) and Sundaram ACLF-LT-Mortality score (SALT-M) have been proposed to help select liver transplantation (LT) candidates with Acute-on-Chronic Liver Failure (ACLF) to optimize post-LT outcomes. We assessed the performance and clinical utility of these scores and their components on a large retrospective international multicenter study. APPROACH & RESULTS: All patients transplanted in the context of ACLF grade 3 (n=336) from nine international cohorts (2008-2024) were included in the study. The primary endpoint was 1-year post-LT survi…

Dissecting the liver inflammation ecosystem identifies annexin A1 as a pro-resolving target for liver failure.

BACKGROUND AND AIMS: Acute-on-chronic liver failure (ACLF) is driven by systemic inflammation and immune dysregulation. This study aims to characterize the immune landscape in ACLF and identify potential therapeutic targets. APPROACH AND RESULTS: We employed single-cell RNA sequencing, vision spatial sequencing, bulk RNA sequencing, and bioinformatics analysis to profile immune cells in ACLF livers compared to acute liver failure, decompensated cirrhosis, and controls. ACLF livers exhibited a distinct immune signature with increased neutrophils, particularly CCL4 + subsets, which drive unreso…

Early prediction of acute-on-chronic liver failure development in patients with diverse chronic liver diseases.

Acute-on-chronic liver failure (ACLF) is a syndrome characterized by the acute decompensation of chronic liver disease, resulting in organ failure and high short-term mortality. The progression of ACLF is dynamic and reversible in a considerable proportion of patients during hospitalization. Early detection and accurate assessment of ACLF are essential; however, ideal methods for this purpose are still lacking. Therefore, this study aimed to develop a new score for predicting the onset of ACLF in patients with various chronic liver diseases.A total of 6,188 patients with various chronic liver …

Living donor liver transplant in acute on chronic liver failure grade 3: Who not to transplant.

BACKGROUND AND AIM: Liver transplantation(LT)offers definitive treatment for acute on chronic liver failure(ACLF) patients. This study was done to analyze and compare the outcomes of living donor LT(LDLT) in patients with ACLF versus Chronic liver disease(CLD) and within the grades of ACLF. Factors affecting mortality in patients with ACLF and ACLF grade3 (ACLF3) following LDLT were also derived. METHODS: Records of adult LDLT between 1/2/2017 and 30/9/2021 were analyzed. ACLF was classified based on EASL-CLIF definition. Post-transplant outcomes of ACLF were compared with CLD and within ACLF…

Kidney Replacement Therapy in Patients with Acute Liver Failure and End-Stage Cirrhosis Awaiting Liver Transplantation.

Providing dialysis to patients with liver failure is challenging because of their tenuous hemodynamics and refractory ascites. With better machinery and increased availability, continuous kidney replacement therapy has been successfully delivered to acutely ill patients in liver failure over the past few decades. Intermittent hemodialysis continues to remain the modality of choice outside the intensive care unit and on occasion needs to be complemented with paracentesis. Peritoneal dialysis has not been widely used, but recent literature shows promising outcomes barring for publication bias. A…
