Large scale networks for human hand-object interaction: Functionally distinct roles for two premotor regions identified intraoperatively.

The development of awake intraoperative brain-mapping procedures for resection of brain tumors is of growing interest in neuroscience, because it enables direct testing of brain tissue, previously only possible in non-human primates. In a recent study performed in this setting specific effects can be induced by direct electrical stimulation on different sectors of premotor cortex during the execution of a hand manipulation task. Specifically, direct electrical stimulation applied on a dorsal sector of precentral cortex led to an increase in motor unit recruitment in hand muscles during executi

Clinical significance of intraoperative bidirectional corticocortical evoked potential monitoring to evaluate language function.

OBJECTIVE: Awake craniotomy is commonly used to resect lesions located near the language area during brain surgery. However, it is often difficult to perform language tasks due to several limitations such as difficulty in awakening during surgery and intraoperative seizures. This study investigated the clinical significance of bidirectional corticocortical evoked potential (CCEP) monitoring as a new approach to evaluate intraoperative language function. METHODS: This study enrolled 12 patients who underwent awake brain tumor resection with intraoperative CCEP monitoring to assess language fun

Sensitivity of intraoperative electrophysiological monitoring for scoliosis correction in identifying postoperative neurological deficits: a retrospective chart review of the Scoliosis Research Society morbidity and mortality database.

BACKGROUND: Surgical intervention is the ultimate treatment for scoliosis, but iatrogenic spinal cord injury is one of the major concerns. Although intraoperative electrophysiological monitoring can aid in detecting and reducing postoperative neurological complications, its use is still controversial. METHODS: A retrospective chart review of 6,577 scoliotic patients who underwent surgery for curve correction with a reported complication was conducted. Our dataset was sourced from the morbidity and mortality database of the Scoliosis Research Society spanning the period from 2013 to 2023. The

Intraoperative electroencephalogram features related to frailty in older patients: an exploratory prospective observational study.

Frailty is an independent risk factor for the increased incidence of postoperative delirium (POD). To date, the effect of frailty on intraoperative electroencephalogram (EEG) changes remains unexplored. The present study, an exploratory analysis of a prospective cohort study, aimed to investigate the differences in EEG characteristics between frail and robust patients. This prospective observational study was conducted between December 2020 and November 2021. The preoperative frailty status was assessed using the FRAIL scale. The patients' baseline (before anesthesia) and intraoperative EEG da

The Influence of Electromyographic on Electroencephalogram-Based Monitoring: Putting the Forearm on the Forehead.

BACKGROUND: Monitoring the electroencephalogram (EEG) during general anesthesia can help to safely navigate the patient through the procedure by avoiding too deep or light anesthetic levels. In daily clinical practice, the EEG is recorded from the forehead and available neuromonitoring systems translate the EEG information into an index inversely correlating with the anesthetic level. Electrode placement on the forehead can lead to an influence of electromyographic (EMG) activity on the recorded signal in patients without neuromuscular blockade (NMB). A separation of EEG and EMG in the clinica

Neurophysiological effects of recombinant interferon-gamma and -alpha in man.

Treatment of malignant tumors with interferon (IFN) is in some patients accompanied by serious neurological side effects. The present study assessed neurophysiological changes in spontaneous EEG activity, visual-evoked cortical potentials (VEPs), and brainstem auditory-evoked potentials (BAEPs) during IFN-gamma or IFN-alpha therapy in 9 patients. In addition, blood pressure, heart rate and body temperature were monitored. In all sessions under IFN, the latency of the P100 component of the VEP was shortened as compared to baseline conditions. IFN also reduced latencies of BAEP components, and d

Resting state differences between successful and unsuccessful restrained eaters.

Restrained eating is a popular weight loss strategy for young women that tends to have limited effectiveness over extended periods of time. Although previous studies have explored and identified possible personality and behavior differences between successful and unsuccessful restrained eaters (REs), there has been a paucity of research on neurophysiological differences.Towards addressing this gap, we assessed brain resting state (Rs) differences in groups of unsuccessful REs (N = 39) and successful REs (N = 31). In line with hypotheses, unsuccessful REs displayed reduced regional homogeneity

Ipsilateral Motor Innervation Discovered Incidentally on Intraoperative Monitoring: A Case Report.

BACKGROUND AND IMPORTANCE: Lesions in the corticospinal tract above the decussation at the medullary pyramids almost universally produce contralateral deficits. Rare cases of supratentorial lesions causing ipsilateral motor deficits have been reported previously, but only ever found secondary to stroke or congenital pyramidal tract malformations. CLINICAL PRESENTATION: Herein, we report a case of ipsilateral corticospinal tract innervation discovered incidentally with intraoperative monitoring during a microsurgical resection of a vestibular schwannoma. Intraoperative monitoring with electric

Somatosensory evoked potentials in acute neurological decompression illness.

Somatosensory evoked potentials (SSEPs) were recorded on posterior tibial nerve stimulation during compression in 21 episodes of acute neurological decompression illness (DCI) involving 20 patients. A further two cases were monitored immediately after treatment only. The results were compared with previously published normal data from this laboratory and with recordings obtained during the compression of four normal volunteers. Compression had no effect on SSEPs in normal subjects: SSEP latency abnormalities were found in 17 of the episodes of DCI. An SSEP improvement at final recording was no

A modular and flexible pipeline for intraoperative electrode reconstruction and localization in patients with brain lesions.

Intraoperative intracranial electrophysiological recordings provide unique access to human cortical dynamics but remain difficult to translate across patients due to inconsistent localization of transient surface electrodes. Unlike chronic implantations, intraoperative electrodes are placed transiently, rarely visible on imaging, and often inconsistently documented. We present an open-source imaging pipeline, ALIGNER (Advanced Localization and Imaging Guidance for Neurosurgical Electrode Recording), designed to reconstruct intraoperative surface electrode array placements and quantitatively ma

Simultaneous cochlear implantation during retrosigmoid resection of a large vestibular schwannoma under continuous electrically evoked auditory brainstem response monitoring.

Hearing preservation during surgery for large vestibular schwannomas (VS) remains challenging, and reports of simultaneous cochlear implantation (CI) via the retrosigmoid approach are exceedingly rare. We report, through close collaboration between neurosurgery and otolaryngology, what is, to our knowledge, the first case of simultaneous CI during retrosigmoid resection of a large sporadic VS under continuous intraoperative electrically evoked auditory brainstem response (EABR) monitoring. A 70-year-old woman with a 30-mm Koos grade 4 sporadic VS and contralateral profound hearing loss present

Intraoperative Motor Stimulation During Asleep Subthalamic Nucleus Deep Brain Stimulation Predicts Postoperative Motor Side Effects.

BACKGROUND AND OBJECTIVES: Asleep subthalamic nucleus deep brain stimulation (STN-DBS) offers therapeutic benefits comparable with awake DBS for Parkinson disease (PD). However, it typically omits neurophysiological confirmation by microelectrode recordings and intraoperative stimulation, which may lead to postoperative motor side effects at low thresholds and necessitate lead repositioning. Therefore, identifying predictors of these side effects is essential. We hypothesized higher pulse widths stimulation intraoperatively will correlate with postoperative lower clinical pulse widths stimulat
