The authors present a new implant for vertebral body replacement in the thoracic and lumbar spine. Synex is a titanium implant designated for reconstruction of the anterior column in injury, post-traumatic kyphosis or tumour of the thoracolumbar spine and must be supplemented by a stabilizing implant. In a prospective study, 33 patients with recurrent anterior dislocation of the shoulder were treated by a modified Bristow-Latarjet operation using a biodegradable poly-L-lactide expansion plug to fix the transferred coracoid bone block. The mean follow up was 12 months. No redislocations occurred. To assess incorporation of the bone block, serial CT scans were obtained in 18 randomly selected patients and 15 showed solid fusion. SUMMARY OF BACKGROUND DATA: VEPTR surgery is a growth-sparing approach for early-onset scoliosis and chest or rib abnormalities. SSIs are an important complication following VEPTR surgery. We aimed to identify modifiable risk factors for SSIs following VEPTR surgery.

Restoration of longitudinal stability of the forearm continues to pose a difficult challenge for the hand surgeon, and no technique has demonstrated success above others. We report 2 patients with fibrous dysplasia who underwent correction, using the Ilizarov technique, of Shepherd's crook deformities and pathological fractures of the left femurs. A 12-year-old boy underwent an opening wedge osteotomy and a 48-degree gradual correction, whereas a 43-year-old woman underwent a 34-degree acute correction without osteotomy at the fracture site. The progression of kyphosis in myelomeningocele is independent of skeletal growth and requires early operative correction and stabilization to prevent a loss of sitting ability. In severe cases, only vertebrectomy makes it possible to achieve correction, stability and skin-closure without tension. A modification of the technique of Chuinard and Peterson for distraction-compression arthrodesis of the ankle was used in twelve patients, whose ages ranged from ten to seventy-one years. Among our polytrauma patients, 36.3% suffered combination injuries of the pelvic ring and the urogenital tract, and 8 of the patients with this type of lesion underwent orthopedic and urological surgery in a single session on the day of the accident. Thirty-three limb segments with various deformities in 28 children were treated by external fixation. Sole bone lengthenings and trauma cases were excluded from this study. An analysis of indications, techniques, results of stabilization and decompression of 100 consecutive spinal tumour cases was carried out. Localized metastatic disease is best operated anteriorly.
