OBJECTIVES: Osteoradionecrosis (ORN) of the temporal bone is a rare, late complication of radiotherapy to the temporal bone region for head and neck or skull base tumours. ORN can occur as a localized or a diffuse type, according to the extension of the affected temporal bone. It can lead to otitis externa, otitis media, aseptic labyrinthitis and may lead to serious intracranial complications. Hearing loss may be conductive, sensorineural, or mixed. A few case studies report of previously irradiated patients presenting with conductive hearing loss presumably caused by ORN of the ossicular chai

PURPOSE: To evaluate the safety and performance of three novel vibroplasty couplers that allow attachment of the floating mass transducer of a transcutaneous active middle ear implant (AMEI) to the round window (RW) membrane, the long process (LP), or the incus body and the short process (SP) of the incus.
METHODS: Retrospective multicenter cohort study of 25 AMEI users with sensorineural or mixed hearing loss that were among the first implanted with an AMEI vibrating ossicular prosthesis in combination with the third generation of vibroplasty couplers between 2014 and 2016. Main Outcome Meas…

Surgical management of cholesteatoma limited to the attic and/or mesotympanum remains controversial. The aim of this study is to evaluate th … t MRI study that was negative for residual cholesteatoma. One patient (4 %) had displacement of the ossicular prosthesis. In conclusion, cholesteatomas restricted to the attic and/or mesotympanum can be removed in a one-stage technique with no visible residual at 1 year, and with closure of the air-bone gap by 50 %.

UNLABELLED: SUPERELASTIC NITINOL STAPES PROSTHESESINTRODUCTION: Many different prostheses are available for stapes surgery. The newly designed Nitinol piston band-shaped stapes prosthesis could minimize postoperative complications due to its new superelastic characteristic and design.
PATIENTS AND METHODS: A stapedectomy and implantation of superelastic Nitinol piston stapes prosthesis was performed in 22 patients with a mean age of 45.2 years. Examination and audiometry was performed pre- and postoperatively.
RESULTS: The average observed air-bone-gap in all frequencies was 12.8 dB postoper

Since 2003 we are using in our stapedotomies the Nitinol 'Smart' Piston. This prosthesis has a Teflon 'vestibular' end and a wire shaft made by Nitinol, with a heat activated self-crimping loop. Nitinol is an alloy of Nickel + Titanium, belonging to the class of the so-called smart materials, i.e. materials with shape-memory and superelastic properties. Nitinol is lightweight and highly biocompatible thanks to the thin layer of Titanium oxide covering the Nickel surface. The special advantage of this piston is that the loop grips by itself very uniformly and quite tightly around the incudal pr

Studies have indicated that stapedectomy can be an effective procedure in children for correcting conductive hearing losses due to juvenile otosclerosis. However, because childhood otosclerosis is rare and children commonly choose to use hearing aids in lieu of undergoing surgery, little outcome data are available. The purpose of this retrospective study was to provide additional outcome data in both the short and the long term. Stapedectomies were performed on 47 children. Preoperative hearing results were compared with 6-month postoperative hearing results. Hearing results for the children w

Active middle ear implants augment sound waves and directly stimulate the middle ear structures. The most frequently utilised active middle ear implant is the Vibrant Soundbridge TM (VSB).CT plays a vital role in appropriate patient selection and surgical planning of active middle ear implant surgery. The VSB TM offers a number of options for implant placement. The ideal location is influenced by the patient's middle ear and mastoid anatomy as well as the type and severity of the hearing loss. CT provides important information on the surgical access to the middle ear and helps determine the mo

OBJECTIVES/HYPOTHESIS: To investigate the efficacy of direct implantation of a Vibrant Soundbridge (VSB) implant in the oval window (OW) without the use of an OW coupler in patients with severe mixed hearing loss.
STUDY DESIGN: Retrospective chart review METHODS: A total of 62 patients underwent VSB implantation between July 2016 and December 2018 at Severance Hospital in Seoul, South Korea. Among them, eight patients (nine ears) with moderate-to-severe mixed hearing loss were implanted with a VSB directly in the OW. A floating mass transducer (FMT) was attached to the stapes footplate and co

OBJECTIVE: The study aimed to investigate whether cortical auditory evoked potential (CAEP) measures could be used to optimize active middle ear implant (aMEI) and bone conduction implant (BCI) fitting, with the goal of improving hearing outcomes in adults.
DESIGN: CAEPs were measured in response to LING sounds /OO/, /AH/, and /SH/ presented in sound field. If CAEP responses were recorded for all sounds, no map adjustments were performed. If a CAEP response was absent for one or more sounds, map parameters were optimized until a CAEP response could be induced. Functional outcomes were measure

The Vibrant Soundbridge is a semi-implantable, active middle ear implant that is a safe and effective treatment for patients with sensorineural hearing loss. Since Food and Drug Administration approval for this indication, many international investigators have expanded its use for conductive and mixed hearing losses. This article reviews the author's experience and the international uses of this versatile device.

A measurement system was developed that permits objective comparisons of the sound conduction of middle ear implants. The implants are fitted into a mechanical middle ear model which approximates the impedances of the eardrum and the inner ear. A defined signal within the frequency range of 0-5 kHz is provided by a miniaturized loudspeaker at the input to the model and is measured by a probe microphone. Displacement of an artificial stapes footplate at the output of the model is measured by a fiberoptic probe with a sensitivity of 5 nm. The transmission function is calculated as the quotient o

OBJECTIVE: Our experimental study evaluated the efficiency of sound transmission with the Floating Mass Transducer (FMT) in forward (incus short process, SP) and reverse (round window, RW) stimulation modes using different coupling configurations.
METHODS: Using laser Doppler vibrometry (LDV) at the stapes and intracochlear pressure difference (ICPD) the equivalent sound pressure level output was determined according to ASTM standard. Coupling configurations included the Vibroplasty-SP-Coupler, the Vibroplasty-RW-Coupler, and the research RW-Precision-Coupler. Reverse stimulation was studied

PURPOSE: To verify the results of active middle ear implant on audibility and auditory speech perception in individuals with external and/or middle ear malformations.
METHODS: Primary, observational, retrospective study, through analysis of medical records of individuals with bilateral external and/or middle ear malformations, unilateral users of active middle ear implant. The data collected refer to auditory thresholds obtained through free-field audiometry and assessment of auditory speech perception - sentence recognition in silence and noise, in the following situations: without the impla

The provision of implantable hearing aids represents an area with high development and innovation potential. On the one hand, this review article provides an overview of current indication criteria for the treatment with active middle ear implants. On the other hand, outcome parameters as well as functional results after implantation of active middle ear implants are demonstrated and discussed. The focus is mainly placed on audiological results as well as the subjective health status. "Patient Reported Outcome Measures" (PROMs) have become an integral part of the evaluation of hearing implant

OBJECTIVE: In patients with conductive (CHL) or mixed hearing loss (MHL), hearing rehabilitation with an implantable hearing system, active middle ear implant (AMEI) or a semi-implantable bone-conduction device (SIBCD), is an option when conventional hearing aids are insufficient, or patients are unable to wear them.
DESIGN: Retrospective analysis of 20 consecutive patients (24 implants) with a comparison of demographic characteristics and audiometric results (air-bone gap = ABG, effective hearing gain = EHG, functional hearing gain = FHG, Freiburg Monosyllabic Test in quiet, Oldenburg Senten
