[Tritanopia and dominantly inherited optic atrophy (author's transl)].

The clinical picture of hereditary dominant optic atrophy is described and the particular importance of the concomitant tritan dyschromatopsia is stressed. Most cases of tritanopia are acquired, congenital ones are rare. Wright has studied 17 tritanopes - the greatest series ever reported. A full analysis of colour vision is given, but no detailed information about other visual functions and the condition of the fundi. As single members of families affected by cominantly inherited optic atrophy show typical tritanopia, a visual acuity of 6/8 and hardly any pallor of their disks, it is possible…

Handy Hints About Raynaud's Phenomenon in Children: A Critical Review.

Raynaud's phenomenon (RP) is a vasospastic disorder characterized by recurrent self-limited episodes of skin pallor, cyanosis, and hyperemia caused by paroxysmal spasms in the small arteries of the fingers and toes and can occur in any age group. Hands, feet, nose, ears, and nipples can be affected. The diagnosis is made clinically, assessing varying degrees of ischemia in the involved areas of skin, but this transient ischemia may also herald the onset of connective tissue disease. Investigation is recommended when RP starts in childhood to exclude an underlying autoimmune condition and close…

Juvenile myelomonocytic leukemia presenting with facial nerve paresis: a unique presentation.

Juvenile myelomonocytic leukemia (JMML) is a distinct myeloproliferative malignancy of early childhood with a varied clinical presentation that may include failure to thrive, malaise, fever, bleeding, pallor, lymphadenopathy, and hepatosplenomegaly. Skin, pulmonary, and gastrointestinal involvement have also been reported. There are no reports of central nervous system (CNS) involvement at diagnosis of this disease. This is a report of a 21-month old boy who had a right facial paresis at presentation. A brain mass was demonstrated on magnetic resonance imaging and cerebrospinal fluid analysis …

[Rhytidectomy and Raynaud's phenomenon: about two cases].

Raynaud's phenomenon is a currently vascular syndrome (8 to 10% of women and 3 to 5% of men). It was defined as episodic ischaemia of the fingers, toes, nose, ears and nipples, which presents clinically as pallor, cyanosis, and often rubor of the skins, in response to cold, emotional stimuli and vasoconstriction agents. The phenomenon is caused by a vasoconstriction of arterials skin. In severe forms of the phenomenon, we can see ulcerations and necrosis. We report here two cases of rhytidectomy flap necrosis in Raynaud's Phenomenon. After review of literature, we explain the elements of physi…

[Disseminated Mycobacterium chelonae infection during HIV. ].

INTRODUCTION: An atypical mycobacterium, Mycobacterium chelonae is a saprophyte germ in the environment, but rarely pathogenic. A few disseminated infections can be noted in immunodepressed patients. OBSERVATION: A 30 year-old man infected by HIV was hospitalised for deterioration in his general status of health with, on clinical examination, pallor, several superficial lymph nodes and hepatosplenomegaly. A biopsy of an axillary node and the liver revealed numerous granulomas with many atypical M. chelonae-like mycobacteria. Adapted treatment led to complete remission. DISCUSSION: Immunodepr…

[Thoracic outlet syndrome with arterial complication treated by the subclavicular transsternal approach: report of a case].

A 42-year-old female was referred to our vascular service because of her right arm fatigue and cold sensitivity. On elevation of right arm, her radial pulse was absent with pallor of the hand. Angiography demonstrated a significant stenosis of the right subclavian artery, which was considered to be complication of thoracic outlet syndrome. We performed resection of the first rib using the standard subclavicular approach and the subclavian-subclavian artery bypass using a new transsternal extension approach to the subclavian artery. After follow-up 18 months, she was working and asymptomatic. T…

Acrocyanosis in anorexia nervosa.

Acrocyanosis, an uncommon disorder of the peripheral circulation, may occur in patients with anorexia nervosa. It is not known why this should be, nor whether acrocyanosis correlates with any other features of the disorder. The findings in an unselected series of 155 anorectics are reported. Acrocyanosis occurred in 32 and was more prevalent among the more severely ill. It was associated also with pallor of the face and trunk, slower pulse rates and higher fasting plasma glucose levels. Acrocyanosis could represent a more extreme form of a heat conserving mechanism not uncommon in anorectics. …

Pallido-luyso-nigral atrophy and amyotrophic lateral sclerosis.

Clinical and neuropathological studies of a case of pallido-luyso-nigral atrophy and amyotrophic lateral sclerosis (ALS) in a young woman with a strong likelihood of a similar familial past medical history have been presented. Microscopic examination revealed neuronal loss and gliosis of globus pallidus, corpus luysii and substantia nigra. Pallor of the pyramidal tracts and neuronal loss in hypoglossal nuclei and anterior horns with gliosis were present. The rarity of the association of a pallido-luyso-nigral atrophy and an ALS, the occurrence of an ALS at such a young age and the fact that he…

Quantitative morphological analysis of red blood cells.

Recently developed methods for making quantitative measurements of red cell morphology are described. High-speed image processing is used to make six simultaneous red cell measurements relating to size, hemoglobin content, central pallor, and shape. These measurements are combined objectively to subdivide red cells into five subpopulations and to characterize each subpopulation by its principal distributional parameters. Also, an objective assessment of the blood sample is made relative to the diagnosis of anemia. Results from five typically encountered blood conditions are presented and analy…

The role of the central nervous system in chronic congestive heart failure.

Even though congestive heart failure is extremely common, the mechanisms responsible for the clinical manifestations remain a puzzle. The central and autonomic (sympathetic) nervous systems are responsible for a large part of the clinical manifestations. The role of the nervous system in CHF is discussed briefly. It is evident that there are many gaps in knowledge that remain concerning the role of the central, peripheral, and autonomic nervous systems in congestive heart failure. The peripheral vascular constriction, increase in venous tone and pressure, tachycardia, sweating, dermal pallor, …

Cone-rod dystrophy in the Guinea baboon.

Three stages of macular degeneration associated with diffuse cone-rod dystrophy have been described in a Guinea baboon (P papio) colony. Clinically, the affected animals displayed abnormal behavior associated with decreased vision. Ophthalmoscopically, the lesion in the macula was the only change observable in early cases; retinal vessel attenuation and optic disc pallor were seen only in the advanced cases. The hyperfluorescence of the maculae was the result of loss of pigment in the pigmented epithelium. Electrophysiology supported a cone-rod sequence of this retinal dystrophy. Histologic ex…

Spinal cord disease in children with HIV-1 infection: a combined molecular biological and neuropathological study.

An autopsy study was performed on spinal cords from 18 children who died with HIV-1 infection, using standard histopathologic techniques as well as in situ hybridization and immunocytochemistry for HIV-1. Of 16 spinal cords examined by histology, nine had inflammatory cell infiltrates and six had multinucleated cells; both types of lesion are associated with the presence of HIV-1 in central nervous system tissue. HIV-1 type lesions were often present in the spinal cord and brain from the same patient. Pallor of myelin in corticospinal tracts in the cord was present in half of the cases; this c…

When history can mislead a physician: a challenging case report of idiopathic pulmonary hemosiderosis-A case report and review of the literature.

BACKGROUND: Idiopathic pulmonary hemosiderosis is a rare condition characterized by non-immune-mediated alveolar hemorrhage. Its nonspecific clinical and radiological features make diagnosis particularly challenging. Idiopathic pulmonary hemosiderosis typically presents in older children with a classic triad of hemoptysis, chest X-ray abnormalities, and iron deficiency anemia. In contrast, preschool-aged children may show subtle signs such as respiratory distress, persistent cough, and pallor, which can mimic respiratory infections and lead to severe anemia. Prompt recognition is critical, as …
