Transcranial Doppler: state of the art.

INTRODUCTION: Transcranial color Doppler sonography permits the accurate assessment of intracranial arteries. The latest Doppler units, using the color and power techniques, can show even very small flow volumes (1 x 1 mm). Low frequency (2-2.5 MHz) and very focused transducers are used in transcranial color Doppler. The skull is a very strong barrier for ultrasounds, which requires the use of some acoustic windows like some thin portions of the skull bone or some natural skull foramina. The use of echocontrast agents in color Doppler seems to increase the applications of transcranial studies.

Spontaneous and experimentally induced lesions in NOD-scid gamma and other NOD-derived mouse strains.

Immunodeficient mice, particularly the NOD.Cg-PrkdcscidIl2rgtm1Wjl/SzJ (NSG) strain and other non-obese diabetic (NOD)-derived lines are widely used in biomedical research due to their profound immunosuppression, which enables stable engraftment of human cells and tissues with minimal rejection. Despite their broad utility, these models exhibit unique immunologic and anatomic features and are predisposed to infectious and noninfectious diseases that may confound experimental outcomes and limit translational relevance. This review summarizes current knowledge on spontaneous, infectious, and exp

Solitary plasmacytoma of the skull compressing the superior sagittal sinus: Case report and literature review.

INTRODUCTION: Solitary plasmacytomas are rare, localized plasma cell neoplasms without systemic involvement. Intracranial manifestations, particularly those compressing the superior sagittal sinus, are exceptionally uncommon and can mimic more common dural-based lesions such as meningiomas. RESEARCH QUESTION: This study aims to highlight the diagnostic and therapeutic considerations in cases of intracranial solitary plasmacytoma involving superior sagittal sinus compression through a clinical case and a targeted literature review. MATERIAL AND METHODS: We report a case of a 52-year-old male

Diffusion-Weighted Imaging Does Not Differentiate Between Bacterial and Fungal Skull Base Osteomyelitis.

OBJECTIVE: Apparent diffusion coefficient (ADC) value helps in differentiating infections from neoplasms on magnetic resonance imaging (MRI). We investigate the diffusion-weighted images in skull base osteomyelitis (SBO) to evaluate if ADC values can differentiate fungal and bacterial SBO and to analyse the microbiology of all SBO patients. DESIGN: Retrospective observational study. SETTING: Quaternary care referral centre. PARTICIPANTS: A retrospective review of 142 patients diagnosed and treated for SBO patients from January 2010 to May 2023 was done. MAIN OUTCOME MEASURE: Chi-square or

Extralaryngeal causes of unilateral vocal cord paralysis: Aetiology and prognosis.

INTRODUCTION AND METHODOLOGY: Unilateral vocal cord paralysis without laryngeal lesions is a relatively frequent entity. It can be the manifestation of numerous diseases of the thorax, neck, skull, or systemic disease. The objective is to study the extralaryngeal aetiology of unilateral vocal cord paralysis, its prognosis, and the relationship of both with different clinical variables. Retrospective study of 116 patients with complete unilateral vocal cord paralysis without laryngeal lesions. The patients underwent cervical-thoracic CT ± evaluation by Neurology with brain MRI to establish the

The endonasal approach for treatment of cerebral aneurysms: A critical review of the literature.

The last two decades of neurosurgery have seen flourishing use of the endonasal approach for the treatment of skull base tumors. Safe and effective resections of neoplasms requiring intracranial arterial dissection have been performed using this technique. Recently, there have been a growing number of case reports describing the use of the endonasal approach to surgically clip cerebral aneurysms. We review the use of these approaches in intracranial aneurysm clipping and analyze its advantages, limitations, and consider future directions. Three major electronic databases were queried using rel

[Ossified choroid plexus papilloma--case report].

While the calcification has been documented radiologically in 4 - 25 percent of the cases with choroid plexus papilloma, the ossification of choroid plexus papilloma has been reported only in 3 cases on literature. In this paper, we present a case of large ossified choroid plexus papilloma in the right lateral ventricle. A 35-year-old man was admitted with left hemiparesis and headache. Skull X-rays showed an abnormal calcified mass (25 mm x 23 mm x 14 mm) in the right temporal region. CT scan revealed an abnormal high density area protruding into the right lateral ventricle, and a low density

Dyke-Davidoff-Masson Syndrome Presenting With Recurrent Seizures and Left-Sided Motor Weakness: A Case Report.

This study aimed to highlight that, regardless of a condition's rarity, every patient seeks timely recognition and effective management, underscoring clinicians' responsibility to remain vigilant and informed to provide optimal care. Dyke-Davidoff-Masson syndrome, first described in 1933, represents a classic radiological and clinical manifestation of this condition. This syndrome is characterized by unilateral cerebral atrophy accompanied by compensatory skull thickening and hyperpneumatization of the paranasal sinuses. The underlying cerebral insult may occur during fetal development or earl

Methylation-based reclassification and risk stratification of skull-base chordomas.

BACKGROUND: Skull-base chordomas are rare malignant bone cancers originating from the remnant of the notochord. Survival is variable, and clinical or molecular factors cannot reliably predict their outcomes. This study therefore identified epigenetic subtypes that defined new chordoma epigenetic profiles and their corresponding characteristics. METHODS: Methylation profiles of 46 chordoma-resected neoplasms between 2008 and 2014, along with clinical information, were collected. K-means consensus clustering and principal component analysis were used to identify and validate the clusters. Singl

Diagnosis and Treatment of Parasellar Lesions.

The parasellar region, located around the sella turcica, is an anatomically complex area representing a crossroads for important adjacent structures. Several lesions, including tumoral, inflammatory vascular, and infectious diseases may affect this area. Although invasive pituitary tumors are the most common neoplasms encountered within the parasellar region, other tumoral (and cystic) lesions can also be detected. Craniopharyngiomas, meningiomas, as well as Rathke's cleft cysts, chordomas, and ectopic pituitary tumors can primarily originate from the parasellar region. Except for hormone-prod

Characteristic CT and MRI findings of intracranial chondroma.

BACKGROUND: Intracranial chondromas are rare benign tumors. To date, few data are available on their neuroradiological features. PURPOSE: To describe a series of patients with intracranial chondroma and to analyze and discuss the computed tomography (CT) and magnetic resonance imaging (MRI) features that may distinguish chondromas from other intracranial neoplasms. MATERIAL AND METHODS: We retrospectively analyzed clinical and medical imaging data of six patients who had pathologically confirmed intracranial chondromas in our two institutions between July 2006 and September 2011. Both CT and

Nasopharyngeal carcinoma.

Nasopharyngeal carcinoma (NPC) is a tumor arising from the epithelial cells that cover the surface and line the nasopharynx. The annual incidence of NPC in the UK is 0.3 per million at age 0-14 years, and 1 to 2 per million at age 15-19 years. Incidence is higher in the Chinese and Tunisian populations. Although rare, NPC accounts for about one third of childhood nasopharyngeal neoplasms. Three subtypes of NPC are recognized in the World Health Organization (WHO) classification: 1) squamous cell carcinoma, typically found in the older adult population; 2) non-keratinizing carcinoma; 3) undiffe

Psychic stress in cranial-cerebral tumors.

From a historical point of view, medicine (in modem society) has set its mission as to cure diseases--medical-type issues--and to ignore patient's condition--illness endurance. By accepting this point of view with reference to their problem, patients have entered some sort of conspiracy of silence and started ignoring the emotional reaction to their medical problems (or to cancel these reactions), regarding them as irrelevant to the problem itself. This approach is also reinforced by the medical pattern that absolutely contests the idea that mind influences the body in a very important way. An

[Resection of malignant tumors involving the anterior cranial fossa].

BACKGROUND: The craniofacial approach has greatly facilitated resections of tumours involving the base of the anterior cranial fossa when compared to either the transcranial or transfacial approach alone. MATERIAL AND METHODS: This approach was used in 11 patients with malignant tumours localized to the ethmoid sinus, orbit and bone or soft tissue of the base of the anterior part of the skull. By combining a low frontal or frontolateral craniotomy with resection of the facial skull, en bloc resections were accomplished. A frontogaleal periostal flap or a muscle flap from the temporal muscle w

[Endoscopic Endonasal Surgery for Skull Base Chordomas and Chondrosarcomas : Basic to Advanced Cases].

Skull base chordomas and chondrosarcomas are rare bone tumors predominantly found in the petroclival areas and involve the cranial nerves, internal carotid arteries, and brain stem. Their clinical aggressiveness and high risk of recurrence necessitate multimodal treatments, including extensive surgical resection and high-dose radiotherapy. Surgery is commonly the first step in their treatment, and ABCD rules should be respected; anatomical preservation of the innocent nasal anatomy for potential repeated surgeries in case of recurrence, block-by-block resection, coverage of the intradural resi
