Voice restoration after circumferential pharyngolaryngectomy with free jejunum repair.

Speech restoration after circumferential pharyngolaryngectomy with free jejunal repair for advanced tumors of the hypopharyngo-esophageal tract remains a difficult problem to solve. We report here the results of secondary voice restoration in six patients who received a Provox 2 type prosthesis and intensive speech therapy after circumferential pharyngolaryngectomy with free jejunum repair. No patient had operative or post-operative complications due to insertion of the prosthesis. No patient had to have the prosthesis removed during the follow-up (8 to 14 months). Analysis of some acoustic pa

Tracheoesophageal voice following tubed free radial forearm flap reconstruction of the neopharynx.

Tracheoesophageal voice restoration after laryngectomy is possible with a variety of neopharyngeal reconstructions. We have used the tubed radial forearm free flap for neopharyngeal reconstruction since 1991. Six patients have undergone voice restoration with the Blom-Singer prosthesis and were available for quantitative and qualitative speech analysis. These patients were compared to five laryngectomy patients with standard pharyngeal closures and similar voice restorations. The free flap patients produced similar loudness levels compared to the standards with soft speech (52.06 dB and 47.19

Timing of hard palatal closure: a critical long-term analysis.

The controversy about timing of cleft palate surgical procedures is focused on early palatoplasty for improved speech versus delayed hard palate repair for undisturbed facial growth. Timing and technique of palate repair are the most important influences on speech and facial growth, yet there is no consensus on the age or technique for surgery. The Oxford Cleft Palate Study was initiated to evaluate critically the long-term follow-up of 44 patients with early versus late closure of the hard palate. A multidisciplinary approach was used to determine the incidence of speech deficiencies, palatal

The Relative Contribution of Cochlear Synaptopathy and Reduced Inhibition to Age-Related Hearing Impairment for People With Normal Audiograms.

Older people often show auditory temporal processing deficits and speech-in-noise intelligibility difficulties even when their audiogram is clinically normal. The causes of such problems remain unclear. Some studies have suggested that for people with normal audiograms, age-related hearing impairments may be due to a cognitive decline, while others have suggested that they may be caused by cochlear synaptopathy. Here, we explore an alternative hypothesis, namely that age-related hearing deficits are associated with decreased inhibition. For human adults (N = 30) selected to cover a reasonably

[First implantation of a totally implantable electronic hearing aid in patients with inner ear hearing loss].

For the majority of patients with sensorineural hearing loss (SNHL) many available hearing aids often do not achieve satisfactory results. For these patients partially implantable hearing devices have been developed, allowing distortion-free hearing and speech intelligibility that may be superior to conventional hearing aids. The external parts of partial implants, however, may result in a patient's stigmatization. Furthermore, they do not use the acoustic properties of the external auditory canal. Recently, we published the successful development of the first totally implantable hearing devic…

Adults with Autism Spectrum Disorder Demonstrate Increased Disfluency in Spontaneous Speech but Not in Reading.

PURPOSE: This study aimed to analyse the frequency and types of disfluencies in spontaneous speech and reading among adults with autism spec … tential or actual clinical implications for this work? Increased speech disfluencies can affect the speech intelligibility and/or social interaction of adults with ASD. Moreover, the integration of a more detailed analysis of disfluencies in individuals with ASD as part of a broader overall assessment might optimize the diagnostical and clinical decision-making process.

Dysarthria enhancement mechanism under external clear speech instruction in Parkinson's disease, progressive supranuclear palsy and multiple system atrophy.

Clear speech refers to intentionally modifying conversational speech to maximise intelligibility. This study aimed to compare the speech behaviour of patients with progressive supranuclear palsy (PSP), multiple system atrophy (MSA) and Parkinson's disease (PD) under conversational and clear speech conditions to gain greater pathophysiological insight. A total of 68 participants including 17 PD, 17 MSA, 17 PSP and 17 healthy controls (HC) performed two readings of the same standardized passage. During the first reading, participants were instructed to read the text in an ordinary way, while dur

Recovery of crossed acoustic reflexes in brain stem auditory disorder.

Crossed acoustic reflexes were serially monitored in a patient receiving radiotherapy for an intra-axial brain stem tumor. Analysis revealed a pattern of systematic recovery as the brain stem lesion responded to therapy. Crossed reflexes were observed at more and more test frequencies, until responses were present on both ears for all test signals. Pure-tone sensitivity and speech intelligibility remained within the normal range throughout the observation period.

Revisiting Consonant Acquisition in Typically Developing Chinese-Speaking Children With Insights Into a Multiword Data Set of Hearing and Deaf/Hard of Hearing Children.

PURPOSE: This study aimed to advance the understanding of consonant acquisition with quantitative and qualitative evidence from various groups of Chinese-speaking children. Normative patterns of phonological development of consonants were affirmed by utilizing phoneme transcription and perceptual judgment of a single-word normative data set, followed by analyses of comparable characteristics of a multiword data set of hearing and deaf/hard of hearing children. METHOD: The single-word normative data set comprised 798 typically developing Chinese-speaking children, whereas the multiword data se

Robust Sequence-to-sequence Voice Conversion for Electrolaryngeal Speech Enhancement in Noisy and Reverberant Conditions.

Electrolaryngeal (EL) speech, an artificial speech produced by an electrolarynx for laryngectomees, lacks essential phonetic features, and differs in temporal structure from normal speech, resulting in poor naturalness and intelligibility. To address this deficiency, sequence-to-sequence (seq2seq) voice conversion (VC) models have been applied in converting EL speech to normal speech (EL2SP), showing some promising performances. However, previous studies mostly focus on converting clean EL speech, thereby restricting the further applicability in real-world scenarios, especially when the EL spe

Hemi-hypoglossal nerve transfer for obstetric brachial plexus palsy: report of 3 cases.

Use of the entire hypoglossal nerve for nerve transfer in obstetric palsy is not recommended because of major donor nerve morbidity in terms of feeding and speech problems. We used a hemi-hypoglossal nerve transfer for biceps reinnervation in obstetric palsy in 3 infants with multiple root avulsions. Two of the 3 infants recovered normal or near-normal elbow flexion. There was no donor nerve morbidity in terms of feeding. Speech was assessed at age 20 to 27 months and was appropriate for age, which indicates that early speech development (speech intelligibility and articulation) were not affec…
