The Impact of Physical Therapy Following Cervical Spine Surgery for Degenerative Spine Disorders: A Systematic Review.

STUDY DESIGN: Systematic review. OBJECTIVE: To characterize the effects of postoperative physical therapy (PT) after surgery for cervical spondylosis on patient-reported outcomes and impairments. Secondarily, to identify associated complications, adverse effects, and health care costs with postoperative PT, and to describe the content, timing, and duration of the PT. SUMMARY OF BACKGROUND DATA: Cervical spine surgery is common; however, it is unclear if the addition of postoperative PT leads to improved patient outcomes and decreased health care costs. MATERIALS AND METHODS: PubMed, Embase,…

Helping spine surgeons detect pre-surgical psychological distress in complex spine patients: an observational pilot study.

STUDY DESIGN: Retrospective analysis. OBJECTIVE: Analysis of a standardized, pre-surgical psychological evaluation program for complex spine surgery. Adult spinal deformity (ASD) patients have a high rate of comorbid mental health conditions. Although there is a body of literature demonstrating the impact of psychological factors, including anxiety and depression, on spine surgery outcome, it is estimated that spine surgeons utilize a psychological assessment only about one third of the time prior to a patient's spine surgery. At this time, there is not a widely reported pre-surgical psycholo…

Prospective spine at risk program for prevalence of intracanal spine lesions in pediatric hereditary multiple osteochondromas.

STUDY DESIGN: Retrospective cohort study OBJECTIVES: To determine prevalence of hereditary multiple osteochondromas (HMO) and utility of MRI surveillance in a prospective Spine at Risk (SAR) program. Unidentified intraspinal exostoses in HMO can lead to neurologic injury in children during sedated procedures but no MRI guidelines exist. We sought to determine the prevalence and age of intraspinal exostoses from MRIs, and indications for MRI surveillance. METHODS: Retrospective review was performed of pediatric HMO patients who underwent total spine MRIs at a single institution after a prospec…

Primary Neuroendocrine Tumor of the Lumbar Spine: Rare Tumor Mimicking Nerve Sheath Tumor of the Spine.

Neuroendocrine tumors (Carcinoid tumors) generally arise from enterochromaffin cells of gut and bronchi. Primary carcinoid tumors of spine are extremely rare and have been described in sacrum and coccyx. Primary carcinoid tumors involving the spinal cord are still rarer, and review of literature revealed only two cases reported. Our patient a 39-year-old male had diagnosed as a case of nerve sheath tumor (intradural extramedullary) at LV4 region on neuroimaging. However, postoperatively, the tumor turned out to be a rare primary spinal carcinoid tumor on histopathological examination and immun…

Depression as an independent predictor of postoperative delirium in spine deformity patients undergoing elective spine surgery.

OBJECTIVE Depression is the most prevalent affective disorder in the US, and patients with spinal deformity are at increased risk. Postoperative delirium has been associated with inferior surgical outcomes, including morbidity and mortality. The relationship between depression and postoperative delirium in patients undergoing spine surgery is relatively unknown. The aim of this study was to determine if depression is an independent risk factor for the development of postoperative delirium in patients undergoing decompression and fusion for deformity. METHODS The medical records of 923 adult pa…

Dendritic spine instability leads to progressive neocortical spine loss in a mouse model of Huntington's disease.

In Huntington's disease (HD), cognitive symptoms and cellular dysfunction precede the onset of classical motor symptoms and neuronal death in the striatum and cortex by almost a decade. This suggests that the early cognitive deficits may be due to a cellular dysfunction rather than being a consequence of neuronal loss. Abnormalities in dendritic spines are described in HD patients and in HD animal models. Available evidence indicates that altered spine and synaptic plasticity could underlie the motor as well as cognitive symptoms in HD. However, the exact kinetics of spine alterations and plas…

Optimal thoracic and lumbar spine imaging for trauma: are thoracic and lumbar spine reformats always indicated?

BACKGROUND: Computed tomography (CT) of the thoracic and lumbar (T/L) spine with reformats has become the imaging modality of choice for the identification of T/L spine fractures. The objective of this study was to directly compare chest/abdomen/pelvis CT (CAP CT) with CT with T/L reformats (T/L CT) for the identification of T/L spine fractures. METHODS: Patients who had both a CAP CT scan (5-mm imaging spacing) and T/L CT reconstruction (2.5-mm image spacing with sagittal and coronal reformats) were selected. A "fracture" group (N = 35) and a "no fracture" group (N = 57) were identified. The…

Torsional flexibility of the thoracic spine is superior to that of the lumbar spine in cats: Implications for the falling cat problem.

Cats are renowned for their air-righting behavior, effectively reorienting themselves in midair to land on their feet. However, how cats move their bodies during air-righting remains unclear. Complex motor factors, such as trunk flexion and axial torsion, may be involved; however, to obtain a definitive answer, quantitative data on the flexibility of spinal axial torsion and how this flexibility differs between spine regions are needed. In this study, we measured the mechanical properties of the spine under axial torsion and examined their relationship to air-righting behavior. Destructive fai…

Reduction in Traumatic Spine Injuries in the Thoracic and Lumbar Spine With Percutaneous Versus Open Dorsal Stabilization.

STUDY DESIGN: Retrospective cohort study. OBJECTIVES: Doubts still remain whether a sufficient reduction of vertebral body fractures can be achieved if a percutaneous surgical approach is adopted. The aim of this retrospective cohort study was to analyze the reduction and perioperative complications using a percutaneous versus open surgical approach for dorsal stabilization. METHODS: Patients suffering from a traumatic injury of the thoracic or lumbar spine who were treated with dorsal stabilization at our level 1 trauma center were included. From January 2007 through June 2009, only the ope…

Dysphagia following lumbar spine surgery in the setting of undiagnosed DISH of the cervical spine: a case report.

Dysphagia is an often multifactorial pathology affecting many elderly patients. In addition to global neuromuscular change with normal aging, one component of its etiology may be direct compression of the pharynx or esophagus from overgrown bone from the anterior cervical spine. Diffuse idiopathic skeletal hyperostosis (DISH) is one condition that may contribute to this phenomenon. Of relatively high incidence (2.5% to 33.3%) in elderly populations, DISH has been described in the cervical spine though more frequently affects other spinal regions. The clinical case of an elderly man who develop…

Total vertebrectomy and spine shortening for the treatment of T12-L1 spine dislocation: Management with suboptimal resources.

Total vertebrectomy with spine shortening has been reported for the treatment of difficult cases of traumatic spine dislocation, both in acute and chronic phase. We report an exceptional case of a five-week-old T12-L1 spine dislocation in a 25-year-old female with complete paraplegia as a result of trauma in Ciudad de León (Nicaragua). In view of the time since the dislocation, we performed a complete L1 vertebrectomy in order to reduce the dorsolumbar hinge. For osteosynthesis material we had only eight screws and two Steffee plates. We therefore introduced pedicle screws at levels T11, T12, …

The Oblique Anterolateral Approach to the Lumbar Spine Provides Access to the Lumbar Spine With Few Early Complications.

BACKGROUND: During the last 20 years several less-invasive anterior approaches to the lumbar spine have become standard, including the extreme lateral transpsoas approach. Although it is associated with a lower risk of vascular injury compared with anterior midline approaches, neuromonitoring is considered mandatory to avoid neurologic complications. Interestingly, despite neuromonitoring, the reported risk of neurologic deficits with the extreme lateral transpsoas approach is greater than observed with other anterior approaches. An alternative lateral, oblique, psoas-sparing approach, recentl…

Computer-assisted minimally invasive spine surgery for resection of ossification of the ligamentum flavum in the thoracic spine.

BACKGROUND: Ossification of the ligamentum flavum (OLF) has been widely recognized as one of the main causes of thoracic spinal canal stenosis and thoracic myelopathy. Decompression is the only effective strategy for treating thoracic myelopathy caused by OLF. The purpose of this study was to describe the clinical outcomes of computer-assisted minimally invasive spine surgery (CAMISS) for posterior decompression in patients with thoracic myelopathy caused by OLF. METHODS: In all cases, the surgical procedure was performed with the assistance of an intraoperative three-dimensional navigation s…

Pedicle screw insertion in the thoracolumbar spine: comparison of 4 guidance techniques in the intact cadaveric spine.

OBJECT: Pedicle screw fixation is a mainstay of thoracolumbar stabilization. Screw insertion using anatomical landmarks and fluoroscopy is common but can be technically challenging and generally involves substantial exposure to ionizing radiation. Computerized navigation has been reported to improve accuracy but is expensive and complex. The authors undertook this study to evaluate these 3 methods in comparison with a fourth technique using standard cervical distractor screws to mark the entry point and trajectory. METHODS: Four cadaveric human spines were used for this study. After an initia…

Exercises for the torso performed in a standing posture: spine and hip motion and motor patterns and spine load.

The purpose of this study was to document the muscle activity, spine motion, spine load, and stiffness during several movement-based or "functional" exercises and to assess the effect of technique change. Eight subjects, all healthy men from a university population, were instrumented to obtain surface electromyography of selected trunk and hip muscles, together with video analysis and electromagnetic lumbar spine position sensor to track spine posture. Exercises included a walkout in the sagittal plane that compared an upright form against a wall with those performed on the floor, overhead cab…

Restoration of Thoracolumbar Spine Stability and Alignment in Elderly Patients Using Minimally Invasive Spine Surgery (MISS). A Safe and Feasible Option in Degenerative and Traumatic Spine Diseases.

Minimally invasive spine surgery (MISS), including percutaneous pedicle-screw fixation (PPSF), mini-open transforaminal lumbar interbody fusion (m-open TLIF), vertebroplasty, and stentoplasty, allows the preservation of neurological function and the restoration of spine stability, while reducing associated risks and complications. This study aimed to analyze the safety and efficacy of MISS in elderly patients suffering from degenerative or traumatic thoracolumbar diseases. Forty-five patients (28 females), with a mean age of 73 years (range 65-89), suffering from osteoporotic vertebral fractur…
