Cardiovascular Disease Update: Heart Failure Update.

Heart failure (HF) is a clinical condition characterized by the heart's inability to pump blood effectively enough to meet the body's metabolic demands; typically this happens because of impaired ventricular filling or ejection. HF affects millions of people in the United States, with increasing prevalence, hospitalizations, and deaths. Classification of HF by left ventricular ejection fraction divides it into HF with preserved ejection fraction (HFpEF), mildly reduced ejection fraction, and reduced ejection fraction (HFrEF). Early diagnosis and appropriate staging, using tools such as natriur

Heart Failure in Patients with Sarcopenia: Systematic Review and Meta-Analysis.

BACKGROUND: The prevalence of sarcopenia and heart failure (HF) is estimated to be between 8%-34% and 3%-11.8%, respectively. The prevalence of HF in patients with sarcopenia and the prognosis of this association remain unclear. METHODS: A systematic review was conducted across MEDLINE/PubMed, Embase, CENTRAL, SciELO, and CINAHL databases, with manual searches in Google Scholar and grey literature. Meta-analysis was performed on the gathered results to assess the prevalence of HF in patients with sarcopenia, estimate phenotypes related to left ventricular ejection fraction (LVEF), and evaluat

Heart failure and obesity: Translational approaches and therapeutic perspectives. A scientific statement of the Heart Failure Association of the ESC.

Obesity and heart failure (HF) represent two growing pandemics. In the general population, obesity affects one in eight adults and is linked with an increased risk for HF. Obesity is even more common in patients with HF, where it complicates the diagnosis of HF and is linked with worse symptoms and impaired exercise capacity. Over the past few years, new evidence on the mechanisms linking obesity with HF has been reported, particularly in relation to HF with preserved ejection fraction. Novel therapies inducing weight loss appear to have favourable effects on health status and cardiovascular r

Coronary microvascular disease in heart failure with preserved ejection fraction.

The prevalence of heart failure with preserved ejection fraction (HFpEF) is increasing because of an aging population and an unhealthy, sedentary lifestyle, predisposing to diabetes, obesity, dyslipidemia, hypertension, and chronic kidney disease. A substantial proportion of patients with HFpEF exhibits coronary microvascular disease (CMD), and the combination of CMD with left ventricular diastolic dysfunction is associated with worse outcomes. Distinct patient clusters within HFpEF populations are based on clinical and biomarker profiles, each with unique prognoses and comorbidity patterns. M

[Update: Heart Failure with Preserved Ejection Fraction].

Heart failure with a preserved ejection fraction (HFpEF) is a multifaceted disease entity. Confirming the diagnosis as well as tailoring the most appropriate therapy remains an ongoing challenge. The 2021 heart failure guidelines of the European Society of Cardiology (ESC) suggested a simplified diagnostic approach, and the guideline update provided in 2023 gave recommendations regarding pharmacotherapy based on recent evidence. Further, the Heart Failure Association (HFA) and the European Heart Rhythm Association of the ESC jointly proposed a scheme how to classify HFpEF patients into phenogr

Atrial Fibrillation Ablation in Heart Failure with Reduced Ejection Fraction.

Multiple randomized clinical trials have demonstrated catheter ablation in heart failure with reduced ejection fraction reduces mortality and hospitalization as well as improves ventricular function, quality of life, and functional status. Catheter ablation has been shown to be superior to alternative rate and rhythm control strategies in these outcomes. Guidelines strongly support the use of catheter ablation to maintain sinus rhythm in patients with atrial fibrillation and heart failure.

Left ventricle segment-specific motion assessment for cardiac-gated radiosurgery.

Purpose.Cardiac radiosurgery is a non-invasive treatment modality for ventricular tachycardia, where a linear accelerator is used to irradiate the arrhythmogenic region within the heart. In this work, cardiac magnetic resonance (CMR) cine images were used to quantify left ventricle (LV) segment-specific motion during the cardiac cycle and to assess potential advantages of cardiac-gated radiosurgery.Methods.CMR breath-hold cine images and LV contour points were analyzed for 50 controls and 50 heart failure patients with reduced ejection fraction (HFrEF, EF < 40%). Contour points were divided in

Mineralocorticoid Antagonism in Heart Failure: Established and Emerging Therapeutic Role.

The pathophysiology of heart failure (HF) is related to the overactivation of the mineralocorticoid receptor, leading to fluid retention and adverse myocardial remodeling. Although mineralocorticoid receptor antagonists (MRAs) are recommended for the treatment of heart failure with reduced ejection fraction (HFrEF), they remain underused due to adverse effects such as hyperkalemia; and their efficacy is controversial in heart failure with mildly reduced ejection fraction (HFmrEF) and heart failure with preserved ejection fraction (HFpEF). Recent trials in people with diabetes and kidney diseas

Epidemiology, Pathophysiology, Diagnosis, and Therapy of Heart Failure With Preserved Ejection Fraction in Japan.

Heart failure (HF) with preserved ejection fraction (HFpEF) is a global health care problem, with diagnostic difficulty, limited treatment options and high morbidity and mortality rates. The prevalence of HFpEF is increasing because of the aging population and the increasing burden of cardiac and metabolic comorbidities, such as systemic hypertension, diabetes, chronic kidney disease, and obesity. The knowledge base is derived primarily from the United States and Europe, and data from Asian countries, including Japan, remain limited. Given that phenotypic differences may exist between Japanese

Characteristics and outcomes of patients with a history of cancer recruited to heart failure trials.

AIMS: Heart failure (HF) therapy trials usually exclude cancer patients. We examined the association between cancer history and outcomes in trial participants with HF and reduced (HFrEF) or preserved ejection fraction (HFpEF). METHODS AND RESULTS: We combined PARADIGM-HF and ATMOSPHERE, which enrolled HFrEF patients (n = 15 415) and we pooled HFpEF patients (ejection fraction ≥45%) enrolled in PARAGON-HF and CHARM-Preserved (n = 7363). The associations between cancer history, cardiovascular (CV) death, HF hospitalization, non-CV and all-cause death in these trials were examined. Incident canc

Functional outcome after cardiac rehabilitation and its association with survival in heart failure across the spectrum of ejection fraction.

BACKGROUND: There is limited evidence regarding the effects of cardiac rehabilitation (CR) in patients with heart failure and preserved ejection fraction (HFpEF). METHODS: We studied 1784 patients admitted to inpatient CR. The patients were grouped into HFpEF (EF≥0.50), HF with mildly reduced EF (HFmrEF; EF 41-49), and HF with reduced EF (HFrEF; EF≤0.40). A standardized 6-min walking test was performed at admission and discharge. Measures of functional outcome were: (1) absolute increase in 6-min walking distance (6MWD) from admission to discharge >50 m and (2) increase in 6MWD to ≥300 among

In Vitro Pressure Measurements Across an Interatrial Shunt for HFpEF Treatment.

PURPOSE: Preserved ejection fraction heart failure (HFpEF) can be treated by installing a shunt in the interatrial septum, which relieves excess pressure in the left atrium by allowing blood to flow from left to right. This technique has proven effective in clinical trials, but the details of the flow through the shunted heart are not well understood. The current study aims to collect quantitative data on the relationship between pressure and flow rate in such shunts. METHODS: An in vitro, shunted double atrium flow phantom was fabricated and used to investigate the relationship between press
