Stepwise Guided Photorefractive Keratectomy in Treatment of Irregular Astigmatism After Penetrating Keratoplasty and Deep Anterior Lamellar Keratoplasty.

PURPOSE: To report the outcome of stepwise ablation using topography-guided photorefractive keratectomy to treat irregular astigmatism after either penetrating keratoplasty (PKP) or deep anterior lamellar keratoplasty (DALK). METHODS: This is a retrospective, interventional analysis including patients with irregular astigmatism after either PKP or DALK, who underwent topography-guided photorefractive keratectomy. The entire cohort was analyzed, as well as the PKP and DALK groups separately. Analysis of factors associated with a better outcome was also performed. RESULTS: Thirty-four eyes of

Preliminary validation of an optimized algorithm for intraocular lens power calculation in keratoconus.

PURPOSE: This study aimed to evaluate the theoretical influence on intraocular lens power (PIOL) calculation of the use of keratometric approach for corneal power (Pc) calculation in keratoconus and to develop and validate an algorithm preliminarily to minimize this influence. METHODS: Pcwas calculated theoretically with the classical keratometric approach, the Gaussian equation, and the keratometric approach using a variable keratometric index (nkadj) dependent on r1c(Pkadj). Differences in PIOLcalculations (ΔPIOL) using keratometric and Gaussian Pcvalues were evaluated. Preliminary clinical

Repair of Irregularly Irregular Astigmatism by Transepithelial Phototherapeutic Keratectomy.

PURPOSE: To evaluate the outcome of transepithelial phototherapeutic keratectomy (PTK) in the management of irregularly irregular astigmatism. METHODS: This was a case series of two patients who underwent transepithelial PTK for irregularly irregular astigmatism. In the first case, the patient complained of diplopia due to corneal scarring caused by a metallic foreign body injury. The topography demonstrated irregularly irregular astigmatism with significant asymmetry in the inferotemporal to superonasal axis. In the second case, the patient complained of blurred vision and ghosting caused by

Repeat Keratoplasty for Failed Deep Anterior Lamellar Keratoplasty in Keratoconus: Incidence, Indications, and Outcomes.

PURPOSE: To analyze the incidence, indications, and clinical outcomes of penetrating keratoplasty (PK) and repeat deep anterior lamellar keratoplasty (DALK) after failed DALK for keratoconus. METHODS: This retrospective interventional case series reviewed the medical records of patients with keratoconus who underwent DALK and were followed up for at least 1 year. Patients who underwent PK or repeat DALK for failed DALK were identified. The incidence, indications, and clinical outcomes after repeat keratoplasty were assessed. RESULTS: A total of 382 consecutive eyes with keratoconus underwent

The Effects of Distance Between the Corneal Apex and the Inner and Outer Corneal Wound Edges on Corneal Astigmatism After Cataract Surgery.

PURPOSE: To compare the effects of the distance from the inner and outer corneal incision wounds to the corneal apex on the changes in corneal astigmatism after cataract surgery. METHODS: Seventy-two eyes with anterior corneal astigmatism less than 2.50 diopters (D) were evaluated. The distance between the incision wound and the corneal apex was measured using the Pentacam (Oculus Optikgeräte, Wetzlar, Germany) Scheimpflug image and the Image J program (National Institutes of Health, Bethesda, MD). Patients were subgrouped into the inner incision distance close (less than 4,816.44 µm) and far

OBJECTIVE: The purpose of the study was to evaluate the efficacy of orthokeratology in correcting myopia. METHODS: The study took place over three ophthalmology practices, which were trained in fitting corneal reshaping therapy lenses. The patients' age ranged from 9 to 59 years. The patients were split into 5 groups: 9 to 13, 14 to 21, 22 to 37, 38 to 42, 43 to 48, and 49 to 59 years. There were a total of 260 eyes being tested and the manifest refraction ranged from -0.25 diopters (D) to -7.75 D with astigmatism up to 1.50 D. Patients were fit using a calculator slide rule program and also

Comparison of keratometric measurements obtained by the Verion Image Guided System with optical biometry and auto-keratorefractometer.

The aim of this study was to compare the keratometric measurements of Verion Image Guided System with an optical biometer (Zeiss IOLMaster 500, Carl Zeiss Meditec, Jena, Germany) and an automated keratorefractometer (AKR) (Topcon KR-8900, Topcon, Japan). In this prospective clinical trial, the right eyes of 52 patients with cataract were examined (mean age 62.25 ± 12.16 years). The measurements were taken by the three systems in a random order. Keratometric data, magnitude of astigmatism, and astigmatic axis measurements from all three instruments were compared. The results were evaluated usin

Bilateral Ectasia After Femtosecond Laser-Assisted Small Incision Lenticule Extraction (SMILE).

PURPOSE: To describe a case of bilateral ectasia after small incision lenticule extraction (SMILE) in a patient with early keratoconus. METHODS: Case report. RESULTS: Bilateral SMILE was performed on a patient even though preoperative topographies showed changes indicating early keratoconus. The right eye underwent further photorefractive keratectomy enhancement 18 months later. The patient developed a bilateral corneal ectasia. CONCLUSIONS: This case underlines the importance of thorough preoperative assessment for possible keratoconus suspect changes with corneal topography to avoid posto

PURPOSE: To evaluate the effects of atopic syndrome on manifestations of keratoconus. METHODS: In this retrospective study, we reviewed patient files and data generated by Scheimpflug imaging of 670 eyes of 434 keratoconus patients. Patients were divided into a study group consisting of patients suffering from atopic syndrome (110 eyes of 75 patients), namely allergic asthma, atopic dermatitis, and/or allergic rhinitis, and a control group of patients without known atopic syndrome (560 eyes of 359 patients). RESULTS: We found a significant difference with the mean age being 36.1 ± 11.7 for t

Corneal Diameter as a Factor Influencing Corneal Astigmatism After Cataract Surgery.

PURPOSE: To evaluate the corneal horizontal diameter [white-to-white (WTW) distance] as a factor influencing surgically induced astigmatism (SIA) and postoperative astigmatism. METHODS: A total of 330 eyes with corneal astigmatism ≤1.5 D underwent cataract surgery with phacoemulsification. A 3-step, superotemporal for the right eye and superonasal for the left eye, clear corneal incision of 3.0 mm was made. Four groups were created according to WTW distance: group A ≤11.6 mm, group B 11.7 to 11.9 mm, group C 12.0 to 12.2 mm, and group D ≥12.3 mm. SIA was calculated by vector analysis using th

Effect of obstructive sleep apnea syndrome on corneal thickness.

Obstructive sleep apnea syndrome (OSAS) might be a risk factor for the development of eye disorders. The aim of the study was to evaluate the effect of OSAS on central corneal thickness (CCT). A total of 195 patients were enrolled in the study, and underwent polysomnography. Patients were divided according to their apnea-hypopnea index (AHI) scores into control group (AHI < 5), mild (AHI, 5-15), moderate (AHI, 15-30), and severe OSAS (AHI > 30) groups. In ophthalmological examinations, CCT, auto refractometer measurement, tear break-up time, and Schrimer's test results were evaluated. Central
