We studied 41 patients with urethral stricture who had failed at least 1 operation, and were being maintained with filiform and follower dilation every 6 to 12 weeks. The patients were instructed in clean intermittent self-catheterization with a 16F red rubber catheter, which was performed every 1 to 30 days. Followup from 9 to 36 months revealed excellent compliance and average peak uroflow rates increased from 5.5 cc per second before dilation to 17.1 cc per second at the last followup visit. Urethral strictures associated with the use of a urethral catheter may be more common after cardiac and aortic surgery when compared with other surgical procedures. The reasons for this are obscure. Fifty-two aortic procedures in males from 1980-1983 were reviewed with an incidence of urethral stricture of 21%. Forty anterior resections of the rectum in which a urethral catheter was used were also reviewed with an incidence of urethral stricture of only 5%. The stop-flow method in the pig, with modified kidneys, produced patterns for sodium, chloride, glucose and PAH that were similar to those obtained previously in other animals with simple kidneys. 2. Ethacrynic acid, under the conditions of the experiment, abolished distal sodium and chloride reabsorption in the pig. 3. We conclude that this method, viewed in perspective, could provide useful in situ information about the physiology of modified kidneys where the more difficult micropuncture and in vitro perfusion techniques are impractical.

Between 1973 and 1986, 57 patients presented with a fracture of the pelvis with rupture of the posterior urethra. All of these patients were treated as an emergency and were then followed periodically with a minimal follow-up of 2 years. We describe an alternative method for the treatment of an entrapped stone basket. When efforts to remove an engaged stone through the intramural ureter met with marked resistance, the basket with the stone within it was advanced into the renal pelvis. Extracorporeal shock wave lithotripsy then was used to pulverize the stone, allowing successful removal of the basket.

A 15F flexible fiberoptic choledochonephroscope was used to perform bedside flexible cystoscopy in 5 critically ill patients. All 5 patients were receiving pressor agents, and 4 were intubated and dependent on a respirator. A ureteral stent was placed in the renal pelvis in 3 patients. The flexible endoscope enables the urologist to bring diagnostic and therapeutic cystoscopy directly to the bedside.

Massive hematuria of renal origin was diagnosed in 4 dogs. In all dogs, blood and blood clots were clearly visible in the urine. Serum urea nitrogen and urine concentrating ability were normal. All dogs were anemic, and results of coagulation and platelet function tests were within normal limits. Computed tomography and ultrasonography are effective methods for diagnosis and localization of renal and perinephric abscesses. In patients with clinical suspicion of sepsis, diagnostic needle aspiration of these lesions can be safely performed extraperitoneally by using sectional imaging for guidance. Three ureter lesions in abdominoperineal rectum amputations are described. The surgical literature on this subject is discussed. Particularly dangerous points in the procedure of abdominoperineal rectum amputation concerning injuries of the ureter are accentuated. Possibilities of avoiding and recognizing early ureter injuries are indicated. More attention should be given to intraoperative lesions of the ureter.

Septicaemia in surgical patients usually occurs after operation or a manipulative event. The incidence of bacteraemia in one surgical unit was observed over a 7-year period. Gram-negative infections occurred three times more frequently than bacteraemia caused by Gram-positive bacteria. The clinical features of septicaemia, the portals of entry of the infection and the bacteria isolated from blood cultures are described.
