The author evaluates diagnostic and therapeutic results in nine patients with Behcet's disease investigated at the uveological ambulance of the First Ophthalmological Clinic, Medical Faculty Charles University in Prague during 1987-1996. Anterior uveitis with hypopyon was recorded twice as frequently as retinal vasculitis. Chronic CME, secondary glaucoma and occlusive vasculitis were the most serious complications of ophthalmological manifestations of BD. Ulcerations of connective tissue and skin manifestations of BD were found most frequently in the authors patients. In two thirds of the pati…

A case of polyarteritis nodosa identified by the American College of Rheumatology (ACR) 1990 criteria in a 44-year-old HIV-infected man is described. The search for cytomegalovirus, HBV and B19 parvovirus infections was negative. In situ hybridization did not reveal proviral HIV-1 DNA in a skin sample. A zidovudine-associated vasculitis was excluded. Corticosteroid therapy resolved vasculitis manifestations and was well tolerated without opportunistic infections during the 10-month follow-up period. An indirect pathogenetic role of HIV as a possible cause of vascular damage cannot be excluded …

Allergic granulomatosis and angiitis (Churg-Strauss syndrome) is characterized by a systemic vasculitis in the context of bronchial asthma and eosinophilia. Pathologically, there is a necrotizing vasculitis of small arteries and veins with extravascular granulomas, infiltration of vessels and perivascular tissue with eosinophilia. Pulmonary, peripheral nervous system, cardiovascular and cutaneous manifestations are most frequently encountered. Renal failure and high blood pressure are more rare. These features, within the setting of bronchial asthma and eosinophilia, differentiate Churg-Straus…

Wegener's granulomatosis is a distinct clinicopathologic entity characterized by granulomatous vasculitis of the upper and lower respiratory tract and kidneys. A 56-year-old Japanese man with Wegener's granulomatosis died of intraperitoneal hemorrhage from a ruptured aneurysm of his left gastric artery. Histological findings revealed necrotizing granulomatous vasculitis involving this artery. No previously published reports have described Wegener's granulomatosis patients with aneurysms in such a large artery. We conclude that this case represents either an atypical form of Wegener's granuloma…

Behçet's disease is generally known as an autoimmune disease. We report a case of Behçet's disease who had fluctuating hearing loss, tinnitus and dizziness. A 37-year-old woman had bilateral hearing loss in low audiometric frequencies. Audiological and vestibular examinations revealed an inner ear lesion. The patient had an enlarged negative summation potential in the electrocochleogram and a positive glycerol test, which suggested endolymphatic hydrops. The pathological characteristic of Behçet's disease is thought to be vasculitis. So this case suggests an association of autoimmune vasculiti…

Antiphospholipid antibody syndrome (APS) has been shown to be associated with arterial and venous thromboses, recurrent miscarriages, and thrombocytopenia. We describe 3 cases of adult respiratory distress syndrome (ARDS) in patients with primary APS. The autopsy in one patient revealed no evidence of vasculitis, but extensive small vessel thrombosis of multiple organs including the lungs was seen. No infectious process, connective tissue diseases, or pulmonary edema could be documented and there was no clinical or laboratory evidence of vasculitis in the other 2 patients. The 2 patients who s…

Two patients with localized polyarteritis nodosa involving the breast are reported. They presented with tender breast masses which were excised because of suspicion of carcinoma. The pathological features were similar with vasculitis of medium-sized and small arteries. The medium-sized arteries showed intimal fibroplasia, fragmentation of internal elastic lamina and inflammatory cellular infiltration. Fibrinoid vasculitis involving small arteries indicated a diagnosis of polyarteritis nodosa rather than giant cell arteritis. Both patients remain well. The differential diagnosis of these two ar…

We describe a previously healthy man who presented with features consistent with Wegener's granulomatosis. While undergoing investigation, he developed acute respiratory failure, thought to represent progression of his vasculitis. Open lung and sinus biopsies were performed to obtain the diagnosis. Vasculitis was confirmed on the paranasasl biopsy, and the lung biopsy showed pneumonia due to Legionella pneumophila, an association not previously reported in Wegener's granulomatosis. If immunosuppressive therapy had been started without making the diagnosis of Legionella pneumonia on lung biopsy…

We report two cases of serious intracerebral haemorrhage occurring in young women following their first use of oral medications containing catecholaminergic agents (phenylpropanolamine in combination with ephedrine or pseudoephedrine). Both women were previously well, and there was no evidence for systemic vasculitis, coagulopathy, aneurysm or arteriovenous malformation. Angiography in both cases, performed approximately forty hours following drug ingestion, revealed the beading pattern typical of that seen in previously reported cases of presumed amphetamine-induced "vasculitis." We believe t…

Involvement of visceral organs is a recognized condition of Reiter's Syndrome. To our knowledge, however, involvement of the colon, as it is seen in other rheumatoid-like processes mostly caused by vasculitis, has not been described before. We present the case of a patient with incomplete Reiter's Syndrome with arthritis, circinate balanitis and psoriasilike lesions of skin and nails who developed ischemic colitis. Angiographic features point to vasculitis as a potential cause, although this could not be proved. It is possible but, as we believe, unlikely that simultaneously administered stero…

Relapsing polychondritis is a systemic disorder characterized by recurrent inflammation and degeneration of cartilaginous tissue. The eyes, ears, nose, larynx, trachea, and articular areas are commonly involved. While ocular involvement, hearing, and vestibular dysfunction are frequently present, aortic valve and root disease as well as vasculitis also may occur. While the cause remains unknown, an autoimmune pathogenesis appears likely. Thus, dapsone and/or systemic corticosteroids remain the predominant effective therapies. Another case of relapsing polychondritis with an associated vasculit…

Activation of the kallikrein-kinin system, as indicated by increased plasma kallikrein and depleted plasma kininogen, prekallikrein, and kallikrein inhibitor, was observed in five patients with Rocky Mountain spotted fever. Four of the patients had petechial rashes characteristic of vasculitis. Three patients had alterations in coagulation consistent with disseminated intravascular coagulation, although no hemorrhagic syndrome was found. Our data, along with the known physiologic actions of kinins, suggest a possible role for the kallikrein-kinin system in the pathophysiology of vasculitis, di…

Epstein-Barr virus (EBV) plays a role in the pathogenesis of a wide variety of acute and chronic diseases. Cutaneous manifestations of EBV include exanthems, chronic infections, malignancies, and small vessel vasculitides. Clinically and histologically, many of these conditions can imitate each other, making diagnosis challenging. In this report, we present a case of a child who developed Gianotti-Crosti syndrome with histopathologic features of anti-neutrophil cytoplasmic antibody (ANCA) vasculitis in the setting of acute EBV infection. Furthermore, we review the literature on EBV-induced cut…

A woman in her late 70s presented with a fever, rashes, and marked proximal muscle weakness. Noninfectious conditions, including myositis and vasculitis, were initially considered. Treatment with cephalosporins was ineffective, but the symptoms improved with minocycline, indicating possible Japanese spotted fever (JSF) despite no apparent history of tick exposure. The diagnosis was confirmed by serological tests. A skin biopsy revealed leukocytoclastic vasculitis with immune complex deposition, suggesting muscle damage due to immune complexes. This case underscores the need to consider JSF in …

Behçet's disease (BD) is an autoimmune disease characterized by multisystemic variable-vessel vasculitis and oral, genital, and intestinal ulcers. Neurological involvement or "Neuro-Behçet" (NB) manifests due to parenchymal inflammation. We present the case of a 21-year-old male with a five-year-old history of intermittent chronic oral and genital ulcers who presented with headache, right hemiparesis, progressive loss of visual acuity, and a thalamic tumor-like lesion on magnetic resonance imaging (MRI). A brain biopsy showed multiple perivascular infarcts associated with vasculitis affecting …

Wegener's granulomatosis is a serious autoimmune disorder characterized by necrotizing small-vessel vasculitis. It is a multisystem disease that primarily affects the lung and kidneys. Previous studies indicated few relapses of vasculitis after hemodialysis due to uremic immunosuppression. Our case report describes an end-stage renal failure patient who had developed non-caseating lung granulomata with giant cell formation and fibrinoid necrosis of arterial media that is consistent with Wegner's granulomatosis for the first time and eight years after initiation of maintenance hemodialysis. We …

Wegener's granulomatosis (WG) is a primary systemic vasculitis involving predominantly the upper and lower respiratory tracts and the kidneys. Few diseases can cause deterioration of renal function and progression to renal failure as rapidly as WG. We describe a woman with WG who developed rapid progression to renal failure within 10 days of starting azathioprine prescribed as maintenance therapy for vasculitis. Her renal biopsy showed acute tubulointerstitial nephritis and no active glomerulonephritis. This case illustrates the importance of the history and urine sediment in differentiating d…

Wegener's granulomatosis is a necrotizing vasculitis disorder that usually presents with clinical involvement of the upper and lower respiratory tract in association with renal disease. Although Wegener's granulomatosis can affect other systems, such as the eye, skin, joints, muscle, nerves, gastrointestinal tract and heart, cardiac involvement is traditionally believed to be rare, even though coronary vasculitis can be demonstrated at postmortem examination. We report a patient who has both respiratory and renal involvement who died unexpectedly following a silent myocardial infarct after a p…

Giant cell arteritis (GCA) is a systemic vasculitis primarily affecting large and medium sized vessels. While the disease may present with blindness or other signs of extracranial vasculitis, symptoms referable to the peripheral nervous system are uncommon. We describe 2 patients with biopsy proven GCA who simultaneously developed peripheral neurologic lesions. The first developed a mononeuritis multiplex superimposed on a diffuse, primarily sensory and distal polyneuropathy; the second, a symmetric, primarily motor and distal polyneuropathy. We review the published experience of GCA with peri…
