A comparison of dog bite injuries in younger and older children treated in a pediatric emergency department.

OBJECTIVE: Dog bites account for a significant number of traumatic injuries in the pediatric population that often require medical treatment. Although agent, host, and environmental characteristics of dog bites have been well documented, no attempt has been made to compare these characteristics by patient age group. The purpose of this study is to determine if differences exist in agent, host, and environmental characteristics among younger (< or =6 y) and older (>7 y) patients treated in a pediatric emergency department (ED) for dog bites. Findings from our study could be used to develop age-

Radiation tolerance of the cochlear nerve at the gamma-knife in rabbits.

Since the first treatment of acoustic neurinoma using the gamma-knife by Leksell, a series of cases have been reported with good control rates. However, the most frequent complication is delayed hearing loss which occurs in more than 50% of patients. The purpose of this study was to define a safe dose by analyzing the radiosurgical dose-response relationship and histological effects on the normal cochlear nerve in rabbit. The rabbits had computed tomography (CT)-guided stereotactic radiosurgery on their cochlear nerves in the internal auditory canal with a 4 mm collimator focusing of a gamma-u

Cutaneous Nocardia farcinica infection in a nonimmunocompromised patient: case report and review.

Nocardia farcinica, the etiologic agent of bovine farcy, is microbiologically related to but distinct from Nocardia asteroides. N. farcinica is noted for its propensity to cause serious systemic infection in both normal and immunocompromised hosts and its marked degree of resistance to multiple antimicrobial agents. We present a case in which a nonimmunocompromised patient who sustained a contaminated facial laceration developed an abscess due to N. farcinica with underlying osteomyelitis. The severity of the infection necessitated surgical debridement followed by administration of intravenous

[Eight years experience of pain clinics (author's transl)].

The first pain clinic in Japan was organized and operated by anesthesiologists at the University of Tokyo. Today, many universities and large hospitals have set up pain clinics in their anesthetics department and have made great progress in the management of diseases in this filed. Almost all those institutions use oriental methods like acupuncture, ryodoraku and kyo in addition to standard methods. At our pain clinic, nerve block has been the sole method from the beginning. Neither drugs (except carbamazepine, imipramine and ergotamine) nor oriental methods have been used. The pain clinic oug

Frequency and pattern of skull base and vault fracture in isolated head trauma.

PURPOSE: In the literature, the number of studies on cases of injury due to isolated head trauma is limited. In addition, there are very few studies and publications on skull base and vault fractures in the forensic literature. This study aims to investigate the frequency and pattern of skull base and vault fractures in individuals exposed to isolated fatal and non-fatal head trauma. METHODS: This retrospective cross-sectional and descriptive study includes 1260 cases who were injured due to isolated head trauma during the 10-year period between Jan 1, 2010 and Nov 30, 2019. In cases of fatal

Utility of customized 3D compression mask with pressure sensors on facial burn scars: A single-blinded, randomized controlled trial.

PURPOSE: A pressure of approximately 15-25 mmHg is used for effective compression therapy to prevent and treat hypertrophic scar formation in patients with burns. However, conventional facial compression garments present challenges owing to inadequate pressure distribution in curved areas such as the cheeks, around the mouth, and the slope of the nose. This study aimed to evaluate the utility of a custom-made 3D compression mask equipped with pressure sensors to treat facial burn scars. METHODS: This single-blinded, prospective randomized controlled trial was conducted between May and October

Novel technique using surgical scrub sponges to protect the nose and face during prone ventilation for coronavirus disease 2019.

BACKGROUND: Coronavirus disease 2019 is an international pandemic. One of the cardinal features is acute respiratory distress syndrome, and proning has been identified as beneficial for a subset of patients. However, proning is associated with pressure-related side effects, including injury to the nose and face. METHOD: This paper describes a pressure-relieving technique using surgical scrub sponges. This technique was derived based on previous methods used in patients following rhinectomy. CONCLUSION: The increased use of prone ventilation has resulted in a number of referrals to the ENT te

Pediatric Facial Trauma.

Pediatric facial fracture management is often complex and demanding. The structure and topography of the pediatric craniofacial skeleton are profoundly different from the mature skull. Consequently, the pediatric facial skeleton responds differently to traumatic force. Although the incidence of pediatric facial trauma is higher than in the adult population, the incidence of facial fracture is significantly lower. The management in younger patients is often more conservative because of potential growth impairment. As the facial skeleton matures, more conventional surgical approaches become appr

Intraoperative Surgical Navigation Reduces the Surgical Time Required to Treat Acute Major Facial Fractures.

BACKGROUND: Assessing bone reduction and implant placement in facial fractures is time-consuming because of limited visibility. An intraoperative navigation system allows real-time confirmation of bone positioning and implant placement on the patient's computed tomographic scan. This circumvents the visibility problem and therefore appears to shorten the surgery time. The goal of this study was therefore to determine whether intraoperative navigation reduces the surgical time required to treat patients with acute major facial fractures. METHODS: In this retrospective quasi-experimental study,

High-resolution characterization of a PACAP-EGFP transgenic mouse model for mapping PACAP-expressing neurons.

Pituitary adenylate cyclase-activating polypeptide (PACAP, gene name Adcyap1) regulates a wide variety of neurological and physiological functions, including metabolism and cognition, and plays roles in of multiple forms of stress. Because of its preferential expression in nerve fibers, it has often been difficult to trace and identify the endogenous sources of the peptide in specific populations of neurons. Here, we introduce a transgenic mouse line that harbors in its genome a bacterial artificial chromosome containing an enhanced green fluorescent protein (EGFP) expression cassette inserted

A Neuromonitoring Approach to Facial Nerve Preservation During Image-guided Robotic Cochlear Implantation.

HYPOTHESIS: A multielectrode probe in combination with an optimized stimulation protocol could provide sufficient sensitivity and specificity to act as an effective safety mechanism for preservation of the facial nerve in case of an unsafe drill distance during image-guided cochlear implantation. BACKGROUND: A minimally invasive cochlear implantation is enabled by image-guided and robotic-assisted drilling of an access tunnel to the middle ear cavity. The approach requires the drill to pass at distances below 1  mm from the facial nerve and thus safety mechanisms for protecting this critical

Total cheek resurfacing with split ascending neck flap: a new approach.

BACKGROUND: Burn scars and other forms of extensive cheek deformities are a tragedy for patients and pose a great challenge to surgeons due to limited availability of well-matching donor sites. The skin of distant regions and skin transplants contrasts with the facial skin. The most suitable site for tissue in cheek reconstruction is the neck, but this resource is limited. Cervical skin expansion is often complicated by tissue necrosis. A new approach (technique) for resurfacing the scarred cheek with a split cervical flap is presented in this paper. METHODS: Sixteen patients with extensive (
