Effects of closed- versus open-system intensive care units on mortality rates in patients with cancer requiring emergent surgical intervention for acute abdominal complications: a single-center retrospective study in Korea.

BACKGROUND: In this study, we aimed to compare the in-hospital mortality of patients with cancer who experienced acute abdominal complications that required emergent surgery in open (treatment decisions made by the primary attending physician of the patient's admission department) versus closed (treatment decisions made by intensive care unit [ICU] intensivists) ICUs. METHODS: This retrospective, single-center study enrolled patients with cancer admitted to the ICU before or after emergency surgery between November 2020 and September 2023. Univariate and logistic regression analyses were cond…

Mortality and complications following total aortic arch replacement: 14 years' experience.

BACKGROUND: Open total arch replacement is one of the most challenging procedures in cardiothoracic surgery and is the gold standard treatment for aortic arch pathology. Total arch replacement is associated with high rates of mortality and neurological morbidity. Using 14 years of data, we studied postoperative, in-hospital mortality, major complications, and examined associated risk factors. MATERIALS AND METHOD: Medical records of patients who underwent open, total arch replacement surgery at Siriraj Hospital from 2006 to December 2019 were reviewed. Demographic data, clinical factors, preo…

Morbidity and mortality among patients with head and neck cancer in the emergency department: A national perspective.

BACKGROUND: Emergency departments are playing an increasing role in cancer management. Emergency department utilization by patients with head and neck cancer, however, is unknown. METHODS: The 2009-2011 Nationwide Emergency Department Sample was queried for patients with a principle diagnosis of head and neck cancer. Descriptive analysis was performed to characterize patient and hospital characteristics, outcomes, and charges. Logistic regression identified predictors of admission and mortality. RESULTS: A total of 31 390 patients were seen in the emergency department with head and neck canc

In adjusted models, smoking was associated with lower in-hospital mortality (relative risk, 0.74; 95% CI, 0.62-0.89 [P=0.001]) but not with in-hospital stroke/transient ischemic attack or myocardial infarction. [How much does a caesarean section cost in Madagascar? Socio-economical aspects and caesarean sections rate in Toamasina, Madagascar 1999-2001].

The maternal mortality is a major issue of public health in developing countries. Essential obstetric cares, especially caesarean section, play a crucial role in the decrease of maternal mortality. WHO, FNUAP and UNICEF estimate that the minimum acceptable rate of caesarean section in developing countries must reach 5% to guarantee safety for both the new-born and the mother. In Madagascar; the average national rate of caesarean section was 0.6% in 1997. In the area of Toamasina, this rate was 0.7%. To increase the number of women who can access to those essential obstetric cares, several solu

[Gynecology-obstetrics at the Yalgado-Ouédraogo National Hospital Center. Eclampsia: epidemiologic, clinical and prognostic aspects].

We present the results of a retrospective study carried out between 1992 and 1995 aimed at describing the epidemiological, clinical and developmental profile of eclampsia in an African maternity unit towards the end of the 20th Century. The incidence of eclampsia was 108 cases in 12,175 births (0.89%), mostly in young patients during their first pregnancy. 40.7% of the patients were less than 20 years old and 59.3% were expecting their first child. Eclampsia occurred between the 28th and 37th weeks of amenorrhea in 37% of cases. Thirty four patients (31.5%) had had at least three episodes of e

Prolonged Length of Stay in the Emergency Department and Mortality in Critically Ill Elderly Patients with Infections: A Retrospective Multicenter Study.

BACKGROUND: This study aimed at investigating whether the length of stay (LOS) in the emergency department (ED) is associated with mortality in elderly patients with infections admitted to the intensive care unit (ICU). Delayed admission to the ICU may be associated with adverse clinical outcomes in elderly patients with infections. METHODS: This was a retrospective study conducted with subjects over 65 years of age admitted to the ICU from 5 EDs. We recorded demographic data, clinical findings, initial laboratory results, and ED LOS. Outcomes were all-cause in-hospital mortality and hospital…

Methicillin-resistant Staphylococcus aureus among elderly patients with community-acquired pneumonia.

INTRODUCTION: The 2019 American Thoracic Society/Infectious Diseases Society of America guidelines for community-acquired pneumonia (CAP) recommend methicillin-resistant Staphylococcus aureus (MRSA) coverage for patients with prior sputum isolation of MRSA. This study aimed to determine the impact of MRSA coverage on in-hospital mortality in elderly patients with CAP among whom MRSA was isolated. METHODS: Consecutive elderly patients who were admitted for CAP and had positive sputum culture for MRSA were retrospectively included, and the association between MRSA coverage and in-hospital morta…
