A poll concerning elective treatment of gastroduodenal ulcer carried out in 1065 surgical hospitals of the Federal Republic of Germany (West) revealed that in cases of ulcer ventriculi type 1 the 2/3 resection is preferred in 91.5%. Billroth I is performed in 50.5%, Roux in 20% and Billroth II in 16.8%. Too, in cases of prepyloric ulcers the resecting procedure is favored. Various methods of vagotomies are dominating in duodenal ulcers (64.9%). There are only little differences in therapeutical concepts of gastroduodenal ulcer between university and nonuniversity hospitals.

We have pursued two blind studies on the plasma glycosphingolipids in patients with the Rett syndrome (RS), other disorders and normal individuals from Baltimore, USA, Vienna, Austria, and Rostock, East Germany. We found the presence of an unusual glycosphingolipid in 70% of patients with RS. Approximately 10% of the plasma from patients with other developmental disorders also contained this glycosphingolipid. Two cases of leptospirosis are described in soldiers who fell into a river together. One developed Weil's disease due to icterohaemorrhagiae serogroup and the other "Mud-Fever" due to grippotyphosa serogroup. The organisms were initially cultured in standard blood culture media. There were six cases of leptospirosis in BAOR in the 10 years to 1980 giving an incidence 10 times greater than in the civil population of UK and West Germany. Leptospirosis is an occupational disease of soldiers in BAOR and the health risks of fresh water immersion should be publicised.

Following the introduction of skateboards into Germany in 1976 an increasing number of accidents and injuries have been noted, affecting mainly children 10 to 14 years of age. The causes were lack of experience and the careless use of the boards on public streets. More than 30% of injuries were fractures mainly affecting the upper limb. Because of the frequent involvement of the epiphyseal plates the post-accidental growth may be seriously disturbed. Severe and sometimes fatal head or abdominal injuries have been reported, mainly after collision with cars.

Genomic surveillance detected clonal Escherichia coli sequence type-361 isolates carrying blaNDM-5, blaKPC-3, blaCTX-M-15, and rmtB1 from a patient in Ukraine and four wounded foreign soldiers evacuated to Germany. Isolates were non-susceptible to carbapenems, aminoglycosides, and cefiderocol and aztreonam/avibactam due to a PBP3 YRIN insertion and the blaCMY-145 AmpC β-lactamase. Coordinated surveillance efforts across civilian, military, and veteran healthcare systems are essential to prevent further spread as international volunteers return home after medical evacuation from Ukraine.

Percutaneus device closure of atrial septal defects offers an alternative to conventional surgical repair. While procedure techniques and devices have improved, severe complications remain that are neither completely understood nor entire predictable. We present a case of a 19-year-old female with left atrial erosion leading to lethal pericardial tamponade 1 month after ASD II closure using the Nit Occlude ASD-R® (NOASD-R, pfm Medical, Cologne, Germany). To our knowledge this is the first reported lethal complication related to this device.

A 2.5-year old male red-necked wallaby (Macropodus rufogriseus) kept privately in an outdoor enclosure in Germany died with severe jaundice and abdominal enlargement. Post mortem examination revealed ascites, and multiple nodular lesions in liver, diaphragm, omentum, mesentery, spleen, lung, hepatic and thoracic lymph nodes. Four alpha 1-adrenoceptor antagonists are currently available for the medical treatment of lower urinary tract symptoms suggestive of benign prostatic obstruction in Germany: alfuzosin, doxazosin, tamsulosin, and terazosin. Indirect comparison based on randomized, placebo-controlled trials as well as several direct comparative studies have shown equal efficacy of all four drugs on urinary flow and symptom relief if suggested dosing regimens are applied. A dosing regimen below this recommendation should be restricted to those patients who have a proven satisfactory response to a lower dose.

The Human Herpesvirus-6 (HHV-6) infection causes exanthem subitum (in typical cases) and a wide range of other symptoms. So far no prenatal or perinatal infections have been reported. For HHV-6 the prenatal infection should be extremely rare, because more than 95% of adults are immune in Germany. Two own cases reported here document that pre- and perinatal HHV-6 infection does occur and can provoke dramatic clinical symptoms of illness with death or cerebral defects like other prenatal herpesvirus infections (HSV, VZV, CMV etc.).

Measles cases in Germany are predominantly imported or import-associated. Two returning travelers with measles vaccine failure are reported: a 49-year-old male with primary vaccine failure after single-dose vaccination who developed severe measles pneumonitis, and a 22-year-old male with secondary vaccine failure in the context of suspected immunodeficiency who presented with a mild course and atypical exanthem. An undersupply of 25-(OH) vitamin D3 (calcifediol) exists in many countries with moderate sunlight, long winters and only moderate fish consumption. Risk groups for vitamin D3 deficiency are older persons over 65 years, geriatric persons in nursing homes, infants and children/adolescents. Therefore, there are also many situations in Germany which justify vitamin D substitution; however, vitamin D3 is currently praised as a "magic bullet" against everything. Para-aortic malignant lymphoma invading the thoracic aorta can cause aortic rupture. Thoracic endovascular aortic repair (TEVAR) is a good option to treat aortic rupture associated with para-aortic malignancies. It is essential to detect the exact tumour location during TEVAR; however, it is often difficult to confirm the location with conventional 2-dimensional fluoroscopic or angiographic images. We describe successful TEVAR using syngo DynaCT (Siemens AG, Forchheim, Germany) in a 64-year-old man with para-aortic malignant lymphoma invading the descending aorta.

Polyflex self-expanding stents (Rüsch, Germany) were used in three young children who had presented with life-threatening long-segment tracheal stenosis with bronchial stenosis in two cases.
