OBJECTIVE: To evaluate the incidence and identify associated risk factors of scleral penetrations and perforations resulting from strabismus surgery.
METHODS: A prospective observational study of patients undergoing strabismus surgery by residents or fellows was conducted. The surgical techniques employed included recessions, resections, advancements, posterior fixation sutures, and transpositions. Indirect ophthalmoscopy was performed on each operated eye following surgery to identify any retinal changes indicative of a penetration or perforation.
RESULTS: Of the 144 patients and 217 operat

Fabry disease is a rare X-linked lysosomal storage disorder caused by alpha-galactosidase A deficiency, leading to globotriaosylceramide accumulation in multiple organs, including the eye, where corneal verticillata represents a typical sign. This case report examines the effect of migalastat on corneal deposits in a female who experienced therapy interruption during two pregnancies. It emphasizes the importance of multidisciplinary care with ophthalmologic follow-up and explores long-term effects of migalastat on corneal deposits and disease progression.

A 61-year-old female patient with a previous diagnosis of Graves disease and mild orbitopathy presented with bilateral optic nerve calcifications and multiple areas of intracranial calcification. Neuro-ophthalmological evaluation did not detect any functional or anatomical compromise of the optic nerves. Her renal function and serum levels of calcium, phosphorus, and parathyroid hormone were normal. Two CT scans of the orbits, performed 7 years apart, showed stable intracranial and bilateral optic nerve calcifications.

Hyperviscosity is an uncommon manifestation of various underlying diseases. Rapid diagnosis and management of the underlying disease is crucial to prevent significant complications, including hypertension, cerebral vascular accidents, pulmonary embolism, bowel ischemia, and ophthalmologic pathologies. Although the acute management of complications arising from hyperviscosity is relatively straightforward, identifying and treating the underlying cause can prove difficult. This case highlights the difficulties of establishing a diagnosis and initiating appropriate management for a patient with h

The Vogt-Koyanagi-Harada syndrome (VKHS) is a unique form of granulomatous autoimmune disease that mostly impacts the pigmented tissues of the body. The main feature is bilateral granulomatous panuveitis, which is detected on ophthalmologic examination, along with additional systemic signs such as vitiligo, white hair, neurological involvement, or hearing loss. This study aims to report two cases of Vogt-Koyanagi-Harada syndrome presented in the children age group, which is unusual and very rare, to improve recognition of this disease to avoid complications and delay referral.

Chondrosarcomas are malignant cartilage-forming tumors representing less than 0.1% of all head and neck malignancies, and nasal cavity is an exceptional location among them. Clinical presentation could be consisting of either rhinological signs or asymptomatic endonasal mass. None of the previous reports mentioned ophthalmological symptoms at the forefront of the initial presentation. We report a case of sino-nasal low-grade chondrosarcoma in a patient who presented initially with isolated bilateral subtotal blindness, managed by endoscopic resection and adjuvant radiotherapy.

Tubulointerstitial nephritis and uveitis (TINU) syndrome in children is an uncommon, underreported condition. Onset of renal and ocular symptoms is often not simultaneous with nephritis which may precede uveitis by several months. A variable treatment response and a propensity for relapse characterize the syndrome. This is a case report of a previously healthy 14-year-old boy with TINU syndrome, emphasizing the need for long-term interdisciplinary treatment and monitoring. Also, the importance of repetitive screening of patients with uveitis for renal disease and ophthalmological evaluation of

The congenital absence of tear production or alacrima is a distinctively unusual clinical sign that harbors a wide variety of etiologies. While alacrima can be only isolated to the lacrimal system, it is more often associated with progressive multisystem involvement from underlying genetic disorders. Recognizing the subtle ocular signs in these diseases will promote a timely diagnosis and management before potential life-threatening consequences occur. Hence, the current article will review the ophthalmological findings, systemic manifestations, genetic associations, and differential diagnosis

Posterior scleritis is a rare sight-threatening condition that typically presents with an acutely painful eye, often with associated reduced visual acuity. Diagnosis can be challenging and requires specialist ophthalmological assessment. Consequences of delayed treatment include permanent loss of vision. We present a case of posterior scleritis initially managed as periorbital cellulitis. We highlight the importance of broad differential diagnoses when assessing painful periorbital swelling, and present a review of current management strategies for posterior scleritis.

The objective of this study was to report a case of congenital toxoplasmosis that illustrates the difficulties in diagnosing this disease. The case highlights the lack of prophylactic guidelines and shortcomings in the gestational screening process. It demonstrates the peculiarities of the non-specific clinical picture of the infection acquired during pregnancy and identifies the challenges of ophthalmological and high-sensitivity exams in newborns that are crucial for an early diagnosis. These factors contribute to a delay in early treatment of both the mother and the newborn. The lack of ski

Visual problems occur frequently in elderly patients and are often caused by ophthalmological problems. In this clinical lesson we show that visual problems can also be caused by posterior cortical atrophy (PCA). PCA is a clinico-radiological syndrome that is often caused by Alzheimer's disease and other neurodegenerative diseases as Lewy body dementia and corticobasal degeneration. It is characterized by progressive decline in visual information processing. In addition, there is relative retention of memory and language in early stages. Brain imaging shows atrophy of the posterior brain areas

Congenital ocular colobomas are due to incomplete closure of the fetal fissure during organogenesis. Ocular involvement can be variable ranging from a simple hole in the iris to a more severe involvement of the posterior pole (coloboma of the optic nerve, of the choroid, of the retina). We here report the case of a typical isolated bilateral iris coloboma. The study involved a 55-year old woman who discovered a typical inferonasal iris coloboma without involvement of the crystalline or of the posterior pole on ophthalmologic examination.

A young male with acute blurring of vision (6/9) complained of an inferior altitudinal field defect in right eye. Clinical ophthalmological examination was normal. Magnetic resonance imaging (MRI) of the brain revealed the expansion and mucosal thickening of right posterior ethmoid and sphenoid sinuses and opacified right maxillary sinus. Surgical intervention (transethmoidal sphenoidotomy) and histopathological examination revealed chronic invasive granulomatous fungal sinusitis. Anti-fungal therapy led to resolution of visual complaints and restoration of visual field defects.

Genetic skin diseases, or genodermatoses, often have extracutaneous manifestations. Ocular manifestations in particular can have significant clinical implications, like blindness. Other manifestations, such as the corneal opacities that occur in X-linked ichthyosis, are asymptomatic but characteristic of a particular genodermatosis. Ophthalmologic examination can aid in diagnosis when characteristic findings are seen. The genodermatoses with ocular manifestations will be reviewed, but neurocutaneous, syndromes, genetic pigmentary disorders, and genetic metabolic diseases are not included becau

Optokinetic nystagmus is a physiological oscillation of the eyes that requires intact development of motion perception, pursuit function, and saccadic function to be smoothly executed. Neuro-ophthalmological disorders often produce distinct perturbations in optokinetic responses that can be used to unmask the clinical diagnosis. With the advent of modern neuroimaging, optokinetic testing has slowly become a lost art in clinical examination. The purpose of this paper is to review the conditions wherein optokinetic testing provides a critical neurodiagnostic role, and to revitalize interest in t

Harlequin syndrome happens in only one side of the face. In the affected half, the face does not sweat or flush even with simulation. Arms and trunk could also be affected. This condition is induced by heat, exercise and emotional factors. The article reports a case of a nine-year-old female with a 3-year history of unilateral flushing and sweating after exercise; a brief literature review is included. Despite the rarity of this syndrome, dermatologists should recognize this condition and refer these patients to ophthalmological and neurological examination.

Ramsay Hunt syndrome is a special form of herpes zoster which is typically characterized by peripheral facial palsy and unilateral herpetic vesicles on the ear. These symptoms are often accompanied by vestibulocochlear dysfunction and other neurological and ophthalmological symptoms. The diagnosis and therapy requires a multidisciplinary approach. The authors present a typical case where the early administration of combined antiviral and systemic corticosteroid therapy led to complete recovery. The authors emphasize the importance of early diagnosis and adequate combination therapy, which impr

Rett syndrome is a neurological disease that occurs only in females and it manifests with mental retardation, seizures, movement disorders, autistic behavior and abnormal breathing. A 19-year-old female with Rett syndrome underwent ophthalmologic surgery under general anesthesia at our institution. Airway control was difficult due to her limited mouth opening. We recommend that anesthesiologists should have proper knowledge about this disease and the patients to avoid the complications and problems that can be encountered during the perioperative period.

Fractures of the facial skeleton can result in the loss of an aesthetically pleasing appearance and basic function, and many cases subsequently require an operative intervention. The surgeon managing these facial fractures must, at the same time, be cognizant of concomitant injuries, including neurologic, ophthalmologic, and cervical spine issues. For most situations, early stabilization in anatomical position using rigid fixation will give the most accurate reduction for the optimal return of preoperative appearance and function, while reducing long-term soft-tissue contracture.
