Case report: high-degree atrioventricular block associated with takotsubo cardiomyopathy: What is the right timing for pacemaker implantation?

BACKGROUND: Takotsubo cardiomyopathy (TCM) is a transient left ventricular systolic dysfunction typically triggered by emotional or physical stress. High-degree atrioventricular (AV) block is a rare but significant complication of TCM. Its occurrence raises complex management questions, particularly regarding the appropriate timing for permanent pacemaker implantation given the possible reversible nature of both conditions. CASE SUMMARY: We report the case of a 65-year-old woman with diabetes who was admitted for syncope and found to have a complete AV block. Echocardiography and cardiac MRI …

Association of cardiac sarcoidosis with coronary slow flow: a case report unraveling the enigma.

BACKGROUND: The exact mechanism of slow coronary flow (SCF) is still not well understood. SCF can manifest as acute coronary syndrome, or in rare scenarios, sudden cardiac death (SCD). Regional microvascular dysfunction (MVD) has been reported in inflammatory cardiomyopathies with a correlation between coronary flow reserve and active myocardial inflammation. CASE SUMMARY: A 52-year-old gentleman presented with refractory ventricular tachycardia (VT) following an aborted out-of-hospital cardiac arrest. Apart from one prior episode of uninvestigated syncope, there was no history of prior cardi…

Multisystemic Assessment in Andersen-Tawil Syndrome: Report of Eighteen Individuals.

Background/Objectives: Andersen-Tawil Syndrome (ATS) is an ultra-rare autosomal dominant condition secondary to deleterious variants in KCNJ … ents across eight families. In our cohort, the main clues that led probands to genetic testing were syncope (three families), which was associated with dysmorphic features in one case; LQTS (one family); asymptomatic premature ventricular contractions (PVCs) (three families); and a case incidentally identified during routine cardiac evaluations and due to …

Mad Honey Poisoning Caused by Grayanotoxin Toxicity in a 75-Year-Old Male Patient Presenting With Hypotension and Bradycardia in the United Kingdom: A Case Report.

Mad honey contains the neurotoxin grayanotoxin, which is produced by the flowers and nectar of the Rhododendron family. Bees incorporate this toxin into honey. Mad honey is consumed as a medicinal remedy in certain parts of the world, leading to persistent vagal activation and potentially serious complications, including hemodynamic instability. We report the case of a 75-year-old male patient with a medical history of hyperlipidemia and hypertension (managed with atorvastatin 20 mg and amlodipine 5 mg, respectively) who presented to the emergency department complaining of near-syncope, dizzin…

Riociguat as inpatient rescue therapy for acute decompensation of inoperable chronic thromboembolic pulmonary hypertension.

A man in his 70s, a retired coal miner with prior asbestos exposure and no smoking history, presented with syncope and progressive exertional dyspnoea. Initial CT pulmonary angiogram (CTPA) revealed bilateral lobar pulmonary emboli, and transthoracic echocardiography (TTE) showed evidence of pulmonary hypertension. He was discharged on rivaroxaban and referred to the heart failure and respiratory clinic. At the respiratory clinic follow-up, he reported worsening breathlessness and was initiated on long-term oxygen therapy (LTOT). Outpatient investigations were arranged, including a ventilation…

From valve prosthesis to coronary artery: a fatal thrombus: a case report.

BACKGROUND: Coronary embolism is an uncommon cause of acute myocardial infarction, often related to atrial fibrillation or valvular heart disease. In such cases, concomitant valvular thrombosis requires additional diagnostic work-up and has major implications for subsequent management. Both conditions may represent significant diagnostic and therapeutic challenges. CASE SUMMARY: We report the case of a 71-year-old woman with double mechanical valve prostheses (aortic and mitral) under suboptimal anticoagulation therapy, who presented with chest pain and syncope. Initial workup revealed severe…

Severe Anemia and Acute Kidney Injury After Pegloticase in a Patient With Glucose-6-Phosphate Dehydrogenase Deficiency.

Pegloticase is an effective therapy for refractory gout but carries a risk of oxidative hemolysis in patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency. Reported cases of hemolytic complications remain limited, particularly those with delayed presentation, where traditional markers such as haptoglobin and bilirubin may have normalized, making diagnosis challenging. We describe a 61-year-old female with known G6PD deficiency who presented with near syncope, nausea, vomiting, and volume depletion four days after receiving her first dose of intravenous pegloticase and recently star…

Age at Menarche and the Risk of Cardiac Events in Women With Congenital Long QT Syndrome.

BACKGROUND: Women with congenital long-QT syndrome (LQTS) experience increased risk of cardiac events (CE) after the onset of adolescence, possibly due to the effect of sex hormones on the cardiac ion channels. We hypothesized that late menarche may affect the risk of CE in women with LQTS. METHODS: Beginning in 2010, information on age at menses onset was obtained from all women enrolled in the Rochester LQTS Registry. Multivariable modeling was employed to evaluate the association of age at menarche with the burden of CE (total number of syncope, aborted cardiac arrest, and LQTS-related sud…

Case Report: Sudden cardiac death due to spasm of multiple coronary arteries.

Diffuse coronary artery spasm (DMV-CAS) is a serious vascular condition characterized by prolonged narrowing of two or more major coronary arteries or their main branches, leading to significant stenosis or blockage (≥70%). This can result in myocardial ischemia, heart attacks, and dangerous arrhythmias. A 68-year-old male with a four-year history of recurrent angina presented with acute-onset chest tightness, palpitations, and syncope. During transport, the patient experienced prehospital cardiopulmonary arrest, with transient return of spontaneous circulation (ROSC) achieved in the emergency…

Development of chronic subdural haematoma from mild head injury: A case report and review of current Malaysian guidelines on traumatic brain injury.

Delayed chronic subdural haematoma (cSDH) is a common but potentially serious complication following traumatic brain injury (TBI). Mild TBIs are commonly managed by primary care providers (PCPs), particularly in large, resource-limited settings such as Malaysia, where access to tertiary neurosurgical services may be delayed. Early identification of red-flag signs and symptoms and timely referrals are crucial to prevent clinical deterioration. We describe the case of a 66-year-old man who sustained mild head injury following a vasovagal syncope. His initial brain CT revealed evidence of a small…
