Retrograde CTO-PCI Using an Internal Thoracic Bypass Graft Segment in a Patient with Acute Inferior ST-Elevation Myocardial Infarction and Cardiogenic Shock.

BACKGROUND: Percutaneous coronary intervention (PCI) of the "culprit" artery is the recommended mechanical reperfusion strategy in the setting of ST-segment elevation myocardial infarction (STEMI). As PCI of bypass grafts may be associated with higher risks and lower procedural success rates, in patients with a history of previous coronary artery surgery, PCI directed at revascularization of the native vessels should be considered, but this may be difficult in the setting of a chronically occluded artery. CASE PRESENTATION: A patient with a history of multivessel coronary artery disease and a

Non-invasive Hemodynamic CMR Parameters Predicting Maximal Exercise Capacity in 54 Patients with Ebstein's Anomaly.

BACKGROUND: Exercise capacity is a well-defined marker of outcome in congenital heart disease. We analyzed seventeen cardiovascular magnetic resonance (CMR) derived parameters and their correlation to exercise capacity in patients with Ebstein's anomaly (EA). METHODS: Fifty-four surgery free patients, age 5 to 69 years (median 30 years) prospectively underwent CMR examination and cardiopulmonary exercise testing (CPET). The following volume/flow parameters were compared with peak oxygen uptake as the percentage of normal (peakVO2%) using univariate and multivariate analysis: right and left ve

From Snapshot to Trajectory: The RV-3 Check for Pulmonary Arterial Hypertension Follow-up.

Pulmonary arterial hypertension follow-up relies on composite clinical risk assessment, yet symptoms and functional markers may not always reflect right ventricular (RV) adaptation. We propose the RV-3 Check, a practical framework for serial RV assessment using three complementary domains: remodeling, systolic performance, and RV-pulmonary arterial coupling. Each domain is classified as improving, stable, worsening, or indeterminate, with overall RV direction assigned when at least two domains change concordantly. The framework is intended to identify clinically important discordance, particul

Secondary pulmonary hypertension in an adult patient with Ebstein anomaly post-atrial septal defect closure; can it be reversible? A case report.

BACKGROUND: Pulmonary hypertension is often a difficult to diagnose condition, in particular in younger population of patients. Early diagnosis and treatment of this condition is crucial to prevent further morbidity and mortality. CASE SUMMARY: A 32-year-old woman with a history of Ebstein anomaly (EA) and secundum atrial septal defect (ASD), who underwent transcatheter ASD closure at age 19, presented with progressive fatigue and exertional dyspnoea. Further evaluation revealed presence of compression of right pulmonary veins (rPVs) by the ASD device resulting in post-capillary pulmonary hyp

Electrophysiological effect of adenosine triphosphate and adenosine on atrial and ventricular action potential duration in humans.

Bolus injection of adenosine triphosphate (ATP) or adenosine is widely used clinically for terminating supraventricular tachycardia. However, bolus injection of these drugs has been reported to provoke atrial fibrillation (Afib). The effects of ATP and adenosine on the monophasic action potential duration (MAPD) of atrial and ventricular muscle was investigated, as well as the changes in the spatial distribution of atrial functional refractoriness caused by adenosine. Bolus injection of ATP and adenosine shortened atrial MAPD; no change was observed in the ventricle. Because local f-f interval

Wide Complex Tachycardia: An Enigmatic Sequence.

BACKGROUND: Differentiating perpetuated aberrant conduction from ventricular tachycardia in atrial fibrillation (AF) is challenging and may lead to inappropriate therapies. CASE SUMMARY: A 64-year-old man with ischemic cardiomyopathy and AF presented with decompensated heart failure. Admission electrocardiogram showed a wide-complex tachycardia with runs of up to 10 consecutive aberrant QRS complexes mimicking nonsustained ventricular tachycardia (VT). Careful analysis revealed perpetuated Ashman-type aberrancy best explained by concealed transseptal conduction. Abrupt normalization despite a

Determining Good Candidates for Atrioventricular Junction Ablation and Device Therapy and Which Device to Implant.

The 'ablate-and-pace' strategy, combining atrioventricular junction ablation with permanent pacing, has re-emerged as a valuable therapeutic option for patients with AF who are symptomatic, have poor rate control or develop AF-mediated cardiomyopathy. While historically considered a last-resort treatment, recent randomised trials and meta-analyses support its efficacy in improving functional status, reducing hospitalisations and potentially enhancing survival, particularly when paired with CRT or conduction system pacing. The success of this approach depends on careful patient selection and ap

Cardiac resynchronization therapy in becker muscular dystrophy.

A 44-year-old male patient with known Becker muscular dystrophy and concomitant non-ischemic dilated cardiomyopathy presented to our department because of worsening heart failure and presyncope. Upon admission, the patient was in New York Heart Association functional class III despite optimal pharmacological treatment; his ECG showed sinus rhythm with left bundle branch block and a wide QRS complex. Non-sustained ventricular tachycardia was recorded during 24-hour Holter monitoring. A complete three-dimensional echocardiographic study was performed and documented the dilatation and concomitant

Ventricle size, cortical atrophy and the relationship with neuropsychological status in closed head injury: a quantitative analysis.

Forty-eight patients with a history of moderate to severe closed-head injury (CHI) were examined with neuropsychological measures and computerized tomography (CT). CT scans were assessed with computerized techniques to determine estimated ventricular volumes (VV) and the degree of cortical atrophy (ATVOL) in each patient. VV's then were used to determine volumetric ventricle-brain ratios (VBR) to correct for head size. The WAIS, Wechsler Memory Scale (WMS) and Halstead-Reitan Neuropsychological Test Battery (HRNTB) were administered to all patients. Correlational analyses were undertaken betwe

The siRNA-mediated knockdown of AP-1 restores the function of the pulmonary artery and the right ventricle by reducing perivascular and interstitial fibrosis and key molecular players in cardiopulmonary disease.

BACKGROUND: Pulmonary hypertension (PH) is a complex multifactorial vascular pathology characterized by an increased pulmonary arterial pressure, vasoconstriction, remodelling of the pulmonary vasculature, thrombosis in situ and inflammation associated with right-side heart failure. Herein, we explored the potential beneficial effects of treatment with siRNA AP-1 on pulmonary arterial hypertension (PAH), right ventricular dysfunction along with perivascular and interstitial fibrosis in pulmonary artery-PA, right ventricle-RV and lung in an experimental animal model of monocrotaline (MCT)-induc

Impact of vagal effect on atrial and ventricular refractoriness and atrial fibrillation: A comparative analysis before and after cardioneuroablation.

BACKGROUND: Extracardiac vagal stimulation (ECVS) provides a simple, reproducible method to study human vagal effects. Although vagus nerve clearly modulates atrial electrophysiology, its influence on human ventricular refractoriness remains uncertain. Likewise, the impact of cardioneuroablation (CNA) on atrial and ventricular refractoriness has not been evaluated in a paired human study OBJECTIVE: This study aimed to quantify the impact of ECVS on atrial (AERP) and ventricular effective refractory periods (VERP) before and after CNA and evaluate a practical atrial fibrillation (AF) ablation f

Prevalence and predictive value of electrocardiographic abnormalities in pulmonary hypertension: evidence from the Pan-African Pulmonary Hypertension Cohort (PAPUCO) study.

BACKGROUND: Pulmonary hypertension (PH) is prevalent in Africa and is still often diagnosed only at an advanced stage, therefore it is associated with poor quality of life and survival rates. In resource-limited settings, we assessed the diagnostic utility of standard 12-lead electrocardiograms (ECG) to detect abnormalities indicating PH. METHODS: Sixty-five patients diagnosed with PH were compared with 285 heart disease-free subjects. The prevalence and diagnostic performance of ECG features indicative of PH and right heart strain were calculated. RESULTS: Compared to the control group, all

Assessing utility of exercise test in determining exercise prescription in adolescent and adult patients with repaired tetralogy of fallot.

Parameters from cardiopulmonary exercise test (CPET) are useful prognostic factors for patients with repaired tetralogy of fallot (TOF). Its application in exercise prescription remains unclear. This study sought to define its role. We made current exercise recommendations in repaired TOF patients according to European Society of Cardiology (ESC) guideline, which were based on ventricular function, pressure/volume load, pulmonary artery pressure, hypoxemia and arrhythmic burden both at rest and during exercise. CPET parameters (peak oxygen consumption, oxygen uptake efficiency plateau, and E/C

Anatomic repair for congenitally corrected transposition of the great arteries: a single-institution 19-year experience.

OBJECTIVE(S): Anatomic repair for congenitally corrected transposition of the great arteries (ccTGA) has been shown to improve patient survival. We sought to examine long-term outcomes in patients after anatomic repair with focus on results in high-risk patients, the fate of the neo-aortic valve, and occurrence of morphologically left ventricular dysfunction. METHODS: We conducted a retrospective, single-institution study of patients undergoing anatomic repair for ccTGA. A total of 113 patients from 1991 to March 2011 were included. Double-switch (DS) repair was performed in 68 patients, with

Determination of right ventricular end systole by cardiovascular magnetic resonance imaging: a standard method of selection.

For reproducible measurements of right ventricular (RV) volume and function, it may be important to use a consistent method to identify end systole (ES). We determined whether a significant difference exists between RV volumes measured using varying criteria from previous studies to define the timing of ES. In three normal subjects and nine patients with congenital heart disease, we measured RV volume from 3D reconstructions generated from 12 short and long axis magnetic resonance images (MRI). Cine frames analyzed included two frames before and three frames following ES, which we determined a

Exercise-induced atrioventricular block in a patient with a pacemaker: a functional consequence of AV search algorithms-a case report.

BACKGROUND: Algorithms designed to promote intrinsic atrioventricular (AV) conduction in dual-chamber pacemakers aim to reduce unnecessary right ventricular pacing and its long-term adverse effects. The AV Search+ algorithm periodically prolongs the AV delay to facilitate native conduction and is particularly useful in non-pacemaker-dependent patients. However, under certain conditions, such algorithms may lead to unexpected disturbances in AV synchrony, especially during exercise. CASE SUMMARY: A 65-year-old woman with symptomatic second-degree AV block (Mobitz II) underwent dual-chamber pac
