CD147 and Ki-67 overexpression confers poor prognosis in squamous cell carcinoma of oral tongue: a tissue microarray study.

OBJECTIVE: Squamous cell carcinoma of the oral tongue (SCCOT) exhibits high risk for recurrence and regional metastasis even after surgical resection. We assessed the clinicopathologic and prognostic significance of a group of functionally related biomarkers. STUDY DESIGN: We used a tissue microarray consisting of SCCOT from 32 patients for this study. These patients were treated at the University of Texas MD Anderson Cancer Center from 1995 to 2008. Biomarker expression levels were examined by immunohistochemistry and graded semiquantitatively to determine their prognostic significance. RES

Effects of ultrasound-guided stellate ganglion block on acute pain after arthroscopic shoulder surgery.

BACKGROUND AND OBJECTIVES: Apart from a few case reports, the effectiveness of stellate ganglion block (SGB) as a monotherapy in acute nociceptive pain has not been determined. We aimed to assess the effects of SGB on postoperative pain after arthroscopic shoulder surgery. STUDY DESIGN: Randomized, blind, controlled, clinical trial SETTING: University Hospital outpatient METHODS: Forty-six patients undergoing arthroscopic shoulder surgery were assigned randomly to 2 groups: group S included patients who underwent SGB prior to surgery and group C did not. In group S, subfascial ultrasound-gu

Deep venous thrombosis in patients with chronic spinal cord injury.

CONTEXT/OBJECTIVE: Deep venous thrombosis (DVT) is a well-known complication of an acute spinal cord injury (SCI). However, the prevalence of DVT in patients with chronic SCI has only been reported in a limited number of studies. The aim of our study was to examine the prevalence of DVT in patients with SCI beyond three months after injury. DESIGN: Cross-sectional study. SETTING: Rehabilitation Department at the Bydgoszcz University Hospital in Poland. PARTICIPANTS: Sixty-three patients with SCI that were more than 3 months post injury. The patients, ranging in age from 13 to 65 years, cons

Improving Outcomes by Implementing a Pressure Ulcer Prevention Program (PUPP): Going beyond the Basics.

A multidisciplinary process improvement program was initiated at the University of Miami Hospital (UMH) in 2009 to identify the prevalence of hospital-acquired pressure ulcers (HAPU) at the institution and to implement interventions to reduce the incidence of HAPU. This deliberate and thoughtful committee-driven process evaluated care, monitored results, and designed evidence-based strategic initiatives to manage and reduce the rate of HAPU. As a result all inpatient beds were replaced with support surfaces, updated care delivery protocols were created, and monitored, turning schedules were ad

Superior laryngeal nerve block: an anatomical study comparing two techniques.

STUDY OBJECTIVE: To determine whether an anterior approach is as successful as the conventional posterior approach to superior laryngeal nerve block. DESIGN: Prospective observational study. SETTING: University anatomy laboratory. PATIENTS: 20 formalin-fixed adult human cadavers. MEASUREMENTS: Simulated superior laryngeal nerve blocks were performed by a single operator using 3 mL of 0.01% analine blue dye with a 23-gauge Quincke tip needle. Two different landmark techniques were used on each undissected cadaver: 1) the conventional posterior approach using the hyoid bone as a palpable lan

Optimizing patient care and research: the Amsterdam Dementia Cohort.

Since its opening in 2000, patient care and research go hand in hand at the Alzheimer center of the VU University Medical Center, both organized in such a way that they mutually strengthen each other. Our mission is to give patients a voice by lifting the stigma on dementia, to find new diagnostic and treatment strategies, and, ultimately, to cure diseases that cause dementia. Our healthcare pathway is uniquely designed to accommodate all necessary investigations for the diagnostic work-up of dementia in one day (one-stop shop). A second unique feature is that research has been fully integrate

Hypercalcemic crisis: a clinical review.

Hypercalcemia is a common metabolic perturbation. However, hypercalcemic crisis is an unusual endocrine emergency, with little clinical scientific data to support therapeutic strategy. We review the relevant scientific English literature on the topic and review current management strategies after conducting a PubMed, MEDLINE, and Google Scholar search for articles published between 1930 and June 2014 using specific keywords: "hypercalcemic crisis," "hyperparathyroid crisis," "parathyroid storm," "severe primary hyperparathyroidism," "acute hyperparathyroidism," and "severe hypercalcemia" for a

Older Adults' Perceptions of Mobility: A Metasynthesis of Qualitative Studies.

PURPOSE OF THE STUDY: Optimal mobility is an important element of healthy aging. Yet, older adults' perceptions of mobility and mobility preservation are not well understood. The purposes of our study were to (a) identify studies that report older adults' perceptions of mobility, (b) conduct a standardized methodological quality assessment, and (c) conduct a metasynthesis of the identified studies. DESIGN AND METHODS: We included studies with community-dwelling adults aged ≥65 years, focused on perceptions of mobility pertaining to everyday functioning, used qualitative methods, and were cite

Endoscopic suture retriever for endolaryngeal keel placement in treatment of anterior glottic webs.

INTRODUCTION: Endoscopic placement of a laryngeal keel has traditionally required the use of a Lichtenberger endo-extralaryngeal needle passer, which is not universally available. We discuss a safe and technically simple alternate technique using an endoscopic suture retriever through a percutaneously placed angiocatheter that obviates the need for the Lichtenberger instrument. STUDY DESIGN: Case series. MATERIALS AND METHODS: Two 14-gauge angiocatheters were passed through the anterior neck under telescopic visualization of the larynx. The suture retriever was inserted through the catheter

Postpartum hemorrhage: state and national response.

PURPOSE OF REVIEW: The rise in maternal mortality has raised a significant concern for epidemiologists and providers. One of the most preventable and frequent causes of maternal death is hemorrhage-related events. Most providers of obstetrical care see such complications on a frequent basis, and the costs in maternal lives and blood usage have become a universal concern. As a result, a number of important responses by state and national groups have taken place to address these preventable bad outcomes. RECENT FINDINGS: A number of well designed step-by-step protocols have had success in preve

Muscle and bone effects of androgen deprivation therapy: current and emerging therapies.

Prostate cancer and treatment with androgen deprivation therapy (ADT) affect significant numbers of the male population. Endocrine effects of ADT are a critical consideration in balancing the benefits and risks of treatment on long-term survival and quality of life. This review highlights the latest advances in androgen manipulation in prostate cancer with an emphasis on the effects of ADT on muscle and bone, which universally affects the health and well-being of men undergoing ADT for prostate cancer. Muscle mass declines with ADT; however, the evidence that this correlates with a decrease in

Hereditary spherocytosis: Consequences of delayed diagnosis.

OBJECTIVE: To determine whether patients with undiagnosed hereditary spherocytosis hospitalized for transfusions might have avoided hospitalization via earlier diagnosis. Family and transfusion history and presence of neonatal jaundice were recorded. Complete blood count and reticulocyte values during infancy were available for 20 of 30 patients, while baseline steady-state values were available for all 30. RESULTS: Transfusions were g
