A clinician aligned vision language framework for stepwise interpretation in fundus fluorescein angiography.

Fluorescein fundus angiography (FFA) is essential for diagnosing retinal vascular diseases, yet its interpretation is expertise-intensive. Here, we present Clin-FFA-VLM, a multimodal vision-language framework that mirrors retina specialists' cognitive workflow by decomposing FFA interpretation into three stages: lesion-aware visual perception, clinical report generation, and diagnostic decision support. Trained and tested on a multi-center dataset of 13,178 FFA images with expert label, 21,717 FFA images with 1790 clinical reports and diagnosis across 7 retinal diseases, Clin-FFA-VLM achieves

[Leber's hereditary optic nerve atrophy].

This review is devoted to Leber's hereditary optic neuropathy (LHON), a rare maternally inherited ophthalmic disorder presenting as optic nerve atrophy and associated with an unfavorable prognosis. LHON is the most common primary mitochondrial DNA disorder, and most patients (more than 90% of cases) carry one of three point mutations. The 11778G>A mutation in the MT-ND4 gene is the most common worldwide and causes the most severe variant of the disease. LHON typically manifests as bilateral, painless, acute or subacute significant vision loss, most often in young men. Environmental factors pla

Bilateral simultaneous acute angle closure glaucoma following anticholinergic therapy for vestibular neuritis.

Attacks of bilateral simultaneous acute angle closure glaucoma are rare. A 59-year-old man developed vertigo and vomiting, with nystagmus in primary gaze, left-beating nystagmus in left gaze and a positive head impulse test. He was diagnosed with acute vestibular neuritis and received standard therapy, using four drugs that have anticholinergic effects. He subsequently developed bilaterally red eyes, initially diagnosed as bacterial conjunctivitis, and then blurred vision with ocular pain. An ophthalmological assessment identified mid-dilated, non-reactive pupils with corneal oedema and marked

A scalable framework for current steering at single-neuron resolution.

To restore and augment complex functions such as vision, electrical neural interfaces require the ability to selectively activate neurons at cellular resolution, a challenge given the limited electrode density of typical neural interfaces. Current steering - passing current through multiple electrodes simultaneously to target locations between electrodes - could enhance resolution, but is intractable if all stimulation patterns must be tested exhaustively. Here, we develop and experimentally validate a framework for scalable cellular-resolution current steering in the macaque and human retina.

Integrated optimization of energy storage and green hydrogen systems for resilient and sustainable future power grids.

This study presents a novel multi-objective optimization framework supporting nations sustainability 2030-2040 visions by enhancing renewable energy integration, green hydrogen production, and emission reduction. The framework evaluates a range of energy storage technologies, including battery, pumped hydro, compressed air energy storage, and hybrid configurations, under realistic system constraints using the IEEE 9-bus test system. Results show that without storage, renewable penetration is limited to 28.65% with 1538 tCO2/day emissions, whereas integrating pumped hydro with battery (PHB) ena

A case of progressive and irreversible visual loss as a consequence of delayed diagnosis in cerebrospinal fluid venous fistula.

PurposeCerebrospinal fluid venous fistulas (CVFs) are a frequently underrecognized cause of cerebrospinal fluid (CSF) leaks, leading to intracranial hypotension and a wide range of symptoms, including visual disturbances. This case highlights the critical role of ophthalmologists in identifying CVFs as a cause of unexplained progressive vision loss.MethodsWe present the case of a 45-year-old woman who developed progressive visual field loss over 15 years, ultimately resulting in blindness in one eye. Early neuroimaging and testing were normal despite symptoms of orthostatic headaches, photopho

A Single-Camera Method for Estimating Lift Asymmetry Angles Using Deep Learning Computer Vision Algorithms.

A computer vision (CV) method to automatically measure the revised NIOSH lifting equation asymmetry angle (A) from a single camera is described and tested. A laboratory study involving ten participants performing various lifts was used to estimate A in comparison to ground truth joint coordinates obtained using 3-D motion capture (MoCap). To address challenges, such as obstructed views and limitations in camera placement in real-world scenarios, the CV method utilized video-derived coordinates from a selected set of landmarks. A 2-D pose estimator (HR-Net) detected landmark coordinates in each

Clinical characteristics of EYS-associated retinal dystrophy in 291 Japanese patients.

Eyes shut homolog (EYS) is the most common autosomal recessive causative gene of inherited retinal dystrophy (IRD) in the Japanese population, yet genotype-phenotype correlation data remain limited. We analyzed 291 probands (141 males, 150 females) with IRD caused by EYS (EYS-RD) from eight Japanese facilities. Clinical variables included age at onset, initial symptoms, best-corrected visual acuity (BCVA), and its progression alongside genotype information. Mean onset was 25.8 ± 14.9 years, most often night blindness (67.0%), and rod-cone dystrophy was observed in 95.9%. Initial BCVA averaged

Child Neurology: Five-Year Update on Siblings With Riboflavin Transporter Deficiency: Stable Visual and Neurologic Status With Continued Riboflavin Therapy.

Riboflavin transporter deficiency (RTD), previously referred to as Brown-Vialetto-Van Laere syndrome, is caused by pathogenic variants in the SLC52A1, SLC52A2, or SLC52A3 genes, resulting in RTD types 1, 2, and 3, respectively. Researchers estimate an occurrence of approximately 1 in 1,000,000. There is only one case of type 1 described in medical literature. Type 2 is characterized by muscle weakness in the arms and neck, vision loss, hearing impairment, and sensory ataxia. In type 3, vocal cord paralysis is more common and muscle weakness is more generalized. In 2018, we described a case of

Tuberculous ciliary body granuloma initially diagnosed as bullous retinal detachment: a case report.

BACKGROUND: Ocular tuberculosis is a relatively rare extrapulmonary manifestation of tuberculosis. This vision-threatening disease is extremely challenging to diagnose, particularly because it can mimic other diseases. We report a case of tuberculous ciliary body granuloma initially diagnosed as bullous retinal detachment. CASE REPORT: A 52-year-old female presented with bullous retinal detachment in her left eye, and ultrasound biomicroscopy (UBM) verified the presence of a lesion with ciliary body granulomatous inflammation. The T-SPOT was positive, and the purified protein derivative (PPD)

Retinal Angiomatous Proliferation in a Patient with Retinitis Pigmentosa.

Retinal angiomatous proliferation (RAP) and other types of choroidal neovascularization (CNV) are very rarely reported in patients with retinitis pigmentosa (RP). We present a case report of a 91-year-old patient with an obvious RP phenotype, who presented with a sudden onset of vision worsening and metamorphopsia in the left eye. Genetic testing on the UK inherited retinal disease panel did not identify a pathogenic variant. Multimodal imaging comprising optical coherence tomography (OCT), OCT angiography, and fluorescein and indocyanine green angiography showed a RAP lesion in the left macul

Patient stem cell-derived in vitro disease models for developing novel therapies of retinal ciliopathies.

Primary cilia are specialized organelles on the surface of almost all cells in vertebrate tissues and are primarily involved in the detection of extracellular stimuli. In retinal photoreceptors, cilia are uniquely modified to form outer segments containing components required for the detection of light in stacks of membrane discs. Not surprisingly, vision impairment is a frequent phenotype associated with ciliopathies, a heterogeneous class of conditions caused by mutations in proteins required for formation, maintenance and/or function of primary cilia. Traditionally, immortalized cell lines

The direct influence of retinal degeneration on electrical stimulation efficacy: Significant implications for retinal prostheses.

Photoreceptor loss and inner retinal network remodeling severely impacts the ability of retinal prosthetic devices to create artificial vision. We developed a computational model of a degenerating retina based on rodent data and tested its response to retinal electrical stimulation. This model includes detailed network connectivity and diverse neural intrinsic properties, capable of exploring how the degenerated retina influences the performance of electrical stimulation during the degeneration process. Our model suggests the possibility of quantitatively modulating retinal ON and OFF pathways
