BACKGROUND: Hemodynamically significant congenital aortic stenosis is usually surgically solved by valvotomy, infrequently aortic valve replacement is necessary. The aim of this retrospective study (period 1979-1995) is to find reasons for valve replacement, frequency of these procedures and early and long term results after aortic valve replacement for congenital aortic stenosis.
METHODS AND RESULTS: Congenital aortic stenosis was solved by aortic valvotomy in 82 patients and by aortic valve replacement in 18 patients. Aortic valve replacement was performed 6x for congenital valvar aortic st

Thirteen year-old boy who received aortic valve replacement with Manouguian's anulus enlargement for congenital aortic stenosis 7 years ago developed valvular and supravalvular stenosis. Pressure gradient between left ventricle and aorta was 55 mmHg. The etiology of supravalvular stenosis was extensive patch plasty of ascending aorta and severe degeneration of expanded polytetrafluoroethylene patch. Moreover, white hard mass existed just outside the patch and was one of the components of the stenosis. This mass was acellular calcification without foreign body reaction. Degenerated prosthetic p

Chronic aortic valvar regurgitation is a particular problem with heart disease because symptoms develop insidiously and are too severe when the first opportunity arises for a low risk operation. The disease may run a long benign course, but delayed surgery may result in the failure to halt progression of left ventricular failure.

Authors presented difficulties in diagnosis of the cor triatriatum with the mitral incompetence in 34 years old female patient. Diagnosis was based on two-dimensional and doppler echocardiographic examinations and then angiocardiographically proved. Therapy consisted in resection of the fibromuscular intraatrial septum and implantation of the St. Jude mitral prosthetic valve.

To assess the role of emergency valve replacement in patients with active infective endocarditis (IE), we reviewed 30 patients who underwent valve replacement within 3 months of the diagnosis of IE. Eighteen patients fulfilling the criteria for active IE underwent emergency surgery. The aortic valve was involved in all cases and was previously thought to be normal in 12 (67%). There were 5 early deaths (27,7%), 4 due to circulatory failure and 1 due to rupture of a mycotic cerebral aneurysm. Prosthetic valve dehiscence (paravalvular leak) developed in only 1 patient, and prosthetic valve endoc

The major advance in the management of valvular heart disease over the past twenty years has been due to prosthetic valve operation. The bioprostheses have good hemodynamic function and a low rate of thromboembolism although the long-term durability is unknown. In this series of 25 patients with 27 valves implanted over five years, there was an 9% early mortality and a 12% late mortality. There have been no thromboembolic episodes. Primary valvular dysfunction requiring operation occurred in 7.4% (2 out of 27). All surviving patients are in New York Heart Association Functional Class I or II.

AIMS: Replacement heart valves that grow with children remain an unmet need. We previously reported valves fabricated from tubes of fibroblast-derived collagenous matrix increased in size while functioning with low systolic gradients and less than moderate regurgitation over 52 weeks in most cases, when implanted as interpositional grafts in the pulmonary artery of lambs. Here, we evaluated valved conduits fabricated by including an inflow segment to the tri-tube valve allowing for myocardial anastomosis as done in a typical right ventricular outflow tract (RVOT) surgical repair, in the same g

The influence of age on outcomes of patients selected for transcatheter mitral valve repair (TMVR) remains largely unknown in the United States. This study sought to assess the outcomes of TMVR in highly aged patients (≥80 years). We queried the National Readmission Database from January 2014 to December 2016 for elective TMVR hospitalizations. Propensity-score matching was used to compare in-hospital and 30-day outcomes between highly aged patients and those less than 80 years. Of 6,025 (weighted national estimate) hospitalizations for TMVR, total of 3,368 included highly aged patients (mean
